APPLICATION OF THE POLYCHROMATIC POLARIZED LIGHT IN CHILDREN'S PHYSICAL THERAPY

Abstract

The analysis of classical and modern sources of literature confirms pathogenetically reasonable possibility of use of polychromatic polarized light in pediatrics. Data of the given results of scientific researches record beneficial influence of the polychromatic polarized light on the clinical course of pyoinflammatory diseases of a skin, on immunologic and reparative processes in the postoperative period of newborns. Changes in skin result in concentration of biologically active agents in a systemic blood flow that promotes realization of therapeutic effects of the polychromatic polarized light at such diseases as atopic dermatitis, bronchitis, bronchial asthma, neurogenic dysfunction of a bladder, traumatic damages of extremities, on a clinic-functional condition of sickly children. In order to optimize therapeutic effects of polarized light special color filters , allowing the device to generate a narrowband portion of the visible spectrum while maintaining the polarization effect, have been developed. As a result of the research was revealed, that courses of a monochromatic polarized light application contributes to the pronounced regression of symptoms of acute respiratory disease, reduce the frequency of recurrent acute respiratory diseases. A significant reduction in the severity of clinical symptoms, improvement of bronchial passability in children with bronchitis, bronchial asthma was found under the influence of a monochromatic polarized light. Biologically positive effects of monochromatic polarized light are reflected in the elimination of itching, soak and elimination of hotbeds of inflammation in children with atopic dermatitis.

DYNAMICS OF NEUROCOGNITIVE STATUS INDICATORS IN CHILDREN WITH TYPE 1 DIABETES MELLITUS ON SANATORIUM STAGE OF REHABILITATION

Abstract

The purpose of this study was to study the dynamics of neurocognitive status in children with type 1 diabetes mellitus and its dynamics on the sanatorium stage of rehabilitation. Studying the initial state of 153 patients with type 1 diabetes mellitus showed the presence of specific complaints, changes of emotional state in the form of increasing levels of anxiety, decline of cognitive function, reduced quality of life. Under the influence of sanatorium-resort treatment positive dynamics of studied indices have been noted in the form of reducing the number of specific complaints, improvement of neuropsychiatric testing and quality of life, more pronounced in children with duration of type 1 diabetes mellitus less than 5 years.

TRANSLINGUAL BRAIN NEUROSTIMULATION IN TREATMENT OF THE PEDIATRIC CEREBRAL PALSY

Abstract

Objectives: to evaluate the efficiency of peripheral nerve neurostimulation for enhancement of the movement rehabilitation in children with cerebral palsy (CP). Device: The electrolingual stimulation was performed by portable neurostimulation device (PoNS). The device delivers electrical impulses through the matrix of 143 flat golden electrodes to the frontal part of the tongue surface. Design: The unblinded pilot controlled trial with a quasi-randomized design. Measures occurred before and after ten days of training. Setting: Sankt Petersburg City Clinic №40. Method: Sixty-five (65) children with CP, who had Gross Motor Function Classification Scores (GMFCS) that ranged from II-IV, age from 3 to 13 years, participated in 10 days of standard physical therapy and movement control training. Active group (n=45) received an additional 20-25 minutes neurostimulation that was performed once a day. The primary outcome measure was scored in three different tests: Ashworth Scale (spasticity), Berg Scale (balance) and GMFCS (general motor control). The secondary measures were the preferred walking speed, step length, lower extremity strength, quality of life. Results: The noninvasive electrolingual neurostimulation, indeed, facilitate and reinforce the efficiency of standard physical therapy. We found statistically significant difference between active and control groups in spasticity of the lower limbs and gross motor functions. The positive changes in quality of life, cognitive functions, and social status were also observed. Conclusions: These preliminary findings support the use of noninvasive electrolingual stimulation for enhancement and facilitation of standard physical therapy for cerebral palsy children. The use of PoNS device can improve the motor control of children with CP.

CORRECTION OF THE NEUROTRANSMITTER MECHANISM OF CIRCADIAN RHYTHM IN CHILDREN WITH AUTISM BY THE METHOD OF THE DOLPHIN ASSISTANT THERAPY

Abstract

More than half of children with autism suffer from sleep disorders. The aim is to study the dynamics of neurotransmitter regulation of circadian rhythm in children with autism in the course of Dolphin assistant therapy (DAT). There were 84 children with autism aged 3 to 13 years. The estimated content in the blood serum and urine neurotransmitters. The source revealed a significant decrease of serotonin, GABA, norepinephrine, dopamine and melatonin. In group I, who received the health research therapy, we observed a statistically significant increase of serotonin (р<0,05) in hard autism, GABA (р<0,01) and night melatonin (р<0,05) in media autism. In group II who received a course of DAT - a significant increase serotonin (р<0,01), GABA (р<0,001) and night melatonin (р<0,01) in hard autism and norepinephrine (р<0,05), day (р<0,05) and night melatonin (р<0,001) in media autism. The inclusion in the rehabilitation course DAT, helps to improve the sleep of children with autism, reducing the desynchronizes.

COMPARATIVE ANALYSIS OF IMMEDIATE RESULTS OF APPLICATIONS OF AEROPHYTOTHERAPY WITH THE USE OF SOME ESSENTIAL OILS OF PLANTS IN CHILDREN IN THE CONDITIONS OF SANATORIUM REHABILITATION TREATMENT

Abstract

The purpose of study: the aim of this work was the comparative analysis of these subjective and objective methods in 74 children with chronic tonsillitis and in 106 children with recurrent bronchitis under the influence of complex sanatorium rehabilitation treatment, including aerophytotherapy with the use of some essential oils of plants. Materials and Methods. In terms of children's health resorts of Evpatoria resort, mainly in spring and autumn the seasons of the year, surveyed 180 children aged 9 to 15 years (boys and girls equally), including 74 with chronic compensated tonsillitis and 106 with recurrent bronchitis. Aerophytotherapy with essential oils of plants (5-6 procedures) was performed in 104 children based on psychological testing and the features of autonomic regulation. Survey methods: assessment of subjective state, peripheral hemodynamics, spectral analysis of heart rate, electrocardiography (ECG), the test SAN, an evaluation of the estimated parameters of the "double works” (DP), adaptive reactions in hematological data and index for immunological resistance. Some of the children before and after rehabilitation therapies have identified indicators of spirography, held psychological test definition of anxiety by the method of CMAS, indicators of fatigue, anxiety and comfort by test TDSFS Doskina. For the objectification of the immediate results of restorative Spa treatment we have proposed a 5-point assessment of the effectiveness of the complexes of sanatorium-resort rehabilitation. Complex restorative treatment included gently-toning sanatorium climatic and motor regime, physical therapy, respectively nosology - massage the neck region or thorax (No. 10), all children were held mud treatment with galvanic current the submandibular lymph nodes or interscapular region (No. 10). 26 children with chronic tonsillitis received a course of sodium chloride baths (10 g/l, 36-37°С, 12-15 minutes, daily, No. 9), 25 children with recurrent bronchitis - salt therapy in the salt chamber in the first mode (No. 10) . In 27 children with chronic tonsillitis and 28 with recurrent bronchitis used lemon essential oil, 21 of the child - essential oil of fir, 28 children with recurrent bronchitis were used E. oil of eucalyptus, which has antiseptic activity. The Results. In children with chronic tonsillitis after applying lemon essential oil improved the performance of autonomic regulation and ECG according to test SAN 12.2% improved well-being; under the influence of the essential oil of fir marked tension regulation at the tissue level and the level of the metabolism and 6.3% improved activity according to the test SAN. Against the average level of functional reserves of the cardiovascular system the most pronounced positive changes of peripheral hemodynamics was observed under the influence of E. oil of lemon. In children with recurrent bronchitis under the influence of essential oil of lemon according veloergometry was a more economical indicator of aerobic provide of the body (largest of maximum oxygen consumption per unit body mass). In children with recurrent bronchitis after treatment, the increased fatigue and irritability moved from the average level in the low gradation level (under the influence of essential oil of lemon and eucalyptus essential oil), but the initial average level of anxiety remained virtually unchanged (in children without conduct of aerotherapy he reached high values moderate level). In children with recurrent bronchitis immediate results of treatment were 3.3% higher when the E. oil of lemon or eucalyptus than without its use. Under the influence of aeropittura in half of the cases noted the high level of reactivity in children with the use of eucalyptus essential oil it was mainly average; the children remained adaptive response activation, mainly of high, characteristic for healthy children. Conclusion. Immediate results of sanatorium treatment was higher when performing basic sanogenetic shown complexes of sanatorium rehabilitation treatment in children with chronic tonsillitis galvanogryazelechenie and sodium chloride baths in children with recurrent bronchitis - galvanogryazelechenie and halotherapy (from the result of the "improvement”). In children with chronic tonsillitis the use of lemon essential oil in the aeropittura in the complex sanatorium treatment contributes to the effect on the regulation mechanisms of the cardiovascular system and improve psychological well-being indicator. In children with recurrent bronchitis treatment there are more economical changes in the indices of respiratory function and aerobic capacity in the application of the aeropittura with lemon essential oil. In children with chronic tonsillitis and recurrent bronchitis complexes of sanatorium-resort treatment of aerotherapy with essential oils of lemon and eucalyptus (with the closest result of sanatorium-resort treatment in the form of ”minor improvement”) can be choices.

THE DIFFERENTIAL APPROACH TO THE USE PELOTHERAPYIN CHILDREN WITH DIFFERENT CLINICAL FORMS OF CHRONIC PYELONEPHRITIS

Abstract

Children with chronic pyelonephritis (CP) had a sanatoriun-resort care with mud electrophoresis rate on the kidney area projection №7. By patients with primary chronic pyelonephritis were normalized urine indicators uIGF-1/uCr, uEGF/uC, uIL-10/uCr and UIL-4/uCr as a result. This indicates the formation of an adequate reparative cells reaction of the renal tubules in the absence of an acute inflammatory process. By patients with secondary obstructive CP proinflammatory indicators UIL-17/ uCr and UIL-12/uCr decreased significantly, but didn’t normalize. uTGF-ß1/uCr number didn’t show any dynamic, remaining significantly high. DDR1 collagen receptor indicators remained strong - this is an evidence of persistent profibrotic focus of immune shifts before and after the spa treatment. ß2-MG serum (except vesicoureteral reflux group) reached the reference value. Urine indicator ß2-MG had a positive trend, but has not been normalized. This indicates a serious tubulointerstitial damage and lack of effectiveness of a course of therapy. In the group with secondary obstructive CP on the background of vesicoureteral reflux took place only a positive trend without significant dynamics and normalization of results. Patients with CP on the background of current dismetabolic nephropathy had normal levels of urinary cytokines uIGF-1/uCr, uIL-17/uCr, uIL-12/uCr and restoration of tubular functions as a result of the treatment. Evidence of this was significant reduction and normalization of the ß2-MG urine level. Thus, the sanatoriun-resort care including mud electrophoresis complex on the kidney area №7 projection is effective by treating patients with primary and secondary dismetabolic variant of chronic pyelonephritis. This treatmeby is not enough effective by obstructive CP, because it does not eliminate the immunological imbalance. Also it is impractical in obstructive CP on the background of the vesicoureteral reflux.

The main stages of the use of physical therapy - prevention, therapy, medical rehabilitation; refined testimony kinds of factors, treatment appointment, methodological features of physiotherapy in relation to children’s gynecology; the sequence of destination and the need to dynamically monitor the girls in the course of treatment; the importance of evaluating the cardiovascular, nervous system, metabolism, psycho-emotional state for an adequate response to treatment. On the basis of research refined the methods of objective individual control over the body’s response to the intensity of physiotherapy effects: according leukogram - the ratio of lymphocytes and segmented neutrophils, lymphocytic index (LI) and nuclear index of neutrophils (NIN), vegetative index Kerdo, EcG and heart rate variability analysis (index of variability), electroencephalography and rheoencephalography, algometric parameters (numerical, verbal and rank, visual-analogue, or a combination thereof the scale, adapted for different ages of children), according to an eight Luscher’s test to assess the psycho-emotional status of patients.

PERSONALIZED MICROWAVE THERAPY IN COMPLEX REHABILITATION ACTIVITIES WITH THE PATHOLOGY OF THE MUSCULOSKELETAL SYSTEM

Abstract

Discusses the author's approaches to the choice of volume, orientation and dose effects when preparing individual rehabilitation programmes for patients with diseases of the musculo-skeletal system based on intensity test of skin sympathetic reactions reflecting the adaptation possibilities of organism. Provides an overview of the experimental work performed in privolzhskiy Federal medical research center to study rehabilitation effects from high to low intensity microwaves in noise radiation mode with different ranges of in vitro and in vivo. When exposed to dermal fibroblasts frequencies 53-78 GGc registered reliable dozozawisimy proliferative effects in healthy cells and optimization of functional activity of stiff altered cells, which allows the use of microwaves in the preparation film coverings and for prevention of formation of hypertrophic scarring in patients with consequences of burns. Found opposite reactions on the part of the hemostatic system when exposed to the blood of healthy donors and burned with different exposure: activates when a 60 second exposure and stabilization with an increase exposure of up to 30 minutes. On the model of experimental ischemia in rats revealed loskute dermal preemptive influence ranges 53-78 and 110170 GGc to various parts of Microcirculatory disorders. In the range 110-170 GGc, containing molecular spectra of O2 and NO more pronounced registered vascular effect range 53-78 GGc showed predominantly anti-inflammatory and antitoksicheskij effects. The results confirm the usefulness of a differentiated implementation method of microwave therapy in rehabilitation of patients with consequences of traumas and diseases of the musculoskeletal system, especially in children with regard to efficiency, minimal, painlessness list of contraindications, side-effects and complications when using low dosages.

THE MAIN QUESTIONS OF BOTULINUM TOXIN A SAFETY IN PEDIATRICIAN NEUROLOGY

Abstract

According to results of multiple researches it was strongly proved that the most efficient method of spasticity control at children with cerebral palsy (CP) is Botulinum toxin A (BTA). However, BTA usage has been limited by hesitations about BTA safety in children among Russian colleagues. Review of a wide range of foreign researches and our own results have given us a strong evidence of BTA safety and efficacy in treatment of children with cerebral palsy. In addition to a high grade of efficacy this could be the strong basis to include BTA on a regular basis as an important part of treatment of children and adolescents with cerebral palsy. That could help with improving of rehabilitative potential of patients with CP

Abstract

The relevance of the development of medical rehabilitation in Russia and the far East showed the importance and necessity of training highly qualified specialists. Discipline was taught at the Department of medical rehabilitation and sports medicine of TSMU students medical, pediatric and dental faculties. Developed educational-methodical complex of the discipline as the integrative, including physiotherapy, balneology, therapeutic physical training, medical control and other non-pharmacological methods of treatment. The first teaching experience has revealed difficulties such as the insufficient number of hours for unit rehabilitation disciplines. In our opinion medical rehabilitation as a clinical discipline must be taught at the undergraduate level, when students had a certain amount of knowledge in the clinical disciplines.

Abstract

Locomotor apparatus injuries and congenital orthopaedic abnormality in children have stable trend to the growth and remain actual medico-social problem. Medical rehabilitation is of great significance due to possibility to prevent some complications provide a more favorable course and outcome. But really rendering of complex medical rehabilitation is considered to be a great problem, caused by insufficiently developed material and technical base of institutions for rehabilitation , deficiency of rehabilitation beds and skilled medical stuff. The work analyzes rehabilitation children care in the Novosibirsk region, as well as the results of the project on rendering of specialized traumatologic and orthopaedic care for children including mechanisms of the state-private partnership. Traumatologic and orthopaedic care for children is conducted as a closed (complete) cycle: specialized reception of children Orhtopaedic Trauma (policlinic)-specialized medical care (including high tech care) (surgery hospital ) - complex medical rehabilitation (ambulance, hospital, in follow-up care in specialized rehabilitation centers, spa treatment) - specialized reception of children Orthopaedic Trauma (policlinic). It allows to provide continuity at the stages of its rendering. The main trends of such care rendering to children are: timely detection-reliable diagnostics and definition indications for treatment -definition of time for treatment-rendering of specialized medical care (including high tech care) rehabilitation - dynamic observation.

PSYCHOLOGICAL REHABILITATION OF STUDENTS WITH FUNCTIONAL INCORRECT POSTURE BASED ON THE METHOD OF BODY-PSYCHOTHERAPY ORIENTROVANNOY

Abstract

The article is devoted to one of the most urgent issues related to the health of students - creating a system of psychological rehabilitation of students with functional disorders of posture. The necessity of the health-friendly environment in educational institutions for students with scoliosis is obvious. According to WHO definitions of "medical rehabilitation" and "psychological" rehabilitation is defined. Rehabilitation is considered as a difficult socio-medical problem, which has several aspects: medical (including psychological), professional (work) and socio-economic aspects. The psychological (psychotherapy) aspect of rehabilitation is defined as the correction of the patient’s mental state (normalization of mental and emotional status, treatment somatogenically), as well as the formation of the rational attitude to the treatment, doctor’s recommendations, the implementation of rehabilitation measures. The necessity of creating a comprehensive program is aimed at the correction of mental conditions in functional disorders of the musculoskeletal system of the students during the training period, in which according to the literature, processes of social and psychological maladjustment are noticed. Proposed and described corrections and methods to carry out psychological rehabilitation are oriented to the needs of a particular student. The described methods and techniques of body-oriented psychotherapy should increase the effectiveness of rehabilitation of students with scoliosis, to create necessary conditions for the recovery of students with functional disorders of posture, improve their mental state, change psychological status, and increase the results of physical and mental performance.
