FEATURES OF THE MECHANICAL PROPERTIES OF THE HUMAN FOOT

Perepelkin A.I., Mandrikov V.B., Krayushkin A.I., Atroschenko E.S.

Abstract

The mechanical characteristics of the feet of 175 healthy (without any pathology of musculoskeletal system) young men and 315 young women of the Volgograd state medical university at the age from 17 to 21 years old were observed. The research purpose was to acquire data about the resilient characteristics of the foot under the natural conditions in young persons of both sexes. The examination of the morphofunctional condition of the foot was carried out by means of the computerized plantography, involving the systemic analysis and graphical analytical interpretation of the digital image of the foot. Scanning of the plantar surface of the foot was carried out consistently under compressive loads on each foot which were equal to 50 % and 80 % of the person's body weight. The height of the longitudinal arch of the foot was measured concurrently Resilience coefficient, deformation coefficient, Young's module and Poisson's coefficient were calculated in both groups. The conducted research revealed that the highest deformation coefficient in the persons of both sexes was found along the vertical axis of the foot. The lowest deformation coefficient was revealed along the sagittal axis of the foot. The persons of both sexes had the greatest Poisson's coefficient along the frontal axis of the foot. Poisson's coefficient of the women foot in this axis was higher in comparison with young men. Tensile modulus or Young's modulus of the young men was greater than that of young women. The obtained data of the elasticity of the human foot make it possible to characterize its amortisation function in normal condition and the extent of its loss in various foot deformations. The proposed computerized diagnostic module and the computerized detection of morphofunctional parameters make it possible to carry out an assessment of the human foot elasticity.

OCCLUSAL CHARACTERISTICS OF STUDENTS APPLYING FOR ADMISSION TO MILITARY SCHOOLS

Lordanishvili A.K., Soldatova L.N.

Abstract

We examined dentoalveolar anomalies in 3200 male students (aged 17-25) applying for admission to military schools. We established that 64,3 % of the males had various dentoalveolar anomalies including distal occlusion (22,4 %), mesial occlusion (4,4 %), deep bite (7,1 %), cross bite (6,1 %), open bite (7,8 %). Misaligned and crowded teeth (10,3 %) as well as diastema (6,2 %) were reported to be the most common dental anomalies. The article highlights that 112 (5,4 %) males with any kind of dentoalveolar anomaly are currently receiving orthodontic treatment using an edgewise-based bracket system. The article focuses on the need for a more comprehensive approach to dental examination of the students applying for admission to military schools by medical and physical evaluation boards. It also discusses the problems of providing orthodontic treatment to those already admitted to military schools. We conclude that a more comprehensive approach to dental examination of students applying for admission to military schools will help reduce the number of males with dentoalveolar anomalies.

Beburishvili A.G., Zubina E.N., Popov A.S., Turovets M.I., Vedenin Y.I.

Abstract

To date, the tactics of treatment of patients with pancreatic necrosis has been debated and explored by a number of the leading clinics around the world. One of the unresolved problems is the appropriateness of the thoracic epidural anesthesia. The present cohort study included all patients (n = 493) with clinical pancreatic necrosis, who have presented to the Clinic of Surgery VolgSMU in the period from 1994 to 2013. We proved that the early use of thoracic epidural anesthesia significantly reduced the incidence of foci of infection of pancreatic necrosis and peripancreatic fluid accumulation (from 34,0 % to 11,8 % of the cases (OR 0,26; 95 % CI [0,16-0,42])). Patients of the main group showed a decreased incidence of potentially fatal complications (OR, 0,37; 95 % CI [0,25-0,55]). For example, the use of thoracic epidural anesthesia reduced the risk of multiple organ dysfunction syndrome (from 14,5% to 4.3% of cases), reduced the incidence of gastrointestinal bleeding (from 12.1% to 8.5% of cases) and the risk of acute myocardial infarction (from 5,3 % to 2,8 % follow-up cases). The mortality rate in the intervention group decreased to 8,1 % of cases, as compared to 22,4 % of the cases in the control group (OR 0,30; 95 % CI [0,17-0,54]). The use of thoracic epidural anesthesia significantly increases the effectiveness of treatment of patients with pancreatic necrosis.

ASSESSMENT OF ORAL MICROCIRCULATION DURING THE TREATMENT OF INFLAMMATORY AND DESTRUCTIVE DISEASES OF THE ORAL MUCOSA USING PLATELET-DERIVED AUTOPLASMA

Makedonova Y.A., Poroyskiy S.V., Firsova I.V.

Abstract

Although a lot of methods for diagnosing and treating diseases of the oral mucosa are currently available, one of the most challenging aspects of dentistry is the choice of the most appropriate treatment. There has been a growing interest in the use of laser Doppler flowmetry as a diagnostic means of oral microcirculation disorders. The mainstay of the therapy of inflammatory diseases of the oral mucosa is a local treatment. The other aspect of treatment involves elimination of the causative agents of the disease. In this relation noninvasive methods hold great potential for the treatment of inflammatory diseases of the oral mucosa. We compared microcirculation before treatment and after 14 days of treatment in patients with lichen planus. All the patients were divided into two groups based on the type of treatment that they received. The first group received conventional therapy; the second group received conventional therapy plus platelet-derived autoplasma. Data analysis showed that laser Doppler flowmetry is useful in evaluating microcirculatory changes in patients with oral erosive and ulcerative lesions. Combination of laser Doppler flowmetry and clinical investigations contribute to a better understanding of how much tissue is affected. Autohemotherapy was shown to improve the oral microcirculation in the short-term.

Abstract

We investigated hypoglycemic effects of ZB-16 (0,1 and 1 mg/kg, per os), a novel GPR119 receptor agonist, in rats with developing diabetes. Diabetes was induced by repeated subdiabetogenic doses (30 mg/kg/day) (i. p.) of streptozotocin. In animals with developing diabetes, ZB-16 (1 mg/kg) and sitagliptin significantly reduced the severity of hyperglycemia and improved the impaired glucose tolerance on 14 and 21 days. The therapeutic effects of GPR119 receptor agonist were comparable to those of the DPP-4 inhibitor.
