PHARMACOLOGISTICS - A NEW PARADIGM IN CLINICAL PHARMACOLOGY

Abstract

The penetration of business trends in the medical context is sometimes justifiably irritating and frustrating for clinicians. Nevertheless, economic realities demand that the current paradigm of clinical thinking should be supplemented with elements to ensure the rationality of treatment options for patients with various diseases. In these circumstances, the familiar "efficacy-safety" coordinate system is insufficient. The integration of the concept of pharmacologistical paradigm into our medical lexicon is an urgent necessity. We will try to show in a few sentences how reasonable this is, and how it is already present in today's circulation of medications.

CONTRAST-INDUCED NEPHROPATHY: PREVALENCE, DIAGNOSIS, PREVENTION AND TREATMENT

Abstract

Resume. Medical imaging has become an important diagnostic and therapeutic tool in clinical medicine in the era of great technological advances. One of the most frequent and important adverse drug reactions during angiographic studies is contrast-induced nephropathy. Study of epidemiology, risk factors for the development of contrast-induced nephropathy, possible ways of prevention and treatment to reduce the frequency and severity of this complication, improve the quality of patient therapy and reduce the cost of treating complications. Materials and methods. The search for information in the database PubMed, Scopus, Web of Science, e-library, etc. was carried out using the following words and phrases: percutaneous coronary intervention, coronary angiography, contrast-induced nephropathy. Results. The incidence of contrast-induced nephropathy during coronary angiography varies from 2 to 25 %, however, in high-risk groups it can reach 50 %. To prevent the development of contrast-induced nephropathy, there are a number of pharmacological and non-pharmacological approaches. Its prevention is most relevant in high-risk patients. Conclusions. Minimizing the volume of the radiopaque agent, preventing and correcting hypovolemia, preventing the activation of vasoconstriction are the most effective measures to prevent contrast-induced nephropathy.

PROFESSIONAL LONGEVITY - WAYS AND MEANS TO ACHIEVEMENT

Abstract

The purpose of this review of the current literature on the problem of longevity was to analyze approaches and actions in the field of public health to ensure professional longevity. The extension of a person's ability to work is carried out by maintaining individual vitality through the receipt of medical services, as well as compensating for the functional ability that decreases with age by using measures and technologies that facilitate working conditions. This prolongation of professional longevity is based on dynamic monitoring of a person's psychosomatic status and the creation of conditions for professional development. Expansion of the scope of periodic medical examinations, corporate programs for maintaining the ability to work, timely medical and rehabilitation measures, innovative personal protective equipment for the musculoskeletal system are most effective within a holistic methodology. The analysis of the literature shows the feasibility of further research aimed at extending professional longevity by substantiating systemic measures at the state level with the full participation of employers and the broad involvement of the employees themselves, as well as the relevance of developing a methodology for extending professional longevity based on maintaining health and performance, covering career guidance, formation and development professional qualities.

THE JUSTIFICATION OF THE DEVELOPMENT NECESSITY OF A NEW CHEWING TEST

Abstract

The aim of this work is to study the advantages and disadvantages of existing chewing tests and to justify the development necessity of a new chewing test. Dynamic chewing tests can serve as a good diagnostic method to assess the quality of dental treatment. Two main groups of chewing efficiency evaluating methods can be distinguished among the existing chewing tests. These are methods conducting in accordance to the crushing degree of the solid test material and to the mixing degree of the two-color plastic test material. However, existing chewing tests are not widely used among practicing dentists as a diagnostic methods, which is caused by their complexity and duration, the negative properties of the proposed test materials. It explains the need to develop a new chewing test. According to the review of existing chewing tests, an evaluating method based on a color analysis of mixing a two-color plastic test materials currently more promising. In addition, the advantage of these methods is the high prevalence of automated results assessment. However, the automation of this process has not reached the required level for quick use in the dental office. Thus, the development of a new chewing test should be aimed at a more suitable test material selection and a simpler and more advanced automated evaluating method development.

The latest data of numerous studies prove that HU is a significant risk factor for the development of not only obesity, DM2, CVD, but also NAFLD.

THE USE OF TRANSCUTANEOUS SUPRAORBITAL NERVE STIMULATION IN THE TREATMENT OF ASTHENIC SYNDROME AFTER COVID-19

Abstract

Resume. Aim: to assess the efficacy of Transcutaneous Supraorbital Nerve Stimulation (t-SNS) with the Cefaly® Device on the parameters of asthenia and anxiety, as the main components of psychosomatic changes after suffering COVID-19. Materials and methods: an open prospective study in parallel groups was conducted. 60 patients, 30 men and 30 women, average age (61,3 ± 4,1) years, after a coronavirus infection (COVID-19) more than 6 months ago, meeting the following criteria: age from 60 to 75 years old, presence of asthenic syndrome, no contraindications to supraorbital neurostimulation was included. By the envelope method, patients were randomized into two groups. In the Cefaly® group (30 people), patients underwent 10 procedures (3 times a week) of t-SNS with the Cefaly® Device (Cefaly Technology sprl, Herstal, Belgium) in combination with restorative treatment according to temporary methodological guidelines for medical rehabilitation for a new coronavirus infection (COVID-19), in the control group (30 people), patients received only rehabilitation treatment. The results were evaluated using the Multidimensional Fatigue Inventory (MFI-20) and the Hamilton Anxiety Rating Scale (HAM-A) at baseline and after 24 days of therapy. Results: Cefaly® in addition to the standard rehabilitation program effectively reduces the severity of asthenia symptoms -in the Cefaly® group it was 33,6 %, in the control group - 14,4 %. The use of Cefaly® affects all components of the asthenic syndrome (reduced activity, reduced motivation, physical and mental asthenia). The standard rehabilitation program had an effect only on physical asthenia and reduced activity, but not on parameters of reduced motivation and mental asthenia. The reduction in total score from baseline on the HAM-A anxiety scale in the Cefaly® group was 55,16%, in control group -19,74 %. Cefaly® therapy had the greatest effect on insomnia and tension. It should be noted that the standard rehabilitation program has little effect on anxiety scores. However, after treatment, both groups maintained a high mean total asthenia score (MFI-20 - 42,5 points in the Cefaly® group, 63 points in the control group), which once again confirms that post-COVID changes (Long COVID) are long-term in nature even with availability of adequate therapy. Conclusions: The use of transcutaneous supraorbital nerve stimulation can more effectively reduce the severity of symptoms of asthenia and anxiety in people who have recovered from COVID-19.

THE FIRST RESULTS OF THE COMPLEX LASER SURGERY TECHNOLOGY APPLICATION FOR PRIMARY OPEN-ANGLE GLAUCOMA

Abstract

Examination and treatment of 50 patients (50 eyes) with the early stage of primary open angle glaucoma before and after application of complex laser surgery technology was carried out. Selective laser trabeculoplasty in combination with YAG-laser trabeculostomy was performed in the projection of theoutflowmeshwork. Hypotensive effect was 31,1 % of the initial intraocular pressure level on the 1st post-op day and remained for a month after the surgery due to the outflow improvement of the intraocular fluid by 62,5 %. The YAG-laser and OCT data allow optimization of YAG trabeculostomy, providing an alternative noninvasive laser treatment for patients with primary open angle glaucoma.

DENTAL AND ELEMENTAL STATUS OF ADOLESCENTS LIVING ON THE TERRITORY OF YAMAL

Abstract

The lack of information on the relationship between the indicators of micro, macronutrient and dental status of adolescents living in Yamal was the basis for analyzing these indicators. A comprehensive dental examination of 112 adolescents aged 12 and 15 years, from among the indigenous peoples living in the territory of the Yamalo-Nenets Autonomous District, an assessment of the actual nutrition and elemental composition of blood serum was carried out. The analysis of the data showed that the prevalence of dental caries among the examined exceeds 99 %. The CPI index ranged from 4,6-7,7. The imbalance of micro- and macroelements involved in ontogenesis and providing the processes of remineralization of hard tooth tissues was revealed. Correction of the elemental status should be included in the complex of therapeutic and preventive measures that increase caries resistance.

THE CLINICAL SIGNIFICANCE OF EVALUATION ARTERIAL STIFFNESS IN PATIENTS WITH CHRONIC HEART FAILURE AND CHRONIC OBSTRUCTIVE PULMONARY DISEASE

Abstract

The study demonstrates the clinical significance of evaluation arterial stiffness in association between level of oxygen saturation, indicators of respiratory function and left ventricular ejection fraction. An increase in the stiffness of the main arterial arteries and a higher activity of low-intensity chronic inflammation were reliably identified in patients with with chronic heart failure and chronic obstructive pulmonary disease in comparison with patients with isolated chronic heart failure. Statistically significant correlations were revealed between arterial stiffness and the level of oxygen saturation, left ventricular ejection fraction. Multiple regression analysis found a significant contribution of the main factors to arterial stiffness in patients with chronic heart failure and chronic obstructive pulmonary disease was established. These were the following factors - level of oxygen saturation, left ventricular ejection fraction and Tiffno's index.

Abstract

Spontaneous pneumothorax associated with COVID-19-related lung damage can result in a more severe course of the disease with a high probability of lethal outcomes. A multicenter, retrospective study, which involved 1356 patients with COVID-19-induced pneumonia, was conducted. 17 patients (1,25%) developed pneumothorax. COVID-19-associated pneumothorax results in increased length of hospital stay, dictates the need for active surgical management and contributes to a higher mortality rate especially in elderly patients.

PROGNOSTIC SIGNIFICANCE OF COMORBIDITY IN ASSESSING THE DURATION OF INPATIENT TREATMENT IN PATIENTS WITH PERFORATED ULCER

Abstract

Resume. Aim of the study. To assess the effect of perioperative factors on the duration of postoperative inpatient treatment of patients with perforated ulcers in conditions of different comorbidity. Material and methods. A study of the results of surgical treatment with a perforated ulcer was conducted in 194 patients. Group 1 included 57 patients (29 %) with a Charlson - Deyo comorbidity index of 0-2 points, group 2-77 patients (40 %) with a comorbidity index of 3-4 and group 3-60 patients (31 %). The Charlson - Deyo comorbidity index > 4 points. Paired and multiple linear regression were used to predict the duration of postoperative treatment. Results. The duration of the postoperative period in the first group was 9 [8-10] days, in the second group - 8 [7-9] days, in the third group - 10 [7-18] days (p < 0,001). Correlation between the level of comorbidity on the Charlson - Deyo scale straight, high closeness (p = 0,703), statistically significant (p < 0,001). In multivariate models for all levels of comorbidity, the significant regressor of the duration of postoperative inpatient treatment was the duration of surgery and the degree of complications according to Clavien - Dindo. In addition, in group 1, the factors were total blood bilirubin and perforation diameter, in group 3 - age, blood creatinine level and perforation diameter. Conclusion. Taking into account the comorbid status in the stratification of patients makes it possible to predict the duration of inpatient treatment of patients with perforated ulcers.

THE ROLE OF RISK ASSESSMENT IN RESECTION SURGERY FOR LIVER ECHINOCOCCOSIS

Abstract

Resume: Introduction. Echinococcosis of the Liver with subtotal organ damage remains an urgent surgical problem. The development of surgical technologies does not stand still. In the modern scientific community, more and more publications appear that speak of good treatment results with the staged application of various surgical technologies and their combination in various options. The purpose of the study is to present and evaluate the results of treatment of operated patients with liver echinococcosis using a new diagnostic and treatment protocoL for surgicaL treatment. Materials and methods. The results of treatment of 10 patients [4 men (40%), 6 women (60%)] with widespread parasitic lesions of the liver, who underwent 26 surgical interventions for liver echinococcosis in the surgical department No. 2 on the basis of the M.A. Podgorbunsky” in the period from 2017 to 2022. The difference in the number of operations performed and actual patients is explained by the use of a staged method of surgical treatment. Criteria for inclusion in the study group: the absence of an initiaL sufficient safe LeveL of Liver functionaL reserves to perform a one-stage extensive Liver resection based on the results of a comprehensive assessment of clinical, laboratory and instrumental tests in conjunction with the use of a statistical prognostic model. Results. All patients (10/100%) from the study group underwent laparoscopic vascular isolation of the portal blood flow of the right Lobe of the Liver in order to create vicarious hypertrophy of the contralateral Lobe and increase the reserve capacity of the Liver. After objective confirmation of the effectiveness of the above surgical stage of treatment, assessment of the perioperative risk using a statistical model, aLL underwent extensive Liver resections as the next stage. In the postoperative period, 8/80% of patients had post-resection hepatic insufficiency of class A (n = 4) and B (n = 4) according to the ISGLS classification. In two patients, the clinical picture and the absence of Liver dysfunctions (synthetic, secretory, detoxification) made it possible to judge the absence of Liver failure in the postoperative period. Against the background of conservative therapy, the phenomena of Liver failure regressed. ALL patients were discharged in a satisfactory condition. Findings. Staged Laparoscopic vascular isolation of portal bLood flow is effective and safe. The use of a modern diagnostic algorithm for perioperative examination in combination with staged surgical techniques makes it possible to achieve good treatment results in a specialized surgical center.

A RARE OBSERVATION OF SMALL INTESTINAL TUBULAR ADENOCARCINOMA WITH A NEUROENDOCRINE COMPONENT

Abstract

Аbstract. Neoplasia of the small intestine are extremely rare. A special place is occupied by tubular adenocarcinomas of the small intestine with a neuroendocrine component, the intravital diagnosis of which is difficult. Materials and methods. The paper presents a clinical observation of a 61-year-old patient who after resection of the small intestine was found to have tubuLar adenocarcinoma with a neuroendocrine component. Results. Morphoimmunohistochemical examination of the tumor revealed tubular adenocarcinoma with a neuroendocrine component (chromogranin A+, NSE+, aLcian+) with invasion into the subserous base. Conclusions. Tubular adenocarcinoma of the small intestine with a neuroendocrine component is a rare neoplasm of the gastrointestinal tract. In vivo pathoanatomical diagnosis of an intestinal tumor should include morphological, histochemical, immunohistochemical studies with the detection of tumor neuroendocrine cells and assessment of malignant potential.

GENDER FEATURES OF THE AREAS OF ATTACHMENT OF THE ANTERIOR CRUCIATE LIGAMENT TO THE LATERAL FEMORAL CONDYLE FROM THE POSITION OF SURGICAL ANATOMY

Abstract

Injury of the anterior cruciate ligament (ACL) remains the most common cause leading to limited knee function and earlier development of gonarthrosis. The results of ACL repair in male and female patients remain heterogeneous. Women have worse results of surgical treatment, as well as higher risks of ACL injuries (from 1,5 to 10 times) when playing sports. It is assumed that the features of the anatomical structure of the knee play the key role in this joint. We have studied 40 unpaired anatomical preparations of the human knee joint obtained from women and men.The morphometry of the distal femoral epiphysis was performed using a digital sliding caliper according to 16 parameters. When examining the areas of femoral ACL attachment, their shape, size, area, and center distance from individual bone structures were assessed. The data obtained were correlated with the type of structure of the knee joint. Were studied the anatomical features characteristic of the "female" type of structure of the knee joint. It was found that the type of structure of the knee joint affects the topography of the femoral attachment area of the ACL, which must be taken into account in anatomical ligament reconstruction.

INVESTIGATION OF MICROCIRCULATION PECULIARITIES IN THE ZONE OF DIGESTIVE ANASTOMOSIS DURING EARLY AND DELAYED POSTOPERATIVE PERIOD AS A PREDICTOR OF ITS INSUFFICIENCY

Abstract

The aim of the research is to evaluate the condition of blood flow in the area of digestive anastomosis in the early and delayed postoperative period in an experiment. The experiment included 30 sexually mature laboratory rabbits-males of the breed Soviet Chinchilla. The animals were done laparotomy, enterotomy, followed by the imposition of a digestive single-row nodular anastomosis "edge to edge", then the evaluation of local blood flow condition in the zone of digestive anastomosis was performed in the early (3 days) and delayed (7, 14 days) postoperative period using the method of Laser Doppler Flowmetry (LDF). The data received demonstrated a decrease of local blood flow in the early postoperative period in the area of digestive anastomosis, that forms the most vulnerable period for the development of its insufficiency. In the delayed postoperative period, there is an improvement in the indicators of local blood flow in the anastomosis zone, with an approximation of their values to the condition of the intact intestinal tract.

SAFETY ASSESSMENT OF KAPPA-OPIOID ANALGETICS - COMPOUNDS RU-1205

Abstract

The effect of Pharmacological substance (PS) RU-1205 on organs and systems of rats was studied after 3 months of intragastric administration in doses of 5, 50, 500 mg/kg. PS RU-1205 with long-term administration did not have clinically significant effects on the indices of the integral state of animals (dynamics of body weight, consumption of water and food, general condition of animals), as well as on hematological and biochemical parameters of blood. It was shown that PS RU-1205 has a reversible mild inhibitory effect on all studied forms of behavioral activity. Pathomorphological assessment of liver and kidney micropreparations revealed a reversible adverse effect of PS RU-1205 at a dose of 500 mg/kg.

Abstract

This article is the extension of our ecological (correlation) study of some environmental factors on child morbidity (0-14 years old) in Astrakhan region at 2011-2019. Due to Spearman Rank Correlation analysis we identified some classes, types and individual diseases the most sensitive to atmospheric air pollution in the Astrakhan region. There are diseases of the respiratory system, endocrine system, blood, hematopoietic organs and individual disorders associated with the immune mechanism. Summary of the article is devoted to a correlation study of the impact of adverse environmental factors on the incidence of children (0-14 years) in the Astrakhan region and the assessment of the proportion of environmentally related components in the incidence of children's population.

AGE-RELATED ADAPTATION OF PORTAL HEMODYNAMICS IN RATS DURING 75 DAY FOLLOW-UP

Abstract

The age-related adaptation of portal hemodynamics was studied in young rats by ultrasound methods. In the mode of color Doppler mapping, a color signal was visualized in the projection of the portal vein, and in the spectral Doppler mode, the spectrum of blood flow in the portal vein was visualized. The diameter and cross-section of the portal vein, linear and volumetric blood flow rates were determined. The study was performed at the beginning and after 60- and 75-day follow-up. It was found that the portal hemodynamics parameters were influenced by a change in the diameter of the portal vein. With an increase in the diameter of the portal vein, the linear velocity of blood flow in the portal vein decreased, but the volumetric velocity of blood flow in the portal vein increased. Conclusions. The mechanisms of age-related adaptation of portal hemodynamics include compensation for a decrease in the linear velocity of blood flow by increasing the volumetric velocity of blood flow in the portal vein.

ASSESSMENT OF ANXIOLYTIC POTENTIAL OF NEW С2,С3- QUINOXALINE DERIVATIVES

Abstract

The anxiolytic effect of a new series of C2,C3-quinoxaline derivatives was studied using the "Anxious-phobic state" and "Light/Dark chamber" techniques in comparison with diazepam. It was noted that the presence of a phenyl substituent at the C3 position, which did not contain elements with a distinct electronegativity, as well as 2-methyl-(4,5-dimethoxyphenyl)-N-methylethane-1-amine at the C2 position, had a positive effect on the manifestation of the antianxiety activity of new derivatives of C2,C3-quinoxaline. COMPARATIVE ANALYSIS OF THE RESULTS OF ANKYLOGLOSSIA TREATMENT IN CHILDREN

Abstract

A comparative analysis of the results of treatment of children with ankyloglossia was carried out. Patients are divided into three age groups (3-6 years; 7-9 years; 10-12 years) and two subgroups depending on the method (conventional surgical and diode laser method). The immediate (14 days) and long-term (9 months) results of treatment were evaluated. The study showed the advantage of using a laser in assessing immediate results and the absence of significant differences in assessing long-term results. Regardless of the method of treatment, a higher efficiency of frenuloplasty is determined at preschool age.

Abstract

The most dangerous consequences of the global epidemic of diabetes mellitus (DM) are its systemic vascular complications - nephropathy, retinopathy, damage to the main vessels of the heart, brain, peripheral vessels of the lower extremities. These complications are the main cause of disability and mortality of patients with DM. Diagnosis of ophthalmoscopic changes in pathological neovascularization in diabetic patients associated with myopia causes certain difficulties in the work of an ophthalmologist. The use of ophthalmoscopy with a Goldman lens and targeted OCT angiography with an expanded scanning area of 10 * 10 mm allows timely detection of hidden areas of ischemia and neovascularization, as well as to determine the tactics and scope of therapeutic measures in the form of panretinal laser coagulation to stabilize the proliferative process.

Abstract

Resume. The study of the features of the structure of the lower jaw with preserved and with various degrees of loss of dentition is relevant from the point of view of dental implantology. Materials and methods. A craniometric study of angular dimensions was carried out on 120 3D models of the mandibles of men and women of the age groups Maturus and Senilis, divided into 10 subgroups. The degree of influence of tooth loss on angular dimensions was assessed. Results. In groups with complete acquired adentia, there was a statistically significant decrease in the angles of inclination and protrusion of the chin in men and women from 6° to 14° (p < 0,05), while in other groups with partial adentia, no statistically significant differences were observed. The angle of the lower jaw branch and the basal angle did not respond to tooth loss. Conclusions. The anterior segment of the lower jaw body undergoes maximum restructuring, which is reflected in the reaction of angular dimensions to the complete loss of teeth

STUDY OF MORPHOLOGICAL VARIABILITY OF THE LIVER VISCERAL SURFACE RELIEF

Abstract

Resume. Due to the widespread introduction of modern imaging methods in medicine, the study of anatomical variations and congenital anomalies of liver development is of genuine interest to both diagnosticians and clinicians. This article discusses cases of detection of non-standard morphological changes in the human liver revealed during the dissections of organs for the purpose of teaching medical university students. Materials and methods. Using descriptive and comparative research methods, the macromorphological parameters of atypical and additional grooves, fissures, processes and lobes of fixed in 10 % formaldehyde solution liver were studied in detail. Results. Atypical structures of the liver have been identified and described, which complement the overall picture of the morphological variability of the hepatobiliary system, which is undoubtedly relevant in the practice of medical specialists and contributes to improving diagnosis when planning appropriate surgical interventions. Conclusions. A supplemented understanding of the anatomy of the liver, its possible malformations and variants of development, makes it possible to reduce the risk of diagnostic errors, to choose the right tactics for managing patients with hepatic diseases.

MICROBIOLOGICAL AND HISTOLOGICAL PICTURE IN LOCAL CRYO IMPACT ON A SUPPURATING WOUND

Abstract

Resume. In light of the significant perspectives for the use of elements of physical processing factors in the treatment of purulent diseases of soft tissues, spesial attention should be paid of local cryoinfluence in the debridement of purulent wounds. Purpose of the work: to study changes in the histological and microbiological picture during cryo-treatment of a purulent wound model. Material and methods. Superficial purulent wounds of soft tissues were modeled on 40 male Wistar rats. A dynamic microbiological study of wound smears and histological biopsy specimens was performed with cryotherapy and without interfering with the course of the wound process. Results. The treatment with cryotherapy revealed a statistically significant acceleration of the elimination of the pathogen from the wound, the rapid development of fresh connective tissue and the restoration of the microvascular bed, and the activation of the rate of epithelialization. Conclusions. Debridement and the use of local cryoprocessing contributes to the rapid elimination of the pathogen from the wound, activates the processes of granulation and epithelialization. Further clinical study and introduction into practice of local cryotherapy in purulent diseases of soft tissues is substantiated.

Abstract

The review presents an analysis of current literature data demonstrating the pathophysiological mechanisms of the development of cardiovascular complications of chemo- and radiation therapy in cancer patients. The undoubted successes achieved in the treatment of oncological diseases can improve survival and reduce mortality. However, there was a problem of increasing cardiovascular complications during long-term treatment, due to various toxic side effects of chemo- and radiation therapy from the cardiovascular system. For comprehensive prevention and treatment, as well as to improve the prognosis, it is necessary to understand the mechanisms and interactions of drugs underlying the interstitial curation of cancer patients. The purpose of this review is to analyze literature sources illustrating the pathophysiological mechanisms of cardiovascular toxicity in the appointment of chemotherapy and radiation therapy in clinical practice in cancer patients.
