LOCAL CRYOTHERAPY IN TREATMENT OF PURULENT SKIN DISEASES AND SUBCUTANEOUS TISSUE

Abstract

Purulent diseases in surgery are the oldest and at the same time not-over-the-topical problem, accounting for at least a third of all surgical diseases, 12 % of deaths after planned and 27 % after emergency operations. There is a constant search for new methods of treatment. The most promising are methods with the use of physical influence on the wound. One of such, known for a long time, but has become widespread only recently is the use of ultra-low temperatures in wound processing -local cryotherapy It has both advantages for the all physical methods: lack of microorganisms resistant to intensive exposure, locality of action, absence of significant side effects and allergic reactions, and unique features for local cryotherapy -it does not cause severe tissue damage, anesthetic properties, efficacy in chronic processes, ease of use for complex cavities debridment. At the same time, the local cryotherapy is largely unexplored topic, requiring further study and adaptation of the technique for the purulent surgery requirements.

SURGICAL TREATMENT OF THE ATONIC LOWER EYELID ECTROPION

Abstract

An atonic reversal of the lower eyelid is of an involutionary character or occurs when the innervation of the facial nerve. When it occurs, in addition to a pronounced cosmetic defect, there is a threat of serious complications from the eyeball. The variants of treatment of atony seamy side of lower century are considered in a scientific review. An effective method of treatment is surgical correction. Various methods of surgical correction of atonic reversal of the lower eyelid were used, but there is still no single universal, optimal method. One of variants applying on universality is hanging of lower century. When the lower eyelid is suspended with synthetic materials, a long-term stable result is created. With the purpose of reduction of amount of complications the modified reticulated implant was used. Positive data of his application are got. His further clinical study is needed.

At present days the great attention is paid to studying of tuberculosis with multidrug resistance (MDR), when mycobacteria tuberculosis (MBT) has resistance to isoniazid and rifampicin. High level of MDR-tuberculosis has essential influence on spreading of tuberculosis by means of accumulation of sources of an infection due to low efficiency of treatment. The MDR-tuberculosis has increased in 5.9 times at the last 14 years.

Abstract

Based on the information obtained in the anatomical study, were developed the criteria for selecting the allograft, taking into account its dimensional and strength characteristics. Based on these criteria, the most suitable tednon graft for plasty was chosen, and its mechanical strength was examined on a tensile machine. Various species of tendons, sterilized in the liquid medium of Belyakov, were investigated. The strength properties of the allograft were compared with the similar characteristics of native PCL obtained at the same stage of the study It was revealed that the optimal length and strength characteristics are the tendon of the long fibular muscle, which allows recommending it for alloplasty of the posterior cruciate ligament.

Abstract

Burkholderia pseudomallei and Burkholderia mallei are the causative agents of melioidosis and glanders, dangerous infectious diseases. These microorganisms are not endemic for the central zone of the Russian Federation, but the possibility of an imported case or deliberate use as a biological weapon is not ruled out. In practical work for accelerated detection and subsequent identification of pathogenic microorganisms, the method of orienting reaction of slide-agglutination is most often used as the simplest, requiring no additional equipment and easy enough to perform. Since in Russia standard (production) diagnostic species-specific sera are not produced for any Burkholderia species, experimental series of preparations are used in the conditions of specialized laboratories for setting diagnostic immunological reactions. In the present work we investigated four types of experimental rabbit immune serum using strains of pathogens melioidosis and glanders collection of Volgograd Research Plague Control Institute. The most active was serum obtained against live B. pseudomallei VPA cells, the least is serum against B. pseudomallei C-141; with complex serum against cells of 5 strains of Burkholderia thailandensis and with serum against extracellular antigen B. thailandensis 264 agglutination was observed only at low dilutions.

PECULIARITIES OF FINGER INDICES OF HAND 1D: 3D AND 2D: 4D IN DEPENDENCE ON GENDER AND TYPE OF THE CONSTITUTION

Abstract

1 D:3D and 2D:4D finger ratios of a hand are widely used for research in the field of predisposition of the individual to certain somatic diseases. 299 young people were surveyed in total with the calculation of the Pignet index, using a flatbed scanner and the author's program HandScaner. As part of the study, it was identified that the finger indices of 1D:3D in young men is statistically significantly greater than that of girls in the hypersthenic group by 2 % (p < 0,05). Finger index of 1D:3D is more by 1 % in young men in the normostenic group, in the asthenic group, the finger index of 1D:3D is more in girls by 1 % (p > 0,05). When studying the index 2D:4D, there were no statistically significant differences between the left and right hands in young men and women (p > 0,05), as well as the relationship with the type of body build. Despite this, many foreign authors note the presence of this connection in males and females in other age groups. In addition, according to foreign authors, this index, as well as in our study is more in girls in comparison with young men.

Work on elimination of the available inflammatory phenomena of a parodont and acceleration of an engraftment of implants of PEEK Optima by means of introduction of a platelet autoplazma is carried out. Using a technique of Plasmolifting, was succeeded to achieve positive results (decrease in inflammatory processes, elimination of bleeding, acquisition by a gum of physiological coloring, acceleration of an engraftment of implants, decrease in sensitivity to infections, a complete recovery of function, esthetic quality).

Abstract

The important goal of tissue engineering is the search for modifying additives of polysaccharide gels of vegetable origin capable of controlling the biocompatibility and integration of the hydrogel matrix in the muscle fiber and the rate of biodegradation of the matrix. The aim of the study was to study the effect of D-asparagine on the biocompatibility of the modified alginate hydrogel with rat's skeletal muscle tissue. In the during of the study were 2 groups of animals: group № 1 (comparison) -14 rats, with 0.5 ml of modified alginate hydrogel without D-asparagine injected into the muscle of thigh; group №2 (experimental) -14 rats, in which 0.5 ml of modified alginate hydrogel with D-asparagine was injected into the muscle of thigh. According to the histological examination, in the muscle with the modified alginate hydrogel introduced on the 30th day, a large-celled matrix is formed, the walls of its cells are populated with cells. The addition of D-asparagine to the hydrogel formulation inhibits the formation of a fibrous capsule around the implant in the muscle, and also increases the number of vessels and cells that populate it.

The stability of the refractive result after excimer laser correction of myopia was determined by the preservation of corneoscleral rigidity which depends on the initial values of intraocular pressure, the size of the anteroposterior segment of the eye and the thickness of the cornea in the central optical zone.

Abstract

The dorsal hippocampus (DH) is responsible for certain forms of learning and memory, and the ventral hippocampus (VH) plays an important role in the realization of stress reactions and anxious forms of behavior. The study was performed on 20 white rats at the age of 12 months. Modeling of combined stress (pulsating bright light, loud sound, vibration) was conducted daily for 30 minutes for 7 days in a special chamber consisting of 6 isolated compartments. In stressed animals (n = 10), in the DH and VH, a decrease the area of the neuronal perikarya (ANP), an increase in the relative number of shrunken neurons with hyperchromatosis of the cytoplasm, an increase in the relative area of GFAP immunoreactive material was observed. In addition, the decrease in relative ANP, width of the pyramidal layer, and the increase in the relative amount of caspase-3-immunopositive neuronal perikarya were observed in DH. With prolonged combined stress in rats at the age of 12 months, the most pronounced changes in the pyramidal layer in the DPH are found in CA3, and in the VH - in CA1.

A comparative evaluation of the variability of the morphometric parameters of the width of the dental arches during the mesocrane skull type is made. The work was performed on 144 preparations of skulls of people of both sexes of mature age with physiological occlusion of teeth. As a result of the study, the reliability of the difference in the average statistical indices of the vestibular and palatal dental arches in men and women at the level of canines and the first molars was revealed.

Abstract

Red flat lichen in the oral cavity is characterized by a recurrent, persistent current. Erosions and ulcers in the oral cavity are very difficult to treat, do not heal for a long time, causing strong painful sensations. Despite a large selection of drugs in gerontostomatology the acceleration of the reparative regeneration of the damaged mucosa is an actual problem. In this study, 60 patients with erosive ulcerous form were examined and treated according to the treatment method. They were randomly assigned to the same treatment group: Group I - Celestodera applications were used in complex treatment: Solcoseryl, in group II - combined treatment (Tisol applications with L-arginine in combination with injections of platelet autoplasma). In both cases positive dynamics was noted, however, the nature and timing of epithelialization was different.

Abstract

A comprehensive examination of 75 patients with chronic generalized periodontitis against a metabolic syndrome (MS) was conducted. Determined the features of vegetative regulation from the data of heart rate variability. General immunity indices in peripheral venous blood were assessed by the number of T and B lymphocytes, IgA, IgG, IgM in peripheral venous blood (PVC). In the course of immunological examination of patients with chronic generalized periodontitis of moderate severity against the background of MS, pronounced changes in the parameters of general immunity were established, consisting of a decrease in the number of T-lymphocytes in the blood, an increase in B-lymphocytes in the blood, and an increase in IgG, IgA, IgM in peripheral venous blood. The activity of the sympathetic department of the autonomic nervous system in patients with CHC on the background of MS has been found to increase.

COMPREHENSIVE ANALYSIS OF CONSUMPTION ANTIBIOTICS IN MULTI PROFILE HOSPITAL REPUBLIC OF TAJIKISTAN

Abstract

The analysis of consumption of antimicrobial drugs of different classes in a multidisciplinary hospital for 5 years, showed that during this period is almost completely changed approaches to starting empirical antibiotic therapy of diseases, with a clear tendency to reduce the consumption of antimicrobial drugs penicillin and the transition to monotherapy cephalosporins 3rd generation (ceftriaxone ) and partly fluoroquinolones. Comparative analysis of the financial costs and consumption of antimicrobial drugs for the period showed that the increase in the cost of the ILA cephalosporin accompanied by a corresponding increase in the consumption of drugs of this group. However, the discrepancy of cash costs of antimicrobials drugs of penicillin group, which came into the class B and in their level of consumption. Monitoring consumption of antimicrobials drugs in hospital wards will allow to take strategic decisions to optimize antibiotic therapy, align the number and range of antibiotics applied to the profile of the hospital departments, as well as the planning of the distribution of costs under «Medicines».

Abstract

Clinical and immunological features of the juvenile idiopathic arthritis in children and adolescents have been studied. It was revealed that most often juvenile idiopathic arthritis was found in females. In articular syndrome the polyarthritis predominated with knee and ankle involvement was prominent. The pain syndrome in the articular apparatus was more intense in girls. The most frequent radiologic sign was a moderately expressed epiphyseal osteoporosis. The inherent sign of juvenile idiopathic arthritis was a persistent increase in the level of interleukin.

The article presents an analysis of the dynamics of the indicator of the critical frequency of the fusion of fuscene flashes on the background of the treatment of glaucomatous opticopathy by a combined method involving the electrooscillation of the optic nerve with the help of the «ESOM» apparatus and the use of the intranasal nootropic Semax 0.1 %. The high efficiency of this method is shown.

Abstract

The article presents clinical case report of treatment of a severe anaerobic, nonclostridialphlegmon of the abdominal wall as a complication after a typically performed appendectomy without draining the abdominal cavity is given. A feature of the treatment in this clinical case is the use of cryodebridement with liquid nitrogen on the purulent cavity of the wound, which predetermined the rapid subsidence of the symptoms of purulent inflammation and created a condition for early and complete closure of the wound by secondary sutures. This case is one of the examples of effective application of local cryoexposure in the direction of purulent surgery of the skin and subcutaneous tissue, which is still poorly understood for this method.

Abstract

The goal of the research is to ensure a high level of practical training of surgeons in the course of thematic advanced training in laparoscopic surgery with the use of a virtual simulator. The developed complex of pedagogical tools allows to assess the level of learning and skills versatile, quickly (in 35 minutes) and objectively, and the questionnaire allows to make justified adjustments to the training program depending on the individual characteristics of the students multifaceted and without significant expenditure of time. Application of the complex allows to accumulate an array of information flows learning, as a foundation for developing new pedagogical technologies.
