Morphological markers in skin basal cell carcinoma prognosis

Abstract

The morphological and immunohistochemical study using p53, Ki-67, bcl-2, E-cadherin, CK8, CK19, VEGF, collagen IV, vimentin, desmin, CD4, CD8, CD68, CD31 in 47.skin basal-cell carcinomas (SBCC) was carried out to reveal the diversity between various types of the tumor. The clinical course of the SBCC strongly depends on the topographical kind, proliferative activity and stromal reactions. The prognostic value of these morphological markers is discussed.

Asymmetric dimethylarginine content in plasma of ischemic heart disease patients

Abstract

Asymmetric dimethylarginine (ADMA) is an endogenous inhibitor of nitric oxide synthase. Plasma levels of ADMA are elevated in ischemic heart disease patients and plasma ADMA level was correlated with severity of the disease. Plasma ADMA concentration was higher in man than in woman and elevated with age of patients.

An occurrence of the bacteriophage genes among Streptococcus pyogenes strains of M49 serotype

Abstract

In this study an occurrence of bacteriophage genes among S. pyogenes strains of M49 serotype was studied by two methods, PCR and DNA-DNA hybridization. It was found, that the presence of the phage genes and/or prophages was associated with geographic region of the strain isolation. Potential correlation between the presence of the phage integrase genes and the genes encoding virulence factors was revealed.

Modern ap proaches to diagnostics of arterial hypertensia at children

Abstract

115 children 13-17 years with arterial hypertensia (AH) were surveyed: stable (SAH) 70, labile (LAH) 45. Daily monitoring of arterial pressure, transcranial dopplerographia, triplex ultrasound research of nephritic haemodynamics was executed for all. At AH authentic increase of high-speed indicators in carotid pool and hypokinetic type of a blood-groove in vertebral pool is registered, and given changes are more expressed in group of children from SAH. By the means of klinoortostatik tests decrease in high-speed indicators at level of the basic trunk, segmentary nephritic arteries, and also IR (<0,52) is revealed. This confirms blood-groove arterioviens shuntings mechanism actuation and allows to diagnose nephroangiopatia at early stages AH.

Combination of multiple myeloma and primary myelofibrosis with transformation to acute myeloid leukemia (review and case report)

Abstract

Review and case report of a patient with simultaneously developed lymphoid and myeloid hemoblastosis is presented. Diagnosis of multiple myeloma and primary myelofibrosis are confirmed by complete blood count, myelogramm, trephinebiopsy, immunohistochemical, cytogenetic, biochemical and radiological examination. Complete remission of multiple myeloma was achieved after 7 courses «VAD» and 11-week treatment with recombinant erythropoietin beta with subsequent maintenance therapy course of bortezomib and reaferon. However, primary myelofibrosis was progressed to acute myeloid leukemia. Disease's progression was delayed with 3 therapy courses «5+2». But 3 months later hemorrhagic syndrome appeared and patient died.

Cerebral perfusion and cognitive impairment in patients with vascular dementia

Abstract

In the article results of the own investigation of cerebral perfusion in patients with different forms of vascular dementia are quoted. The investigation embraces the patients with different variants of clinical course of vascular dementia, including it's mixed (vascular-neurodegenerative) forms. Perfusion evaluation has performed by single-photon emission tomography (SPECT) with contrast strengthening. All patients were evaluated by complex neuropsychological scales. Basing on comparison of Cnnical data, neuropsychological measures and cerebral perfusion, the patterns of cognitive disorders in the case of different hypoperfusion zones were marked. The significance of inadequate blood supply even without structural changes in the manifestation of cognitive decline was demonstrated. Single-photon emission computer tomography allows to prognose the cognitive impairment progression in the dynamic evaluation.

Cancer clones evolution in some forms of haemoblastosis

Abstract

The process of cancer clones evolution in haemoblastosis is going in two directions. The first direction comprises development of independent clones each of them accumulating specific injuries not find in other clones. The second direction consists in one initial clone becoming more complex by addition of new injuries. Both directions may be explained by high genetic unstability of the cells. Haematological cancer may have origin in normal stem cells and in normal precursor cells as it was shown for some forms of leucosis.

Surgical interventions on blood vessels in oncological practice

Abstract

Here we have analyzed our experience of using surgical interventions on blood vessels in oncological practice. In the period 2006-2009, surgical interventions on blood vessels were performed in 167 patients with malignant tumors. Out of those, local tumors were diagnosed in 86 patients, including 47 persons with tumors in the hepatoduodenal region and 39 with kidney cancer leading to inferior vena cava thrombosis. 81 patients that were in the remission phase of the oncological disease were operated on for obliterating vascular disease of the lower extremities and abdominal aortic aneurysm. Operations on blood vessels in patients with local tumors enabled us to increase the patients'operability, improve their life quality and life expectancy.

Combinative pelvic disorders at children

Abstract

Обследовано 165 детей 4-15 лет (90 мальчиков, 75 девочек) с сочетанными нарушениями функций тазовых органов (гиперактивный мочевой пузырь и запоры). Для объективизации жалоб пациентов создана специальная таблица, в которой в баллах определяются симптомы нарушений тазовых функций отдельно для мочевого пузыря и толстой кишки, а затем суммируются. В зависимости от суммы баллов определены степени нарушений функций тазовых органов. Выявлена зависимость между степенью нарушения функции толстой кишки и мочевого пузыря, между объемом толстой кишки и мочевого пузыря и баллом расстройств тазовых функций.

Hormonal markers assessment (leptin, ACTH, cortisol, testosteron) after surgical correction of abdominal obesity

Abstract

The treatment results of 181 patients with abdominal obesity were assessed. 109 patients underwent surgical treatment of abdominal obesity - anterior abdominal lipectomy, 72 were treated conservatively. Surgical group of patients with obesity stage 1-2 showed decrease in leptin, cortisol and testosterone concentration in 1 year after surgery, but surgical patients with obesity stage 3 and conservatively treated patients showed no changes in concentrations of studied hormones. There were no changes of ACTH concentration in both groups. Hormonal status normalization assume body mass stabilization and maintenance.

Options of surgical treatment of liver metastases of colorectal cancer

Abstract

We have performed 108 liver resections in patients with colorectal cancer metastases between 2005 and 2010.52 patients (48.14%) had bilobar multiple hepatic metastases. Of those, in 11 cases liver resection was combined with local ablation of metastases using a newly designed device based on local application of ultra high frequency energy. No significant differences in survival rates were revealed between operated patients with monolobar and bilobar hepatic metastases. The median survival of operated patients amounted to 22.8 months. We furthermore propose ways of one-stage and two-stage surgical treatment.

An adequate surgery and intensive therapy of an acute cholangitis: searching of harmony

Abstract

In work the problem sharp cholangities on the basis of experience of inspection and treatment of 358 patients in clinic of faculty surgery of S. P. Fedorov is considered. Medical tactics was based on verification of the basic disease and its complications, and also a complex estimation of a condition of the patient. Surgical tactics assumed strictly well-founded performance of radical operative interventions at 86,4% patients and palliative - at 13,6% that has allowed to lower the general and postoperative mortality to 4,0% and 2,5% accordingly. The presented data provides standardization of diagnostic algorithm and medical tactics.

Statistics of social conditionality of public health

Abstract

In the course of carrying out in territory of the Novgorod region of complex mediko-sociological studying of public health indicators of social conditionality of public health for the first time are developed and approved: a health self-estimation, the valuable relation to the health, knowledge of the population of available diseases, behavioral strategy of the population in health sphere. On the basis of interface of parameters of a selfestimation and self-identification five socially-psychological categories of the population are allocated. It is established that about m of adult population know nothing about the diseases and more % patients do not know about half of diseases and do not receive corresponding treatment. The majority of the population does not have developed valuable relation to health which is not perceived by the person as a necessary vital resource.

Role of prenatal ultrasound in management of monochorionic twin pregnancy

Abstract

The twin-to-twin transfusion syndrome and twin reversed arterial perfusion are a serious complication of monochorionic twin pregnancies. Partly as a result of an inadequate understanding of the pathophysiology of the syndromes, there is a lack of consensus in clinical management. We sought to review the available information on the etiology of both syndromes, to identify parameters that contribute to the severity of the syndromes, and propose a rational management plan based on pathophysiology, clinical presentation and the efficacy of therapies.
