TOPICAL ISSUES OF DIET THERAPY IN THE TREATMENT OF NON-ALCOHOLIC FATTY LIVER DISEASE

Abstract

Non-alcoholic fatty liver disease (NAFLD) as a pathology 25 % of the world's adult population is progressively growing every year, which is a global health problem, as it is often associated with obesity, metabolic syndrome, type 2 diabetes mellitus, cardiac vascular diseases. Currently, in the absence of approved drug therapy, the most appropriate strategy for the management of NAFLD patients with gradual weight loss by reducing the total amount of calories is quite diverse due to the individual characteristics of each case, as well as associated pathology [1]. Obviously, the optimal balance of diet and physical activity for weight loss should be considered as the most logical approach to lifestyle changes in terms of health, but this is not always possible, therefore, the primary emphasis in such cases should be on the individualization of the dietary strategy. The aim of our review is to compare the most current clinical guidelines and approaches to dietary therapy for NAFLD.

ANALYSIS OF CLASSICAL AND MODERN METHODS OF BIOMETRIC EXAMINATION OF DENTAL ARCHES IN THE PERIOD OF PERMANENT TEETH (LITERATURE REVIEW)

Abstract

A review of the literature on the methods of biometric diagnostics was conducted. The aim of the work was to analyze the classical and modern methods of odonometrics and biometrics of dental arches. The analysis of domestic and foreign literature showed the feasibility of conducting additional research, using modern ideas about the options for the shape and size of dental arches. The analysis is devoted not only to the clarification of data on the individual norm, but also to diagnose various forms of anomalies and deformations of dental arches in different directions. This review will help researchers determine the best methods of biometrics in the study of certain types of dental arches. In addition, clinicians can choose biometric methods for predicting dental arches, which must be guided in the treatment of patients, choose the design of orthodontic devices and dentures, taking into account the requirements of individual medicine.

THE MAIN RISK FACTORS FOR DEVELOPMENT OF PERIODONTAL COMPLICATIONS AT THE STAGE OF NON-REMOVABLE PROSTHODONTIC TREATMENT IN PATIENTS WITH CHRONIC PERIODONTITIS

Abstract

The absence of objective morpho-functional indicators of the periodontal tissues state increases the risk of complications at the stage of preparing and prosthodontic treatment of patients with periodontal diseases. The analysis of more than 100 academic papers on the research topic was carried out, the results of which are presented in this review. According to the literature data, the most common complications in non-removable prosthodontic treatment are acute inflammatory reactions, occlusal trauma and functional periodontal overload with significant morphological disorders. The Understanding of physiology and individual variations of the regenerative process in periodontal complex will help the dentist to avoid most complications during prosthodontic treatment and to achieve a long-term positive effect after its completion

COMPARATIVE MORPHOLOGICAL CHARACTERIZATION OF THE HUMAN BRAINSTEM IN THE FIRST PERIOD OF MATURE AND OLD AGE

Abstract

The knowledge of brain aging patterns, in particular its trunk, is necessary for understanding age-related compensatory resources of the body's nervous tissue. Objective: to determine the human brain stem volume in the first period of mature age and in old age using magnetic resonance imaging (MRI) and to compare these parameters in age-related aspect. Material and methods: we analyzed the results of brainstem morphometric study of 94 people (48 men and 46 women) using MRI, who underwent brain examination in the Department of Radiation Diagnostics in the period 2019-2021. Criteria for inclusion of subjects in the study: the first period of mature age or old age of the subject; craniotype - mesocranial; history without diseases and injuries of both central and peripheral nervous system organs, exclusion of drug and alcohol addiction; absence of signs of pathology of brain departments detected during the study. Results: MRI of the brain stem revealed a statistically significant decrease of its volume indices both in men and in women by senile age p < 0,01). There was revealed a tendency for predominance of brain stem volume indices in men both in the first period and old age in comparison with women without statistically reliable difference (p > 0,05). The obtained results will serve as a basis for understanding age-related anatomical changes of the brain stem and allow more accurate diagnostics of patients in the conditions of personalized medicine.

PSYCHOPHYSICAL DISADAPTATION AS A FACTOR OF INITIATION OF THE CONSUMPTION OF PSYCHOACTIVE SUBSTANCES IN THE STUDENT ENVIRONMENT

Abstract

The aim of the work was to determine the relationship between indicators of psychophysical maladjustment, individual functional status and consumption of psychoactive substances among students. The study involved 137 students of state universities in Arkhangelsk. Identification of the experience of using psychoactive substances was carried out using questionnaire and blank methods of retrospective self-assessment of the study participants. Determination of psychophysical maladaptation was carried out on the basis of a combination of psychological and psychosomatic symptoms of stress reactions and anxiety-depressive disorders. Stress resistance was assessed by analyzing the level of general nonspecific reactivity of the organism. The psychological status was determined by a combination of indicators of accentuations and personality traits that characterize the temperament and character of the individual. It was revealed that the regular use of alcohol (2 times a week or more) among students, in the overwhelming majority of cases, is associated with psychophysical maladjustment against the background of low human stress resistance. The level of smoking and samples of narcotic substances among students are positively, linearly related to the frequency of alcohol consumption. The psychological conditionality of the formation of anxiety-depressive and acute stress disorders simultaneously provokes the demand for psychoactive substances. A comprehensive assessment of the functional and psychological indicators of predisposition to psychophysical maladjustment can provide a prediction of the risk of chemical addiction formation among students.

Abstract

Photocolorimetric analysis of the degree of pigmentation of the trabecula along the entire circumference of the anterior chamber angle in patients with primary open-angle glaucoma became possible with the use of the Nidek GS-1 gonioscope. According to RGB indicators, values were determined for weak, moderate, pronounced and pronounced pigmentation of the trabecula. The use of the RGB color model makes it possible to assess with high reliability the differences between the degrees of pigmentation of the trabecula in patients with primary open-angle glaucoma.

THE PLACE OF PROGRESSIVE SPECTACLES CORRECTION IN AN INTEGRATED APPROACH TO THE THERAPY OF THE EARLY STAGES OF AGE-RELATED MACULAR DEGENERATION

Abstract

The purpose: to determine the place of progressive spectacles correction in an integrated approach for the treatment of early stages of age-related macular degeneration (AMD) in hyperopic patients. Material and methods: 30 patients (60 eyes) aged 42-75 y.o., average age (58,8 ± 6,5) years, with early stages of AMD and hyperopia were under observation. The observation group was represented by 20 patients (40 eyes). After AMD revealing they applied progressive spectacles correction of «universal» type and received orally dietary supplement Retinorm® 3 capsules one time per day. The comparison group consisted of 10 patients (20 eyes). They received the same oral dietary supplement and used the previous monofocal correction. Methods of examination of patients included: checking of visual acuity with correction, refractometry, tonometry, computer static perimetry, ophthalmobiomicroscopy with Goldmann lens, optical coherence tomography of the central fundus area with measurement of retinal thickness in the fovea zone and choroid in manual mode. The duration of observation in both groups was 4 months. Results. The thickness of the retina in the fovea zone after the use of correction and prescribed treatment did not change either in the observation group (p > 0,1) or in the comparison group (p > 0,1). The thickness of the choroid after the prescribed therapeutic measures significantly increased in the observation group (p < 0,05) and remained without changes in the comparison group (p > 0,1). An increase of visual acuity was established, as well as a decrease in the number of relative scotomas in the central part of the visual field both in the observation group (p < 0,02) and in the comparison group (p < 0,05). Conclusion. The use of «universal» type of progressive spectacles correction in combination with taking dietary supplement Retinorm® increases the effectiveness of treatment of early stages of AMD in hyperopic patients due to both the activation of choroidal blood circulation, and the improvement of metabolic processes in the choroid and retina.

THE PLACE OF ULTRASOUND DIAG NOSTICS IN THE TREATMENT OF INVERSE STRABISMUS

Abstract

The study included 12 children. The children's age was 10-17 years. Children have complained about developed reverse strabismus after operations performed for concomitant strabismus. Ultrasound biomicroscopy (UBM) of the anterior segment of the eye was performed to estimate the topography of previously operated extraocular muscles and plan subsequent surgical treatment. UBM with a frequency of 50 MHz makes it possible to accurately measure the distance from the limbus to the attachment point to the sclera of the horizontal rectus oculomotor muscle after a previous intervention. In cases the details of the previous operation are not available, the information provided by UBM may be useful in determining the surgical tactics.

EVALUATION OF THE RESULTS OF FRENULOPLASTY USING A DIODE LASER IN CHILDREN

Abstract

The article is devoted to the analysis of the results of surgical treatment of ankyloglossia in children using a diode laser. The patients were divided into three age groups. The immediate and long-term results of treatment were evaluated. The postoperative period was favorable in 98,5 % of cases. After 6-9 months, very good and good treatment results were most common in the first age group (96,4 %). Improvement or normalization of sound pronunciation was noted in 86,7 % of all operated children.

FEATURES OF DIAGNOSIS AND TREATMENT OF ANOMALIES IN THE POSITION OF SEMI-TRAINED WISDOM TEETH OF THE LOWER JAW

Abstract

Retinated wisdom teeth, especially on the lower jaw, are still the subject of discussion by most specialists and, especially, orthodontists. The aim of the work was to determine the indications for orthodontic treatment of semi-trained wisdom teeth on the lower jaw and to evaluate the effectiveness of the measures taken. 12 people with semi-trained wisdom teeth on the lower jaw were examined and treated. The diagnosis of the position of the teeth was evaluated according to the orthopantomogram, on which the lines were applied, and the stress axis was determined, which is a guideline for choosing a treatment method. In this method, tangent lines were drawn to the branch and the body of the lower jaw, which formed an angle at the intersection. The bisector of this angle served as the boundary between the branch and the body of the jaw (stress-axis). The indication for orthodontic treatment was the location of the wisdom tooth in front of the stress axis. In this case, the conditional median vertical of the wisdom tooth, as a rule, intersected with the tangent line to the body of the lower jaw in front of the mandibular angle, regardless of its size.

Abstract

The article discusses the main causes of development and clinical features of the instability of the cervical spine in children of different ages. A clinical and instrumental study of 146 children was carried out to determine the relationshi p between dysplastic instability of the cervical spine and clinical manifestations of neuroorthopedic pathology. Risk factors for the formation of undifferentiated connective tissue dysplasia (UCTD) were identified, signs of osteoligamentous dysplasia on radiographs of the cervical spine and external phenotypic osteoarticular and muscle signs of UCTD in children with instability of the cervical spine were identified. Predicting the development of connective tissue dysplasia before the birth of a child with preventive measures, as well as early diagnosis of pathological conditions and timely treatment will prevent the formation of chronic somatic, neurological and orthopedic diseases.

Abstract

The purpose of the work was to study the effect of high-intensity laser exposure in the therapeutic mode on the morphofunctional state of rat thymus. Material and methods. The experiment was carried out on 54 rats, divided into 2 groups: control (intact animals) and experimental - a single exposure of laser radiation on the thymus region (970 nm, 1 W/cm2 IRE-Polus, Russia). Extraction from the experiment and material sampling were carry out after 1 hour, 1, 3, 7 day after exposure. Histological sections stained with hematoxylin-eosin were study using digital morphometry. Results. A single laser exposure on the thymus led to a change in the relative area of the functional zones of the thymus and their cell density, as well as an increase in the number of Gassal's bodies in the thymic slices and an increase in the relative area of the vascular channel.

Abstract

The article is devoted to an urgent problem - cognitive impairment in patients with arterial hypertension. The aim of the study was to establish objective qualitative and quantitative EEG criteria for cognitive impairment in patients with hypertension in order to use them later to determine the optimal regimen of cerebroprotective therapy and evaluate its effectiveness. The study involved 192 people (patients with hypertension and potentially healthy people without hypertension and CN). The number of patients with an established diagnosis of AH and with CN was 99 people. They were divided into three groups in accordance with the regimens of cerebroprotective therapy prescribed by the neurologist. As a result of the work, an algorithm of treatment tactics was developed and an application for patent No. 2021126327, priority date 06.09.2021, was issued.

CLINICAL PROFILE AND FEATURES OF CARDIONEPHROPROTECTIVE THERAPY IN PATIENTS WITH HYPERTENSIVE FORM OF CHRONIC GLOMERULONEPHRITIS

Abstract

The aim of the work was to evaluate the clinical profile of patients and the frequency of cardionephroprotective therapy in chronic glomerulonephritis (CGN) with hypertensive form (GF), the presence of chronic kidney disease (CKD) and associated clinical conditions. Material and methods. Patients with HF CGN, 102, men - 52, women - 50, (55,0 ± 11,84) years, and stage III EAG, 98, men - 45, women - 53, (64,45 ± 12,1) years, were examined, the diagnosis was verified in the Tver Region Regional Clinical Hospital. Results. Differences in the frequency of associated clinical conditions, stages of CKD and the administration of hypotensive, lipid-lowering and antiplatelet drugs were revealed between patients with HF CGN and EAG. Conclusions. The clinical profile of patients with GF CGN consists of stage 3b-4-5 CKD, CVB, DM, the clinical profile of EAG patients includes stage 1-2-3a CKD, CVB, DM, CHF and CHD. Cardionephroprotective therapy in patients with GF CGN consists in the early inclusion of lipid-lowering and antiplatelet drugs in therapy before the appearance of stage 3b CKD, CVB and always in the presence of DM.

Abstract

The paper presents data on studying biocompatibility and tissue response in the implantation zone of collagen-stimulating coated hernio implants. Endoprosthesis of the anterior abdominal wall was carried out in a supraponeurotic manner on rats of the Vistar breed. When using ascorbic acid coated hernioimplant, less pronounced reactive changes were observed in the surrounding tissues, manifested by the absence of a significant number of inflammatory cells, the predominance of fibroblastic elements in the field of view, the activation of which contributed to the early engraftment of the hernioimplant as a result of stimulation of collagenesis. Therefore, it was concluded that the most optimal is hernioimplant coated with ascorbic acid.

Abstract

Clinical signs of systemic connective tissue dysplasia in children with acute and recurrent torticollis were studied. 225 clinical cases were analyzed. The predominance of joint hypermobility and asthenic physique in the group of children with recurrent torticollis was revealed and proved by the medical statistics methods. In this group, such skeletal clinical forms as flat-valgus deformity of the feet, kyphosis, scoliosis were observed.

THE EFFECTIVENESS OF REGIONAL LYMPHOSTIMULATION IN THE PREVENTION OF COMPLICATIONS AFTER APPENDECTOMY 117 WITH AN ATYPICAL LOCATION OF THE APPENDIX

Abstract

Despite the improvement of the technique of surgery and treatment, the frequency of complications after appendectomy with an atypical location of the appendix varies from 10 to 30 %. The aim of the work is to evaluate the effectiveness of the use of regional lymphostimulation and ozonated solutions in the prevention of complications after appendectomy with an atypical location of the appendix. Materials and methods. 45 patients with an atypical location of the appendix were subjected to analysis (a prospective study). 2 groups were identified: the first (comparison group) included 21 patients who underwent process removal and traditional treatment in the postoperative period. The second, main group, included 24 patients who had regional lymphostimulation, irrigation of the process bed and the operating wound with an ozonated sodium chloride solution at the time of surgery. Results. In the comparison group, there was suppuration of the wound in 4, and infiltration in 2, intestinal paresis in two and early adhesive intestinal obstruction in one. A relaparotomy was performed. In the main group, one suppuration and infiltration of the postoperative wound were noted. There were no fatal outcomes. Conclusion. The use of regional lymphostimulation and ozonated solutions can reduce the frequency of complications and the length of hospital stay in acute appendicitis with an atypical location of the appendix.

Abstract

The processes of mesenchymal-epithelial and epithelial-mesenchymal transitions are associated with embryonic development, tissue differentiation and regeneration. We have analyzed liver tissue in rats from day 10 of embryonic development to adults to prove mesenichymal-epithelial and epithelial-mesenchymal transitions in liver morphogenesis. The immunohisto-chemical study showed that during the liver developmet expression of the epithelial marker (cytokeratin 18) increases, while the expression of the mesenchymal marker (vimentin) decreases, with the exception of the period from 1 to 17 days after birth, when the amount of vimentin increases. Revealed immunohistochemical evidencesof epithelial-mesenchymal and mesenchymalepithelial transitions at the stages of hepatogenesis.

FEATURES OF COMPLEX PREGRAVID PREPARATION OF WOMEN WITH RECURRENT MISCARRIAGE 129 OF INFECTIOUS GENESIS

Abstract

One of the most important issues of practical obstetrics at the present time is the problem of miscarriage, the frequency of which reaches 20 % [1]. The habitual loss of pregnancy is a consequence of many reasons, among which the role of an infectious factor, in particular chronic endometritis, is beyond doubt. According to various authors, up to 10% of the entire structure of miscarriage can be caused by this pathology [2, 3, 4]. An infectious and / or inflammatory process in the uterine cavity leads to irreversible changes in the cervix, which can act as a pathogenetic factor for cervical insufficiency (CI) [5]. Assessment of risk factors for cervical incompetence, primarily chronic endometritis as one of the main reasons for the development of this pathology, is especially important at the stage of pregnancy planning. In this regard, carrying out a full-fledged pregravid preparation in patients with recurrent miscarriage against the background of chronic endometritis is beyond doubt.

CYTOTOXIC ACTIVITY OF NEW AZOLOAZINES IN THE STUDY OF HUMAN BREAST CANCER MCF-7 133 CELL CULTURE

Abstract

The work presents the results of study the cytotoxic activity of new azoloazine derivatives to establish the potential its possibility of use as antitumor agents, including for chemotherapy of metastatic breast cancer. The relevance of the work is due to the wide spread of oncological diseases and high cancer mortality, which dictates the need to constantly obtain cell lines and improve cultivation protocols for testing new antitumor drugs and obtaining new information about the chemistry of cancer. The culturing MCF-7 cells and determining cytotoxicity in a methyltetrazolium test are base methods used in this study. Comparative cytotoxicity was carried out by determining the concentration causing 50 % cell death for 11 azolotriazine derivatives taken at concentrations from 0.25 to 10.0 pM/L. Epirubicin, widely used in breast cancer chemotherapy, we used as a comparison drug. As a result, it was found that not all substances have toxic properties, despite the similar chemical structure and mechanism of action on the cell. The least toxic substances 10 and 11, in which, at the maximum studied concentration of 10 |uM/L, cell survival was 86 and 75 %, respectively. The most toxic compounds against MCF-7 cells are substances 4 and 9 derivatives of with cytotoxicity indicators higher, respectively, by 11.0 and 3.1 highly compared to Epirubicin. The obtained data can be used as a basis for the selection of substances 4 and 9 for further study of their properties on cell models and laboratory animals as substances with potential anticancer activity.

EFFECT OF DIRECT ORAL ANTICOAGULANTS ON THE HEMOSTASIS SYSTEM 142 IN BACTERIAL SEPSIS

Abstract

The inflammatory reaction that arises in patients with sepsis is interconnected with the coagulation system and leads to the development of hypercoagulable changes in the body. Therefore, in this study, we studied the effect of direct oral anticoagulants dabigatran etexilate and apixaban on the hemostasis system without and under conditions of a systemic inflammatory response. Hypercytokinemia was created by intravenous administration of lipopolysaccharide into the tail vein of a rat. The anticoagulant activity of the studied drugs was assessed by their effect on the parameters of the rat blood coagulogram. Dabigatran etexilate showed a pronounced antithrombin effect, increasing the thrombin time by 10.5 times relative to the control in intact animals. Under conditions of hypercytokinemia, this activity increased by 12.8 times. Apixaban did not cause changes in thrombin time, but it increased the prothrombin time index both in intact animals and animals with a systemic inflammatory response by 7 and 8.6 times, respectively. Thus, an increase in the anticoagulant effect of dabigatran etexilate and apixaban under conditions of hypercytokinemia indicates that they may have anti-inflammatory activity.

Abstract

The aim of the study was to study the effect of GABA derivatives on the functional activity of intact and oxidative stress-damaged mitochondria. Isolated mitochondria were obtained by differential centrifugation. Oxidative damage was simulated by incubation with tert-Butylhydroperoxide (200 nmol/ml, 10 min). The studied substances (baclofen, mefebut, neuroglutam, RGPU-238 compound, salifen, tolibut, phenibut, phenotropil) were added to intact and damaged mitochondria (1*10-5 M per 100 μl, 10 min of incubation), the functional state of mitochondria was studied using a polarographic method. It was revealed that salifen and phenotropil had a pronounced protective effect on the respiratory complexes of the electron transport chain.

Abstract

The aim of this study was assess the effect of the heptral on the cognitive functions of female rats exposed to chronic alcohol intoxication (CAI). CAI was modeled with 6-month replacement of drinking water with a 10% solution of ethyl alcohol with sucrose (50 g/l). The "One-way step-down inhibitory (“passive”) avoidance test", "Extrapolation escape task test", "Novel object recognition test" and "Barnes maze test" were carried out at the age of 16 months. The experimental data obtained indicate that there is impairment in preservation of the memory track in female rats after CAI. Heptral neutralized cognitive impairment in animals exposed to prolonged alcohol abuse.

Abstract

The aim of the study was to analyze the effect of laser irradiation and branched chain amino acids (BCAAs) on the restructuring of the vascular component of m. biceps femoris during systematic swimming training. Material and methods. The experiment was conducted on 25 rats divided into 5 groups: 1) intact control, 2) dynamic control, 3) training with amino acid intake, 4) training with infrared laser exposure, 5) training with laser exposure and amino acid intake. Tissue samples of the biceps femoral muscle were fixed with formalin with further use of classical histological wiring. Morphometric analysis of the digital image of objects was carried out. Results. Exposure of infrared laser radiation, as well as the use of amino acids, contributes to a reliable increase in the area of the vascular bed relative to the area of connective tissue. When the studied factors were combined, statistically significant differences were found with isolated laser or amino acid exposure groups. Conclusions. The intake of amino acids and the use of laser radiation stimulates the increase of vascular bed area during the training cycle

ULTRAMICROSCOPIC FEATURES OF THYROCYTES STRUCTURE OF RAT’S THYROID GLAND AFTER 60-DAYS SODIUM BENZOATE 162 ADMINISTRATION

Abstract

Introduction. Sodium benzoate is widely used in both food and pharmaceutical industries. There is evidence of its adverse health effects; However, the effect of sodium benzoate on the ultrastructure of thyroid gland cells is currently not well understood. Materials and methods. The animals were divided into two groups: the group 1 was injected with 1 ml of sodium benzoate solution at a dosage of 1000 mg/kg/body weight intragastrically for 60 days, and the group 2 served as a control. Samples of thyroid gland after tissue processingwere examined under an electron microscope. Results. In the experimental group, it was found that thyrocytes had a cubic, less often flat shape, an expanded rough endoplasmic reticulum with single accumulations in the cavities of fibrous structures, significant lumps of heterochromatin under the nuclear membrane with uneven contours of the latter, an electron-dense mitochondrial matrix. Conclusion. The above changes indicate the effect of sodium benzoate administration both locally (through damage to the DNA of the nucleus and mitochondria) and at the systemic level (through a decrease in leptin levels and, as a consequence, the effect on the hypothalamic-pituitary-thyroid axis).

Abstract

The search for drugs that can prevent the development of acute renal injury (ARI) is one of the main tasks of modern pharmacology. Methods.The experiment was carried out in 50 white Wistar male laboratory rats weighing 280-320 g. The animals were injected with a peptide mimicking the spatial structure of the B erythropoietin chain (pHBSP) and infliximab. Results.The combined use of a peptide mimicking the spatial structure of the B erythropoietin chain (pHBSP) and infliximab before renal ischemia-reperfusion injury is more effective than the nephroprotective properties of the above substances when it is selectively used. Conclusion. According to the results of the study, we can talk about the prospects of the combined use of the peptide mimicking the spatial structure of the B erythropoietin chain (pHBSP) and infliximab in order to correct renal ischemia-reperfusion injury.

Abstract

A change of pain sensitivity in rats of different sex and age (21st day, suckling period; 30th day, infantile period; and 60th day, juvenile period) was studied after prenatal stress of forced swimming in cold water. The perceptual component of nociception in prenatally stressed animals was shown to decrease on the 60th day of life compared to that in 21-day-old specimens. Importantly, the intensity of pain perception in juvenile rats subjected to intrauterine stress was smaller than control specimens. After prenatal stress, the emotional component of nociception in juvenile males and females was lower than in younger specimens. Therefore, prenatal stress is accompanied by a decrease in the perceptual and emotional components of nociception during the juvenile period, but not at the earlier stages of postembryonic development. The observed fluctuations in the degree of perception of the pain stimulus in male animals are slightly greater than those in females.
