ETIOPATHOGENETIC ASPECTS OF LIVER INJURY IN PATIENTS WITH COVID-19

Abstract

Infection caused by the new coronavirus SARS-CoV-2 initially associated with respiratory damage and the development of respiratory symptoms. However, it was subsequently shown that COVID-19 can be considered as a systemic inflammatory process with multiple organ damage. One of the most frequent extra pulmonary manifestations of SARS-CoV-2 infection is liver damage. Several factors of influence on the liver in COVID-19 are currently being considered: viral immunological damage, hypoxia, systemic inflammatory process, drug toxicity and progression of existing liver diseases. This review is devoted to the analysis of available data on the mechanisms and clinical manifestations of liver damage in a new coronavirus infection.

The role of elastin and laminin disbalance and the genome «major proteins» that participate in tissue structures' formation remains an understudied issue.

EPIDEMIC MANIFESTATIONS OF COVID-19 IN VOLGOGRAD REGION: INTERIM RESULTS

Abstract

An assessment of the manifestations of the COVID-19 epidemic process in the Volgograd region in terms of intensity, territorial distribution, structure, and clinical patterns of the disease was carried out. The study used the official data of the Rospotrebnadzor Office in the Volgograd region. From March 24 to October 18, 2020, 17 811 cases of COVID-19 were registered in the region, the incidence rate was 715 per 100 thousand of the population, the case fatality rate was 0,98 %. An uneven distribution of morbidity over the territory was established: 42,3 % of cases were detected in Volgograd, 10,1% in Volzhsky and 6 % in Kamyshinsky district. The age groups 40-49 years old (17,6 %) and 50-59 years old (19,2 %) are most involved in the epidemic process. In the cases structure, the prevalence of females was noted (58,3 %). The clinical patterns of the disease are characterized by the predominance of mild forms (46,8 %), there is a trend to reduce asymptomatic forms and an increase in moderate manifestations. In the Volgograd region, a lower frequency of severe forms and deaths registration has been established in comparison with the Russian Federation. In the social structure of the infected people, the largest share is made up of pensioners (26,3 %) and workers (16,9 %).

THE ROLE OF ANGIOPOIETIN-LIKE PROTEINS IN THE DEVELOPMENT OF RENAL DYSFUNCTION IN PATIENTS WITH RHEUMATOID ARTHRITIS AND METABOLIC CHANGES

Abstract

The article is devoted to the assessment of the relationship between the serum concentrations of angiopoietin-like proteins of types 3 and 4 (ANGPTL 3 and 4) and the development of renal dysfunction in patients with rheumatoid arthritis (RA) with metabolic changes. We examined 158 patients with RA (91,8 % - women and 8,2 % - men) aged 21 to 80 years old with the average duration of diseases - 9 [4-15] years. Negative correlations of average strength between the indices of the estimated glomerular filtration rate (eGFR) according to the 2009 CKD-EPI formula and the level of ANGPTL 3 (r = -0,32, p < 0,001) and ANGPTL 4 (rS = -0,31, p < 0,001) were revealed. It was found that renal dysfunction and the presence of metabolic syndrome (R2 = 0,33) are the two factors which have a direct effect on the ANGPTL 4 concentration in RA patients' serum. ANGPTL type 4 should be considered as a key factor linking the development of renal dysfunction and metabolic changes caused by rheumatoid inflammation.

THE INFORMATIVE VALUE OF IMMUNOLOGICAL AND GENETIC TESTS IN THE DIAGNOSIS WEST NILE FEVER

Abstract

We used ELISA and PCR for laboratory verification of West Nile fever (MUC 4.2.3009-12). We analyzed serum and whole blood samples which had been sent to West Nile fever reference monitoring centre in 2018-2019. A total of 270 blood samples obtained from patients with presumed viral encephalitis of unknown etiology, acute respiratory viral infection, acute respiratory viral infection, meningitis, acute gastroenteritis were analyzed. Antibodies against WNV were detected in 193 (71,4 %) blood samples, while 146 samples were found to be capable of developing an immune response with viral RNA not being detected in them. Both WNV antibodies and viral RNA were detected in 47 (17,4 %) samples tested. WNV RNA was detected in only 14 (5,1 %) cases. Thus, laboratory confirmation of WNF was obtained in 76,6 % of cases (207 out of 270).

CUMULATIVE PROPERTIES OF THE NEW ANTI-MIGENOUS COMPOUND IN THE TABLET MEDICAL FORM

Abstract

Cumulative properties of 2-metoxifenil-imidazobenzimidazole derivative (RU-31) in tablet medical form (8 mg) were studied for development of new drug for migraine treatment. Four-week experiments were carried out on white rats of both sex, with intragastral administration of RU-31 water solution in the doses of 10 mg/kg and 100 mg/kg. On the 29th day the dose of tested compound equal to LD50 was administered to the animals. Due to the calculation of accumulation coefficient (more than 20 units) the tablet form of RU-31 shows the lack of cumulative properties.

Abstract

Aim of the research. To provide retrospective analysis of nearest results of laparoscopic surgery for cardiospasm (according to the materials of the department of thoracic surgery of the «Volgograd Regional clinical Hospital № 1» for 2018-2020). Materials and methods. The research include 32 laparoscopic surgical treatment cases of cardiospasm II-III stage. Results. Analysis of the nearest results of laparoscopic surgery for cardiospasm rated the advantages of the laparoscopic method in perfoming exstramucosal esophagocardiomyotomy are assessed. Conclusion. Marked decrease traumatism and time of the operation. Redused the patients hospital stay and postoperation rehabilitation was also redused in patients with II-III stage of the disease.

The article is devoted to the study of the patients with acute ST-segment elevation myocardial infarction (STEMI) complicated by acute heart failure (AHF). Clinical data were assessed, including the level of stimulating growth factor (ST-2) and growth differentiation factor (GDF-15), depending on the degree of AHF classification of T. Killip. It has been established that ST-2 is a marker of AHF prognosis in STEMI patients at the hospital stage of treatment. GDF-15 reaches high values in acute left ventricular failure.

Abstract

A topical issue in dentistry is the problem of restoring teeth after endodontic treatment. There are many materials available to restore them. However, each of them has its own negative properties. The purpose of this work was to determine the degree of influence of the oral environment on the strength characteristics of endocrowns made of hybrid ceramics. 30 samples from hybrid ceramics were examined. One of the groups was confirmed by thermal Cycling, and the other was not. Further, both groups on the Instron-5982 device were subjected to vertical compression load before the destruction began. According to the results of this study, it can be noted that endocorns made of hybrid ceramics perfectly distribute the occlusal load and have good elasticity, but thermal Cycling has a negative impact on the strength characteristics of this material.

Abstract

The caesarean section is an important factor characterizing of obstetric care. In recent decades, the frequency of cesarean section has become pandemic. The large variability of the initial state of women in labor and the variety of indications for surgery do not allow us to identify the main determinants. In order to develop effective measures to reduce the frequency of abdominal delivery, WHO recommends using the M. Robson classification (TGCS). A study using the M. Robson scale identifies the patient population, among which a decrease in the number of abdominal delivery is possible and measures can be developed aimed at reducing the frequency of cesarean section in level II hospitals.

Abstract

In the working population of pre-retirement age in Volgograd and the Volgograd region, studies of psychophysiological stability, individual features of the organization of the nervous system and the functional state of the sensorimotor sphere were performed. The majority of persons of the surveyed individuals living in areas with hypo comforted conditions shown decrease in psychophysiological stability. In pre-pensioners living in areas with precomfort conditions, involutionary mental depression manifests itself more significantly than if the aging body is affected by negative exogenous factors of a complex socio-ecological situation. The development of situational symptoms must be considered when follow-up surveys of people over 50 years old.

Abstract

The peculiarities of the skin formation as an organ are the basic knowledge for studying the aging and regeneration process. During studying this issue it is necessary to take into account the exogenous factors affecting the skin, which, in turn, is a "barrier organ". The study was carried out on the skin of male Wistar rats in the late prenatal and early postnatal periods. At the initial stages of its formation, the skin undergoes its morphofunctional changes such as thickening of the epidermis and the appearance of the stratum corneum, as well as thickening of the dermis and changes in the collagen fibers shape that make up its structure. The formation of the skin as a full-fledged "barrier organ" occurs in early postnatal ontogenesis.

The present study examined the role of intercellular communication of the epidermis (claudin-1, occludin and ZO) in sepsis (autopsy material; n = 30) by immunohistochemical method. The detected decrease in immunostaning for proteins of intercellular communication, claudin-1 - (64,1 ± 1,1) %; occludin - (19,3 ± 2,1) %; ZO - (17,5 ± 1,4) % сauses minor structural changes, manifested by a violation of local homeostasis and epidermal barrier dysfunction.

COMPARATIVE ASSESSMENT OF THE FUNCTIONAL STATE OF THE NERVOUS SYSTEM OF CADETS AND SCHOOLCHILDREN

Abstract

The aim of the work was a comparative assessment of the functional state of the nervous system of students of secondary schools and specialized educational institutions on the example of the Cossack cadet corps. In the course of the work, 65 students of the cadet corps and 103 students of secondary schools aged 11-13 years were examined using a hardware and software complex for screening and assessing the level of psychophysiological and somatic health, recommended by the Ministry of Health of the Russian Federation. The state of the nervous system of both secondary school and cadet students corresponded to the average values of this age group. The high level of neuroticism in both groups was revealed. Indicators of the speed of complex visual-motor reaction among cadets were significantly higher than among schoolchildren (p = 0,044, student's criterion). Based on the data obtained, the capabilities of the nervous system of the cadets of the Cossack cadet corps are significantly higher than those of secondary school students. The study allows us to draw a conclusion about the need to control the capabilities of the nervous system of children and adolescents studying in both specialized and General education institutions.

DIVERTICULAR DISEASE: FROM TRADITION TO INNOVATION

Abstract

This article presents current data from the world medical literature and a five-year study of surgical clinic on the effect of perioperative intestinal decontamination in patients with acute complicated diverticulitis on the frequency of postoperative complications after elective resection of the sigmoid colon and the formation of primary colorectal anastomosis. The possibility of selective perioperative intestinal decontamination in patients with acute sigmoid diverticulitis complicated by covered perforation, the number of postoperative surgical and general complications was observed. The used scheme of topical antibacterial sanitation, along with orthograde intestinal cleansing with osmolar solutions, significantly reduced the frequency of insufficiency of colectal anastomoses, surgical (abdominal) and the total number of postoperative complications.

The simultaneous increase in the level of expression of core proteins of all studied heparan sulfate proteoglycans and the content of heparan sulfate in pathological tissues indicates the important role of glycosylated molecules in the pathogenesis of ovarian endometriosis.

INDICATORS OF PHYSICAL DEVELOPMENT OF GIRLS OF YAKUTIA ON THE BASIS OF CENTILE ANALYSIS

Abstract

The study of the physical development of the population requires regular monitoring and identification of various trends to establish normative indicators based on ethnicity, sex, age and region of residence. The aim of the work was to determine the regional indicators of physical development of girls in Yakutia using centyl analysis. 1284 girls of 16-20 years of Yakut nationality were examined. Somatometric examination was carried out by the method of V. Bunak, the absolute values of the main components of the body are calculated by the formulas of Matiegka. The centyl analysis is carried out. Smaller overall indicators and greater value of a fat component of a body Yakut girls in comparison with girls of other regions of Russia are established. On the basis of the centyl analysis regional standards of physical development of girls of the yakut nationality of 16-20 years are established.

Abstract

The article describes a rare localization of extrapulmonary tuberculosis in the form of a unilateral lesion of the middle ear in a 47-year-old woman without immunodeficiency, but having a history of pulmonary tuberculosis with residual changes. Sluggish middle purulent otitis media in the patient for 1.5 years was regarded as nonspecific, and only surgical intervention with histological examination of the surgical material made it possible to establish a tuberculous etiology of the process.

PATHOGENETIC APPROACH TO EARLY DIAGNOSIS OF MILD NEURO-ORTHOPEDIC PATHOLOGY IN CHILDREN

Abstract

216 children and adolescents from 3 months to 17 years with mild neuroorthopedic pathology - torticollis, abnormal foot placement, flat-valgus feet, posture disorders, scoliosis, shortening of the limb were examined. The etiological factors of Natal trauma and the pathogenesis of torticollis formation were determined with confirmation by instrumental research methods. During the growth of the child, there is a progression of neuroorthopedic pathology in the form of scoliotic spinal deformity with a misalignment of the pelvic bones due to an asymmetric flat-valgus installation of the feet. The program for differential diagnosis and prevention of mild neuroorthopedic disorders was developed depending on the age of children. It is shown, that the leading link in the pathogenesis of neuroorthopedic pathology is natal trauma. Timely diagnosis, treatment and prevention of disorders of the musculoskeletal system will prevent the formation of pathology in various age periods.

INFLUENCE OF MICROPULSE LASER TREATMENT OF DIABETIC MACULAR EDEMA ON THE RETINAL PHOTOSENSITIVITY

Abstract

72 patients (72 eyes) were examined with flat diffuse or focal diabetic macular edema up to 500 microns high. Patients were divided into two groups. The first group included 40 patients (40 eyes) with a treatment using a Supra 532 laser (Quantel Medical, France). The second group (control group) included 32 patients (32 eyes) with a treatment using a Supra 577 laser (Quantel Medical, France). Significant decrease of macular edema and increase of focal threshold were statistically achieved in both groups. At the same time, a statistically significant difference in the treatment efficacy between the first and second groups was not detected.

Abstract

According to the literature, women with a history of ovarian surgery have reproductive health problems not only during the planning stage of pregnancy, but also during the gestational period. The aim of the study was to reduce the frequency of obstetric complications in pregnant women with operated ovaries with hyperhomocysteinemia by developing a scheme of therapeutic and preventive measures. The study included 117 pregnant women with surgical interventions for benign ovarian tumors/ovarian tumors in the presence of hyperhomocysteinemia, of which 60-the developed scheme was used (group E1) and 57-observations and treatment were carried out according to the orders of the Ministry of health of Ukraine and the Ministry of health of the DPR (group E2). The control group (group K) consisted of 30 conditionally healthy pregnant women. The analysis of the obtained results showed the effectiveness of the proposed complex of therapeutic and preventive measures in pregnant women with operated ovaries with hyperhomocysteinemia.

Abstract

The parasitic contamination of the soil of rural areas of the Astrakhan region for the presence of eggs and larvae of helminths and cysts of pathogenic intestinal protozoa was studied. The highest number of unsatisfactory soil samples was observed in the Limansky district of the Astrakhan region - 12.2 %, when dead larvae of Strongyloides stercoralis and unfertilized eggs of Askaris lumbricoides were found. The presence of unfertilized eggs of Askaris lumbricoides in soil samples taken from playgrounds in rural areas of the Astrakhan region indicates that these objects are contaminated with the feces of infected people. Contamination of soil in rural areas of the Astrakhan region eggs and larvae of helminths, in the Krasnoyarsk area above 4.1 times higher than in lymans'ke and 6.7 times compared to Kamyzyaksky.

Teeth whitening technologies figure prominently in this row. The method of home teeth whitening using mouth guards is not only one of the most effective, but certainly also the most convenient, used in dental practice today. In his normal daily life, a patient can easily use a mouthguard made by a doctor with a whitening gel, or buy ready-made mouth guards containing a whitening gel, which is an undoubted advantage, because in the modern world one of the problems is lack of time, and the desire to have a beautiful smile does not disappear anywhere.

Abstract

A disturbed uterine contraction activity is a basic stage in the pathogenesis that result in a pathological or abnormal labor. Now some new ways f uterine activity were discovered that may be helpful to find promising targets for future medications. This article presents a clinical case of pregnancy with an abnormal labor and describes diagnostic and corrective methods for contractile activity disturbance making an impact to purinergic system. A way to predict a protracted pregnancy and special considerations for induced labor management were suggested. The reported clinical case indicates usefulness of protracted pregnancy prevention and introducing of the developed series of preventive measures to a wide practice aiming to obtain healthy newborns and to reduce pathological labor occurrence.

The analysis of the ratio of the severity of disorders in the perinatal and pubertal periods of development with the risk of CPPS formation in the adult state in 136 men aged 18-40 years was carried out.

EFFECT OF ADJUVANT PREPARATIONS ON THE EXPRESSION OF ANALGETIC ACTIVITY OF MORPHIN IN SOMATOGENIC PAIN

Abstract

Combination of traditional opioid with coanalgesics is recommended to reduce the dose of analgesics and the side effects as well as the relief of different pain severity. In this study was evaluated the effectiveness of different adjuvant drugs: haloperidol (0,45 mg/kg), metoclopramide (5 mg/kg), gabapentin (90 mg/kg), diazepam (1 mg/kg), midazolam (0,3 mg/kg), clonidine (1 mg/kg), atropine (2,7 mg/kg) with subanalgesic dose of morphine (1 mg/kg) in the somatogenic pain test (somatogenic pain was modelled with tail-flick). It was shown that the severity of the antinociceptive effect of morphine was significantly increased when combined with haloperidol, clonidine, gabapentin and metoclopramide; did not change when co-administered with midazolam and atropine; decreased when combined with diazepam.

Bimanual coordination is necessary for successful performance of monotonous activities accompanied by monotonous complex-coordinated movements. Different types of complex operator tasks require a variety of coordination patterns with different levels of interaction between attention and hand movements. The article presents the results of studying the state of monotony when performing purposeful fine manipulative movements, considering the individual features of perception. The developed method of studying bimanual coordination allows us to establish the development of fatigue and monotony, which directly depends on the psychophysiological characteristics of a person.

The lack of a sufficient amount of data on the structural features of the paraneural structures of the sciatic nerve at different periods of ontogenesis contributed to the conduct of this study, as a result of which it was revealed that with an increase in the number of days from the moment of birth, a complication of the structure of the «paraneuria» was observed in an inextricable connection with the complication of the structure of the endo, peri-, epineuria and the conductive component of the sciatic nerve. «Morphological improvement» was manifested both in the appearance of new structures in the «paraneuria» and in a change in the ratio of the cellular and fibrous components of the connective tissue, which is its morphological substrate.

Abstract

The clinical significance of iron deficiency in heart failure patients with preserved ejection fraction is not well understood. 158 patients with chronic heart failure with preserved ejection fraction were examined. The clinical manifestations of heart failure, the level of NT-proBNP, iron, ferritin, transferrin, were studied in all patients, echocardiography was performed in one-dimensional, two-dimensional, and Doppler modes (pulse-wave, constant-wave, and tissue) in standard positions according to the generally accepted technique. It has been established that iron deficiency in patients with heart failure with preserved ejection fraction worsens the indices of central hemodynamics and promotes an increase in the clinical manifestations of heart failure, especially in the development of anemia.

USING OF PERSONALIZED SCREW GUIDE FOR LATARGET PROCEDURE

Abstract

The advisability of using a personalized guide in patients with anterior instability of the shoulder joint during Latarget procedure is shown. A multiplanar reconstruction of postoperative CT was carried out and the position of the bone block of the graft in the sagittal and axial planes relative to the articular process of the scapula was assessed. The angle between the screws and the plane of the articular process of the scapula was also evaluated. The use of preoperative CT planning with the manufacture of a personalized guide for positioning and fixing the bone block of the graft during the Latargier operation allows to increase the accuracy of the restoration of the defect in the articular surface of the glenoid, to position the implants more correctly and thereby reduce the risks associated with damage to the suprascapular nerve branch.

Abstract

This article presents the results of a comparative analysis of the content of individual amino acids in the overground part of Glycyrrhiza glabra L. and Glycyrrhiza echinata L., growing in the Volgograd region. By the HPLC study, 16 amino acids (Ala, Gly, Tyr, Ser, Asp, Glu, Arg, His, Pro) were identified in the plant raw materials, among which 9 are replaceable amino acids and 7 are irreplaceable amino acids (Thr, Val, Met, Ile, Leu, Phen, Lys). At the same time, of the total content, the share of replaceable amino acids in Glycyrrhiza glabra L. is 10,09 %, and irreplaceable ones - 5,84 %. In the overground part of Glycyrrhiza echinata L., these indicators are slightly higher - 11,47 % of replaceable amino acids and 6,80 % of irreplaceable amino acids.
