PSYCHOLOGICAL ASPECTS OF DEVELOPMENT OF THE PROBLEM - BASED LEARNING APPROACH AS A NEW TECHNOLOGY IN THE MEDICAL EDUCATION

Vinokur V.A., Boluchevskaya V.V.

Abstract

Nowadays the necessity of implementation of new learning technologies in medical education is considered more and more actively. These technologies allow to move from the teacher-centered approach, which is based on the directive style of learning programs mainly because they are overloaded by lectures, to the student-centered approach which suggests more active educational technologies, such as problem-based learning. The principal ground of education in medicine is aimed mainly at receiving a new information but not at development of practical skills so the compliance between the educational and professional activities, knowledge and skills becomes the essential divergence instead. The experience obtained in different countries allows consider the problem-based learning as the most effective educational technology. It's main principle is a self-directed learning via active group work and supportive, stimulative and facilitative activity of a teacher. In these circumstances the acceptance of new knowledge becomes easier the students this process has internal stimuli while the proper group process supports the carrying out this motive. Key words of this approach are “find, create, master”. These principles are fulfilled in the creation of skills which are necessary for the successful practice. Among the skills to be developed here are hard - skills and especially soft - skills, related to a creativity, competence in communication, cooperation in a team, cognitive flexibility, the ability to cope the emotional arousal in the work etc. The effectiveness of a problem-based learning helps to solve the tasks, related to the medical staff education and training and could be essentially increased while using the new information technologies. The active learning makes the process of acceptance and mastering of knowledge and skills more individually based. The paper presents the reasons for the advanced implementation of the problem-oriented learning into the structure of the postgraduate medical studies. The psychological aspects of this process, methods and stages are described. The role of a teacher in this process and the core aspects of the carrying out of the problem-oriented learning are analyzed.

STRUCTURAL PROTEINS OF THE SARS-COV-2 CORONAVIRUS: ROLE, IMMUNOGENICITY, SUPERANTIGENIC PROPERTIES AND POTENTIAL USE FOR THERAPEUTIC PURPOSES

Bruyakin S.D., Makarevich D.A.

Abstract

In early December 2019, the first cases of pneumonia of unknown origin were detected in Wuhan City, Hubei Province, China. High throughput sequencing has identified a novel beta coronavirus, currently called the 2019 novel coronavirus (SARS-CoV-2). Hyperinflammation in coronavirus infection (COVID-19) cause cytopenia, coagulopathy, tissue damage, liver dysfunction and macrophage activation, and also has signs of reactive hemophagocytic lymphohistiocytosis. Overproduction of inflammatory cytokines can cause organ dysfunction and ultimately death. These symptoms and associated laboratory parameters strongly resemble toxic shock syndrome, an increase in the cytotoxic adaptive immune response, when pathogenic superantigens bind to MHCII molecules and T-cell receptors (TCR). The article examines structural proteins of the coronavirus, which are involved in the development of such a response of the human immune system, discloses the molecular organization and antigenic properties of SARS-CoV-2 proteins. The role of structural molecules in the pathogenesis of coronavirus infection, as well as the possibility of their use as components of vaccines and promising ligands for efferent methods of treating this pathology, is described.

TYPE I EPITHELIAL-MESENCHYMAL TRANSITION AS AN IMPORTANT BIOLOGICAL PROCESS IN EMBRYOGENESIS

Dvoryashina I.A., Velikorodnaya Y.I., Terentev A.V., Zagrebin V.L.

Abstract

Epithelial-mesenchymal (EMT) and mesenchymal-epithelial (MEP) transitions play a significant role in the processes of embryogenesis, organogenesis and cell differentiation. The epithelial-mesenchymal transition also promotes the regeneration of damaged tissues, but in the case of its aberrant activation, it can initiate and aggravate the course of pathological processes such as fibrosis, metastasis, and carcinogenesis. In this review, we described the history of the study of these processes in the context of embryonic development, and also discussed the problems of terminology and different views on the concept of the determinism of the pathway of mature cells.

ASSESSMENT OF THE PHYSICAL DEVELOPMENT OF SCHOOLCHILDREN AGED 9-10 YEAR-OLD LIVING IN THE ALTAI REPUBLIC

Bauer P.S., Borodina G.N., Trebushinina T.G., Fedina I.Y.

Abstract

The article presents the results of the medical examination of schoolchildren aged 9-10 years living in the Altai Republic. Anthropometric measurements assessing the level of schoolchildren physical development were carried out. The study revealed a low level of schoolchildren physical development in the presented age group, underweight in children of both sexes being more prominent in females. At the beginning of the academic year, children were characterized by an asthenic body type. At the end of the year, the parameters characterizing the body type were within the recommended values. The causes of the low level of physical development may be the special characteristics of the climate and geographical zone where the studied children live, the diet and the lack of physical activity which is necessary for the normal body development.

INFLUENCE OF GROWTH FACTORS ON SPERMATOGENESIS AFTER AIMED IRRADIATION OF RAT TESTES WITH ELECTRONS IN A DOSE OF 8 GRAY

Demyashkin G.A.

Abstract

The morphological assessment of spermatogenesis using platelet-enriched plasma after electron irradiation at a dose of 8 Gray was studied. A week after irradiation, a sharp decrease in the number of germ cells was found; decrease in the area of the testis and the height of the spermatogenic epithelium; the appearance of seed balls. Positive effects after the application of LP-PRP + IGF were observed already in the third week, which persisted throughout the entire experiment. The growth factors of platelet-rich plasma in combination with IGF promote the activation of poorly differentiated spermatogonia and the restoration of the pool of germ cells after irradiation with the NOVAC-11 pulsed electron accelerator at a dose of 8 Gray.

PREVENTION OF INCISIONAL VENTRAL HERNIAS AFTER MIDLINE LAPAROTOMY: CURRENT STATE OF THE ART

Vanzha Y.E., Vertyankin S.V., Yakubenko V.V., Samygin S.A., Vertyankina D.S.

Abstract

Despite an active search and an abundance of scattered recommendations for the prevention of incisional ventral hernias, there is still no consensus on many issues. Discussions continue about the use of a continuous or interrupted suture of the aponeurosis, the prophylactic use of a mesh endoprosthesis during the closure of abdominal wall incisions. The generally accepted methods of prevention include the use of monofilament non-absorbable synthetic materials when suturing the aponeurosis using the "small byte" technique. The first experience with a new prevention method, such as laparotomy with umbilical dissection, shows good results and requires further study.

DEMOGRAPHIC IN DICATORS OF POPULATION AND DYNAMICS OF THE ONCOLOGICAL DISEASE MORBIDITY OF THE CHILD POPULATION OF THE RUSSIAN FEDERATION IN THE PERIOD OF 2013-2018

Novikova E.A., Kudentsova G.V.

Abstract

The demographic situation in Russia remains rather complicated at present. National programs that are developed and adopted by the government are aimed at solving demographic problems. An increase in the birth rate, a decrease in mortality, and an increase in natural population growth are priority components of this situation. According to the Federal Statistics Service, as of January 1, 2020, the country's population is 146 745 098 people, and Russia occupies only 9th place in the world ranking. The population is the state's renewable resource, which depends on many demographic indicators, one of which is the mortality and birth rates. Oncological diseases are one of the causes of population mortality, the mortality rate from which is about 20 %. The problem is important and occupies a major place in the incidence of oncological diseases in the child population.

CLINICAL CASE OF THREE-WAVE HYPEREOSINOPHILIA IN A BOY IN THE DYNAMICS OF OBSERVATION

Zriachkin N.I., Buchkova T.N., Kushnereva T.T.

Abstract

The wide spread of parasitic infestations determines the relevance of the problem. Parasitic hypereosinophilia accounts for 17-25 % of large eosinophilia. The peculiarity of the presented clinical case is three waves of hypereosinophilia. The first hypereosinophilia was detected at the age of 8 years against the background of a massive mixed invasion (ascaridosis and enterobiosis), and antibodies to ascarids and helminth eggs were not detected in the feces. Tangle ascarids and pinworms departed after taking nemasole. The second wave of hypereosinophilia was diagnosed at the age of 10 years against the background of opisthorchiasis (IgG - 1:400 antibody titer). The third wave of hypereosinophilia was established at the age of 11 years and 6 months against the background of toxocarosis and Giardia (antibodies to toxocars and Giardia in the diagnostic titer in August 2018) and hypereosinophilia persisted until December 26, 2019.

EVALUATION OF PARAMETERS FOR THE HEART RATE VARIABILITY AND POSTURAL DISORDERS IN PATIENTS WITH DISTAL AND PHYSIOLOGICAL OCCLUSION

Zangieva A.S., Sorokina N.D., Pertsov S.S., Gioeva Y.A., Demyanenko M.V., Selitsky G.V.

Abstract

Our study revealed the correlations between parameters that reflect changes in the systemic regulation of physiological functions during abnormal occlusion accompanied by disturbances in postural control. Estimating the heart rate variability, studying the posture (by means of computer optical topography), and performing the tests for activity of the autonomic nervous system showed that a normal functional state is characterized by coordinated activity of the postural control apparatus, occlusion of dentition, and other physiological systems. By contrast, the predominance of sympathetic autonomic regulation and anomaly of occlusion in the form of distal occlusion are is accompanied by the certain postural disorders (scoliosis of the 1st - 2nd degree in the frontal plane). Orthodontic treatment was followed by a decrease in activity of the sympathetic nervous system for heart rate regulation, which illustrates an increase in the adaptability of cardiovascular functions. These data are promising for an improvement of instrumental procedures for functional diagnostics in physiology and dentistry.

COMPARISON OF THE NEURO PSYCHOTROPIC PROPERTIES OF THE (R)-ISOMER AND THE RACEMIC FORM OF THE TOLIBUT

Kovalev N.S., Bakulin D.A., Kurkin D.V., Pustynnikov V.E., Reznikov A.N., Klimochkin Y.N.

Abstract

The use of biologically active enantiomers instead of racemic forms is currently a promising approach to improving the properties of known drugs. In such derivatives of GABA as phenibut and baclofen, the pharmacological activity is determined by the (R)-isomer. The potential for the possible clinical use of tolibut is limited by a narrow spectrum of therapeutic action. One of the ways to expand it may be the approach of isolating and studying the active enantiomer. The sedative, anxiolytic, nootropic, antihypoxic effects, as well as the toxicity of the (R)-isomer and racemate of tolibut were evaluated. The performed study showed that the (R)-isomer of tolibut has a more pronounced muscle relaxant, antihypoxic effect, exhibits less pronounced anxiolytic activity and less toxicity compared to racemate.

PECULIARITIES OF THYROID MORPHOLOGY IN CEREBRAL HYPOPERFUSION IN THE COMPLEX WITH SHORT-TERM EXERCISE IN RATS WITH DIFFERENT RESULTS IN THE MORRIS LABYRINTH

Krishtop V.V., Rumyantseva T.A., Nikonorova V.G.

Abstract

The aim of the study was to assess the dynamics of structural changes in the follicular apparatus and the hemomicrocirculatory bed of the thyroid gland in rats with cerebral hypoperfusion in rats with different levels of learning ability under prolonged exposure to physical exertion. Materials and methods: using classical histological stains, the state of the follicular and interfollicular epithelium, as well as perifollicular hemocapillaries in Wistar rats after bilateral ligation of the common carotid arteries was investigated, separately for animals with high and low test rates in the Morris water maze. The thyroid gland was studied 1, 6, 8, 8, 14, 28, 35, 60 and 90 days after surgery (comparison group), as well as when it was combined with 15 minutes of physical activity (experimental group). Results and discussion: changes in isolated cerebral hypoperfusion are characterized by periodicity - when a short-term increase in organ activity is replaced by its decrease. Also, results were obtained on a greater amplitude of changes in the studied parameters and a later growth of signs of organ activity in a subgroup with a high level of cognitive abilities. A single physical activity leads to a greater increase in the activity of the thyroid gland during cerebral hypoperfusion, which may have a positive value, based on the ability of its hormones to stimulate the neuroprotective mechanisms of astrocytes and shift the vector of differentiation of progenitor cells towards neurons. Conclusions: physical activity against the background of cerebral hypoperfusion leads to a restart of the adaptive-compensatory syndrome, which, in turn, is accompanied by an increase in the activity of the thyroid epithelium in the subgroup with a high level of cognitive abilities after 3 weeks, associated with a decrease in mortality.

INDICATORS OF AXIAL SECTION AREA AND SPLEEN VOLUME IN CHILDREN AND ADOLESCENTS ACCORDING TO INTRAVITAL IMAGING

Chemezov S.V., Lozinskiy A.S.

Abstract

The aim of the study was to establish age and gender patterns of axial section areas and spleen volume in children and adolescents according to intravital imaging data. The computed tomograms of the abdominal cavity of 75 children and adolescents without visible pathology of the abdominal organs were analyzed. The subjects were divided into 4 age groups: early childhood, first childhood, second childhood, and adolescence. The areas at the level of the middle of the vertebral bodies from Thxi to Liii, the vertical dimensions and volumes (the product of the vertical size and the mean sectional area) of the spleen were calculated. The obtained data were subjected to variational statistical processing with the determination of the mean value (M), standard error (m) and the significance of differences according to the Student's t-test. A significant increase in the areas of axial sections was established among children of the period of the first childhood - (18,4 ± 1,7) cm2, compared with children of early childhood - (11,5 ± 1,2) cm2, and adolescents - (35,1 ± 2,7) cm2, in comparison with children of the second childhood - (23,1 ± 1,7) cm2. This trend is mainly due to differences in indicators among boys. Gender differences in the areas of axial sections were noted only between the indicators of boys and girls in early childhood at the ThXii level. Significant differences in the indicator of the volume of the spleen were established in the surveyed first childhood compared with children of early childhood and adolescents compared with children of the second childhood. Among girls, the volume of the spleen was significantly higher among those surveyed in the second childhood compared to those surveyed in the first childhood and among adolescents compared to those surveyed in the second childhood. Among boys, the studied indicator was significantly higher among children of the period of the first childhood compared with children of the period of early childhood and in the adolescent group compared with the group of the period of the second childhood. The spleen volume indicator was significantly higher among boys, in comparison with girls only among children of the first childhood.

СHARACTERISTICS OF THE EXPRESSION OF ENDOTHELIAL NO-SYNTHASE IN THE DENTATE GYRUS OF RATS WHEN MODELING A DEPRESSIVE STATE AND THE POSSIBILITY OF PHARMACOLOGICAL CORRECTION WITH PHENIBUT

Smirnov A.V., Ekova M.R., Tyurenkov I.N., Volotova E.V.

Abstract

Depression is one of the leading causes of declining working capacity in the population and remains one of the most common mental disorders in the world in all age groups. The features of the expression of endothelial NO-synthase in the dentate gyrus in mature rats were assessed when simulating a depressive state, as well as the possibility of pharmacological correction of the revealed changes with phenibut. Evaluation of changes in the level of expression of immunoreactive material in the dentate gyrus was determined in the granular and subgranular layers by assessing the intensity of staining in points from 0 to 3. In rats with an experimental depression, compared with the control group, there was a decrease in the degree of expression of immunoreactive material in the dentate gyrus. The use of phenibut contributed to the correction of the studied signs.

Abstract

Acute hematogenous osteomyelitis (AHO) is a severe disease of childhood and leads to disability in 70-87 % of cases. Despite the achievements of modern science, research methods are not always able to diagnose CSO in children in time. The number of scientific works devoted to the early diagnosis of CSO in children based on the use of morphological and functional characteristics of bone tissue is limited. The material for the study was a biopsy of bone tissue and red bone marrow obtained after osteoperforation in children with CSO. After preliminary processing of the biopsy, histological and immuno-histochemical studies were performed, followed by an assessment of the prognostic value of the data obtained. The study revealed histological changes in bone tissue characteristic of the intramedullary and extramedullary phases of CSO, IHC indicators of an increase in the population of cytotoxic T-lymphocytes in the red bone marrow, indicating the development of a severe inflammatory process.

Abstract

In hypertension, the administration of several single-component antihypertensive drugs from different groups increases the choice of pharmacotherapy options. Conducted a quantitative assessment of these options at various stages of intensification of antihypertensive therapy. The international nonproprietary name, trade name and dosage of the drugs were taken into account. The methods of combinatorics were used for the calculation. At each stage of the intensification, the number of possible combinations of drugs increased significantly, which could eventually reach 1 834 555 464. The obtained results revealed the problem of choosing rational antihypertensive therapy during its intensification, which is the justification for the development of a specialized decision support system.

PHARMACOVIGILANCE IN YAROSLAVL REGION ON THE EXAMPLE OF ANTIBACTERIAL DRUGS

Mayevskaya V.A., Gorelov K.V., Korozina N.S., Stromova A.S.

Abstract

Good pharmacovigilance practices have been introduced for continuous and updated exchange of information on drug safety. The pharmacovigilance system provides continuous monitoring of medicines, which can reduce morbidity, mortality and costs to health care providers. The purpose of this study was to analyze the structure and information of spontaneous messages received by the Pharmacovigilance Center in the Yaroslavl Region in 2018-2020. The work was divided into 2 stages: collection of information and analytical stage. According to the Federal Service for Surveillance in Healthcare, today antibacterial drugs for systemic use occupy a leading position in the Pharmacovigilance database, which is also confirmed by the data of our study. Antibiotic use has been reported in more than 70 % of COVID-19 cases. Inappropriate antibiotic treatment for COVID-19 patients can lead to preventable complications, including increased bacterial resistance.

EFFECT OF CHRONIC STRESS ON PRIMARY AND SECONDARY LYMPHOID O RGANS OF RATS AND ITS REVERSIBILITY DEPENDING ON THE DURATION OF STRESS EXPOSURE

Muraeva N.A., Smirnova T.S., Fedorova O.V., Tkachenko Y.A., Vondrachek L.V.

Abstract

The influence of chronic stress on the lymphoid organs and its reversibility depending on the duration of exposure was studied. Male rats were subjected to stress - restricted movement and forced swimming daily for 2, 4 and 8 weeks. After each exposure period, the rats were allowed to recover for 6 weeks. Stress exposure led to a decrease in the mass of the thymus and axillary lymph nodes, the number of spleen lymphocytes, thymus and axillary lymph nodes, and an increase in the apoptotic index of splenocytes, thymocytes and lymphocytes of axillary lymph nodes, depending on its duration. The thymus was the most stress-sensitive lymphoid organ. After 2 and 4 weeks of stress exposure, the spleen parameters and most of the parameters of the thymus and axillary lymph nodes in rats returned to the control level after recovery, but not in the group with 8 weeks of stress exposure. The severity of stress on the lymphoid organs increases with the duration of exposure, and with a shorter period of exposure, recovery occurs faster.

RESPIRATORY SYSTEM DISORDERS IN ANTIPHOSPHOLIPID SYNDROME (CLINICAL CASE)

Vatanskaya I.Y., Olenitskaya E.S., Strekozova I.P., Puzina I.O., Vatanskaya Y.O.

Abstract

The article discusses a clinical case of diagnosing secondary antiphospholipid syndrome (APS) in a young patient. The multiple organ symptoms and the need for additional special laboratory research methods make it difficult to make this diagnosis. APS is diagnosed with one clinical (thrombosis or pregnancy complication) and one serological criterion (lupus anticoagulant, antibodies to cardiolipin or p2-glycoprotein I). Patients with APS should be under long-term dispensary supervision to prevent recurrence of thrombosis and their prevention., it is necessary to control the activity of the underlying disease, timely detection and treatment of concomitant pathology, and the impact on corrected risk factors for thrombosis with secondary APS.
