PREVALENCE, PATHOGENESIS AND PRINCIPLES OF TREATMENT NONALCOHOLIC FATTY LIVER DISEASE

Abstract

The article analyzed current data for the prevalence, risk factors, pathogenesis and principles of treatment of nonalcoholic fatty liver disease (NAFLD). It was concluded that NAFLD has a high prevalence in Russian population. The main risk factors for this disease are obesity, type 2 diabetes mellitus, hyperlipidemia, and metabolic syndrome. Currently, non-drug treatments of NAFLD are low-calorie diet with sufficient quantities of polyunsaturated fats and dietary fiber, graduated exercise for weight reduction. Among the drugs, insulin sensitizers, essential phospholipids, ursodeoxycholic acid and antioxidants take a leading position.

Abstract

The review provides information on the main components and the localization of liver interstitial tissue, as well as the modern understanding of the mechanisms of formation, progression and regression of hepatic fibrosis. The role of stellate cells, hepatocytes and endothelial cells in maintaining the normal composition of the extracellular matrix of the liver is presented, as well as the role of these cells in the mechanisms of fibrosis in pathological conditions. Regulatory substances and mediators involved in the activation and transformation of stellate cells in myofibroblasts synthesizing collagen, which may be potential targets for antifibrotic therapy, are presented. Characteristics of repair processes in acute and chronic liver damage are demonstrated. Modern methods and new directions in the treatment of hepatic fibrosis are analyzed in details. The mechanisms of action and efficacy of standardized silymarin (Legalon) in the prevention and treatment of fibrosis in acute and chronic liver diseases are represented.

Abstract

This article is devoted to one of the most common complications of hepatic cirrhosis - hepatic encephalopathy (HE). The definition of HE, the main issues of the pathogenesis and clinical picture are presented. Methods for clinical laboratory and instrumental diagnostics, including an assessment of their sensitivity with respect to the latent stage of HE, are particularized. The key principles of medical correction and prognostic aspects are discussed. Particular attention is paid to the place and role of lactulose preparations, in particular Duphalac, and their mechanism of action in the treatment of HE. Authors own evidence for high clinical and pharmacoeconomic effectiveness of prolonged use of Duphalac in the treatment of patients with hepatic cirrhosis with PE are presented.

Abstract

As exemplified by pantoprazole (Controloc), the article presents data on the characteristics of the acid inhibitory mechanism of action of modern drugs commonly used for treatment and prevention of symptomatic ulcers in the upper gastrointestinal tract in patients receiving drugs with ulcerogenic action. Data from randomized trials on the intensity and duration of reduction of gastric acid production in patients receiving pantoprazole, the nature and extent of drug-drug interactions and metabolic effects on the cytochrome P450 system are discussed. It is concluded that pantoprazole can be used as first-line drug therapy in the treatment of NSAIDs-induced lesions of the upper gastrointestinal tract.

Abstract

The article analyzed the role of nutritional disorders in the pathogenesis of chronic pancreatitis. It is shown that micro-and macronutrient deficiency is one of the most important factors in the development and severe course of the disease. The results of epidemiological, clinical, biochemical and experimental studies are presented; these results suggest association of malnutrition in chronic pancreatitis with pancreatic carcinogenesis. Some of the controversial aspects of replacement enzyme therapy are discussed.

Abstract

The article considers the indications for antiviral therapy (AVT) of liver cirrhosis against the background of HCV-infection (a combination of pegylated interferon-α and ribavirin). The features of AVT and the difficulties of treatment of such patients, including preparing for a liver transplant operation, are discussed. It is emphasized that patients with compensated liver cirrhosis (Child-Turcotte-Pugh score = 5), particularly with initial signs of portal hypertension - are the only category of this group of patients entitled to AVT in accordance with all the modern guidelines, when AVT is targeted to achieve of sustained virological response.

Abstract

For decades, hereditary pancreatitis (HP) remained a rare disorder of the pancreas, and only genetic studies in the 1990s revealed the nature of inheritance, the type of mutation and classified the NP. It was found that HP is sufficiently broad and inhomogeneous nosological form, because in addition to the previously proposed and established first dominant mutation (cationic trypsinogen gene), many recessive mutations and polymorphisms in different genes predisposing to chronic pancreatitis were discovered. Further clinical studies have revealed features of clinical progression and diagnostic characters of different subtypes of HP with different genetic defects and mode of inheritance. The article presents basic data for the pathophysiology, diagnosis and treatment of HP, and new, previously unpublished in Russian, information.

Abstract

Functional dyspepsia (FD) is widespread chronic disease in the world. It is manifested by abdominal pain or discomfort in the absence of a clear organic cause, which could be determined during the detailed examination. Rome III criteria have presented a definition of FD and versions of its course - postprandial distress syndrome and epigastric pain. Multiple mechanisms, such as delayed gastric emptying, impaired gastric accommodation, increased exposure of hydrochloric acid, visceral hypersensitivity and dysfunction of the central nervous system, are probably involved in the pathogenesis of FD. Although the possibilities of drug therapy of FD are still limited, the experience of use of proton pump inhibitors, prokinetics, tricyclic antidepressants, and a number of alternative methods for the treatment of this disease is accumulated.

The article is dedicated to the issues of pathogenetic treatment depending on the stage and clinical manifestations of cholelithiasis; in particular, the selection criteria for surgical treatment, indications for oral litholytic therapy are presented. In terms of treatment strategies for cholelithiasis, differential diagnosis between somatic and visceral pain have a greatest value. The nature of biliary pain in cholelithiasis may be functional and/or organic. It is emphasized that selective miotropic spasmolytic Duspatalin (mebeverine) is the drug of first choice for relief of biliary pain of functional and organic origins.

APPLICATION OF THIOCTIC ACID IN NONALCOHOLIC STEATOHEPATITIS

Abstract

70% of patients with non-alcoholic steatohepatitis (NASH) experienced insulin-dependent diabetes mellitus, and the remaining 30% of patients experienced glucose intolerance in different periods of the disease. Decrease in the sensitivity of tissue receptors to insulin is a pathogenetic basis of NASH associated with heredity. Thioctic acid prevents the development of fatty liver disease and is a regulator of the number and functionality of membrane receptors, including insulin receptors. Ursodeoxycholic acid, metformin, and thioctic acid (Thiogamma) is well-studied, optimal and safe combination for the integrated pathogenetic treatment of NASH. Participation of thioctic acid in the antioxidant defense allows of conclusion about the advisability of administration of Thiogamma in all patients with elevated body mass index and insulin resistance for the prevention and treatment of hepatic steatosis and NASH.

Abstract

The clinical trial has examined 52 elderly patients with erosive esophagitis treated with esomeprazole 40 mg/day for 8 weeks, and 50 patients with the same disease treated with omeprazole 40 mg/day. After 4 weeks of treatment, recovery was observed in 94.2% of patients treated with esomeprazole and in 70.0% of individuals in the group treated with omeprazole (P<0.001). After 8 weeks of treatment, the there were 96.2 and 80.0%, respectively (P<0.001). The advantage of esomeprazole was especially pronounced in grade C and D esophagitis according to Los Angeles classification. It was concluded that esomeprazole in the treatment of erosive esophagitis in elderly patients has a higher efficiency compared with omeprazole.

Abstract

The issues of evolution of antihelicobacter therapy in recent years are considered. One of the problems in this area is the increasing resistance of Helicobacter pylori to metronidazole - a standard component of eradication therapy. In this regard, the Central Research Institute of Gastroenterology has been developed and tested a scheme of eradication therapy, including proton pump inhibitor, antibiotic clarithromycin, and Enterofuril® (nifuroxazide) as a third component. The article presents the results of study included 120 adult patients with H. pylori infection and various diseases of upper digestive tract treated with 7-day eradication therapy involving pantoprazole 20 mg twice a day, clarithromycin 500 mg 2 times a day, and Enterofuril or metronidazole as a third component. It was shown that the triple therapy of helicobacteriosis including suspension Enterofuril is very effective eradication scheme in adults, with superior to scheme with metronidazole efficacy and tolerability.

Abstract

The study including 165 patients with acute intestinal infections (AII) has shown a high rates of detection of serum markers of toxins of acute intestinal pathogens - Shiga toxin, toxins A and B, Clostridium difficile, as well as enterotoxin Clostridium perfringens (58 -34%) in the circulating immune complexes (CIC); choleriform toxin was rarely detected. In the CIC, levels of toxin markers differed markedly depending on the type of marker, the period of the disease and method of treatment. The frequency and level of detection of toxins markers in the CIC correlated with the severity of symptoms of intoxication, particularly fever and diarrhea. Treatment of acute intestinal infections with ciprofloxacin led to increase of level of Shiga toxin marker in CIC, while marker levels decreased without the use of this drug. The findings demonstrate the importance of evaluation of antitoxic CIC in patients with AII to verify the diagnosis, early diagnosis, and assess the effectiveness of treatment and prognosis.
