30 YEARS ON THE BORDER OF INNOVATION IN OPHTHALMOLOGY

Abstract

30 years activity of the Volgograd branch of FSAI NMRC ISTC «Eye Microsurgery named after academician S.N. Fedorov» of Public Health Ministry of the Russian Federation is constantly aimed at improving the efficiency and safety of treating eye diseases which is possible only if innovative technologies are developed and introduced into the practical work of the regional ophthalmological service. The period 1988-2018 of the ophthalmic care provision was analyzed. There were performed more than 610000 laser and surgical operations during this period. Fulfillment of work on the territorial state guarantees program for compulsory medical insurance, application of modern diagnostics methods, monitoring and treatment made it possible to increase the efficiency and availability of primary, specialized, high-tech ophthalmic care.

THE ORGANIZATION OF OPTICAL KERATOPLASTY IN THE REHABILITATION OF PATIENTS WITH ANOMALIES AND CORNEAL OPACITIES

Abstract

The article analyzes the organization and clinical efficacy of keratoplasty for visual purpose, conducted in the Khabarovsk branch of the FSAI NMRC ISTC «Eye Microsurgery named after academician S.N. Fedorov» of the Russian Federation. 111 patients (111 eyes) were operated on for: bullosa keratopathy 3-4 degrees - 55 eyes; stage 4 keratoconus - 37 eyes; posttraumatic cornea belmas of 3-4 categories - 14 eyes; hereditary corneal degeneration - 5 eyes. In 88 eyes, the technique of penetrating keratoplasty was applied, in 19 eyes - anterior deep layered keratoplasty, in 4 eyes - descemet membrane transplantation.

Abstract

84 patients (150 eyes) with different studies of glaucoma, glaucoma suspects and healthy eyes were examined. Using HRT and OCT similar parameters of the optic disc, peripapillary retina and macula area were studied. Optical coherence tomography (RTVe-100 OCT) and Heidelberg laser retinal tomography (HRT-3) have almost the same opportunities for early diagnosis of glaucoma.

Abstract

Experiment was carried out on 80 mongrel rats in which diabetes was called forth by alloxan monohydrate injection. Experimental animals were divided into 2 groups. In the main group substitutional insulin therapy was begun, in control group animals didn't receive special therapy. In 3 days of insulin therapy experimental animals of both the groups were excluded from investigation for intraocular liquids to be taken in which VEGF-A concentration was determined. We didn't reveal reliable decrease of VEGF-A concentration in intraocular liquid between the groups.

Abstract

The clinical study involved 55 people aged from 17 to 36 years old with acute optic neuritis (ON) associated with HVI. Data analysis showed that more significant positive dynamics was noted in patients, who received additional muscle injections of Cycloferon. The method of etiotropic therapy at ON, associated with HVI, developed by us, allows to shorten recovery time and improve functional results of treatment.

THE REMOTE OUTCOMES OF GLAUCOMA SURGERY

Abstract

We perform the results of examination of 38 patients (45 eyes), undergone primary open-angle glaucoma surgery. The patients underwent non-penetrating deep sclerectomy with the implantation of hemostatic sponge impregnated with sol. Dexamethasone into intrascleral space. The mean values of IOP was (17,4 ± 0,18) mm Hg in 6 months postoperatively The given technique increases and prolongs the hypotensive effect, allows receiving the well-formed filtrating bleb and intrascleral area, decreases the possibility of trabecular descemet membrane thickening.

Abstract

Frequency of recurrences of increase of intraocular pressure (IOP) in postoperative period of phacoemulsification (PE) using our own surgical techniques at combination of primary open-angle glaucoma (POAG) and age-related cataracts was studied. The clinical material consisted of 83 patients (83 eyes) with developed or far-advanced stages of glaucoma, a rigid pupil, a dense nucleus, a lens subluxation of the 1st degree, which had antiglaucomatous operations. The use of our own methods of performing some stages of PE helped to reduced intraoperative trauma and frequency of recurrence of increase of IOP in period of observation to 3 years.

INNOVATIVE CAPABILITIES OF NON-INVASIVE VITAL ASSESSMENT OF VASCULAR STATUS OF MICROCIRCULATORY BED IN DIABETIC RETINOPATHY

Abstract

The examination included 102 patients, divided into 2 main groups. Control group comprised 48 patients. We analyzed the results and made conclusions about reducing the relative density of microcirculation in the vascular plexus of the central retinal zone according to OCT-angiography data. By capillaroscopy we revealed the pathological alterations of nail bed capillaries. The correlation relationships between alterations of nailfold microcirculation and those of the relative density of microcirculation in the vascular plexus of macular zone were defined.

ANALYSIS OF THE EFFECTIVENESS OF SCLEROPLASTIC OPERATIONS IN CHILDREN WITH PROGRESSIVE MYOPIA WITH LONG-TERM FOLLOW-UP. NEW POSSIBILITIES OF DRUG TREATMENT OF PROGRESSIVE MYOPIA

Abstract

There were performed a retrospective study of scleroplasty results in patients with progressive myopia (1282 eyes) and a prospective study of patients with myopia after scleroplasty (192 eyes). The age of patients is from 5 to 17 years, mean age is (12,5 ± 0,07) years. The observation period is from 1 to 7 years. The studies showed slowing rate of myopia progression after sclera operations in the majority of operated patients - in 92 % of cases. The maximum stabilizing effect of scleroplasty is observed one year after the operation. After 4 years and 7 years after operation the stabilizing effect is reduced. The combination of weakness of accommodation, constantly redundant tension of accommodation (CRTA) and finding intraocular pressure values in the upper normal range indicate the progression of myopia. The use of 0,005 % solution of Latanoprost (Prolatan) in the complex treatment of patients with myopia leads to normalization of intraocular pressure, increase of corneal hysteresis and rigidity index of corneoscleral membrane, as well as increase in the coefficient of accommodative response and decrease in the coefficient of the ciliary body microfluctuations.

USE OF LATANOPROST INTHETREATMENT OF PROGRESSIVE OF MYOPIA

Abstract

There was performed a study of 36 patients with myopia after scleroplasty (36 eyes). The patients' age is from 5 to 17 years old, average age is (13,4 ± 2,5) years. The combination of accommodation weakness, constantly redundant tension of accommodation (CRTA) and intraocular pressure values in the range of 18 to 23 mm Hg indicates the presence of ophthalmic-hypertensive syndrome of overstretching accommodation and the progression of myopia. The use of 0,005 % solution of Latanoprost (Prolatan) in the complex treatment of patients with myopia leads to normalization of intraocular pressure, increase of corneal hysteresis and rigidity index of corneoscleral tunic, as well as increase in the coefficient of accommodative response and decrease in the coefficient of the ciliary body microfluctuations.

Abstract

We evaluated the results of accommodography before and after laser stimulation by the accessory «Rubin». The values of the accommodative response (AR) and the number of «droppings» in the amblyopic eyes exceed these indices in the fellow eyes. AR values of amblyopic and fellow eyes differ significantly before treatment, after treatment the differences of AR are not significant. We revealed the significance of the differences between numbers of «droppings» in histograms of the amblyopic and fellow eyes. Their number decreased significantly in the amblyopic eyes after treatment.

CHANGES IN HORMONAL REGULATION IN PATIENTS WITH ACUTE AND CHRONIC FORMS OF CENTRAL SEROUS CHORIORETINOPATHY

Abstract

A study of the concentration of stress hormones was conducted in 16 patients with acute and in 18 patients with the chronic form of central serous chorioretinopathy. In patients with central serous chorioretinopathy, there is an increase in blood concentrations of cortisol and 17-OH-oxyprogesterone, a 25-32 % reduction in the concentration of thyroid stimulating hormone with a constant content of thyroxin was detected, а decrease in testosterone concentration and the concentration of melatonin in the urine was significantly reduced. Thus, the obtained data prove the validity of the idea of a close connection and interdependence of the revealed changes in the hormonal regulation in the pathogenesis of central serous chorioretinopathy.

Abstract

The examination included 40 patients (58 eyes) with diabetes mellitus: without any alterations in the eye fundus, with nonproliferative diabetic retinopathy (NPDR) and with proliferative diabetic retinopathy (PDR). Patients with diabetic retinopathy (DR) underwent complete panretinal laser coagulation (PLC). PLC in case of DR did not lead to the significant alterations of the relative density of vessels of microcirculatory bed in the superficial vascular plexus of macula.

MORPHOLOGICAL ASPECTS OF THE DYNAMICS OF REGENERATION CORNEA IN TREATMENT OF INFECTED INJURIES OF VARYING METHODS

Abstract

An actual problem in modern ophthalmology is the large number of infected traumatic lesions of the cornea. The study was carried out on 90 mature rabbits of the chinchilla breed, of which experimental groups were formed, included 30 animals (60 eyes) in each group. In order to study the structural changes in the eyes, a morphological study was performed in rabbits. According to the results of the study it was found that the use of antibacterial polymer membranes when used after an infected trauma can be achieved by rapid restoration (on day 5) of the morphological parameters of the cornea to normal values. With the use of standard antibiotic regimens, complete restoration of the cornea occurs on day 7.

PECULIARITIES OF INDICATORS OF BINOCULAR DYNAMIC PUPILLOMETRY IN CATARACT (PILOT RESEARCH)

Abstract

Peculiarities of pupil response parameters deviation among patients with age-related cataract and among patients with diabetic cataract and among patients with type II diabetes, which is complicated by autonomic neuropathy, are noted. A certified digital automated binocular pupillometric complex of KSRPRd (complex screening registration of simultaneous binocular pupillary response to a light stimulus digital to assess the functional state of the body) was used for estimation pupillary reaction, which makes it possible to carry out dynamic pupillometry. Based on the results of pupillometry among patients with age-related cataracts, a expressed weakening of the pupillary response on the light stimulus was noted: low rates of pupil contraction and dilatation, pupil contraction amplitude, latency time of pupillary reaction start-up, with pupil diameter, contraction time, and expansion time corresponding to age indices. Among patients with diabetic cataracts decrease of the pupil diameter, increase of the latency period, narrowing speed reduction and pupil dilatation speed, and the amplitude of pupillary narrowing were noted. The pupillary response to the light stimulus is significantly weakened. The weakening of the pupil response to light and the reduction in the amplitude-velocity characteristics of the pupillometry among patients with age-related cataracts can be attributed to a violation of the passage of a light stimulus due to clouding of the lens. The revealed deviations require further study. Shifts of these parameters of pupillometry in diabetic cataracts which is complicated by autonomic neuropathy are connected not only due to metabolic disorders which are associated with insulin deficiency and the violation of carbohydrate metabolism causing clouding of the lens, but also with the defeat of efferent parasympathetic and sympathetic pathways leading to dysfunction of the pupil sphincter and its narrowing.

SPONTANEOUS INTRACHIASMAL HEMORRHAGE: CLINICAL CASE

Abstract

This article discusses a clinical case intrachiasmal hemorrhage. Patient V., 51 yo. entered the hospital with complaints of a lack of vision in the right eye, a severe deterioration in the vision in the left eye and a diffuse headache. The examination revealed optic nerve chiasm change having the features of hemorrhage in the subacute phase. During the operation, pay attention significantly increased and gray-blue color optic chiasm and the initial sections of the right optic tract. When dissecting the pial membrane of the chiasm at a depth of 1 mm, a dense cystic formation was found. The masses corresponding to the hematoma in the organization stage are removed from the cavity of the cystic formation. In the early postoperative period, the patient noted an improvement in vision in the left eye. The described clinical case demonstrates the possibility of early decompression of optic and the importance of radical removal of chiasmatic hematomas without significant deterioration of the neurological and visual deficit in the postoperative period.

ADOPTED IMPACT IN THE PREPARATION OF YOUNG SPRINTERS

Abstract

The effects of the use of essential oils as relaxation agents affecting the performance of muscle activity are considered. For the effects of muscle relaxation, an odorant effect was used. It was carried out by inhalation of a mixture of essential oils of lavender, bergamot geranium, basil, rosemary and eucalyptus, dissolved in the basic neutral grape seed oil. Participants in the study were regularly practicing young sprinters, aged 14-15 years. The results of the odorant exposure were assessed by the nature of the impact on the stride length and the results of the test sprint. During the preliminary study a decrease in the running speed was found at the final part of the test distance with slightly varying parameters of the stride length. After the odorant exposure, a significant and statically significant increase in the stride length occurred. As a result, the speed in the 80-90 m segment decreased significantly less than during the preliminary study; this decrease was statically insignificant. It is assumed that the mechanisms of this phenomenon are multidimensional. Most likely, they are associated with changes in the biomechanical structure of the stride length, i.e. maintaining its length. In addition, there may be a significant increase in the efficiency of running, due to muscle relaxation.

The study is devoted to the development of related substances determination technique in the original dipeptide, which is fourth loop of nerve growth factor mimetic - GK-2, and was carried out using methods of thin-layer chromatography and high-performance liquid chromatography with spectrophotometric detection (HPLC and TLC). The technique for determining of the related impurities content includes two stages: analysis by thin-layer chromatography and analysis by high-performance liquid chromatography. The developed technique allows detecting impurities in the substance in the content of 0,1 % and more.

Clinical cases of treating patients with inflammatory genesis of infertility using assisted reproductive technology (ART) techniques with different outcomes are presented. The follicular fluid (VF) was studied in patients with the method of wedgeshaped and marginal dehydration. The outcome of the use of ART technologies was predicted by the structuring of the VF. The introduction of methods into the practice of a reproductive expert will contribute to the ability to determine the readiness of the ovaries to apply the methods of ART, assess the degree of residual metabolic disorders in follicular structures and the organism as a whole, and predict the effectiveness and outcome of ART.

ASSESSMENT OF INFLUENCE OF THE PIR-4 CONNECTION ON BRAIN EDEMA AT BILATERAL OCCLUSION OF THE GENERAL CAROTIDS AT RATS

Abstract

The study of the decongestant activity of the new compound PIR-4 on the background of cerebral ischemia in rats was conducted. The experiment was performed on 50 male Wistar rats. Global cerebral ischemia was modeled by irreversible ligation of common carotid arteries. It is established that bilateral occlusion of the carotid arteries causes a decrease in the level of calcium ions in the blood (2,31 ± 0,06) mmol/l, metabolic lactic acidosis (11,4 ± 0,03) mmol/l, decrease of glucose consumption of brain tissue to (0,49 ± 0,34) mmol/l, which leads to swelling of the brain. Preventive administration of Vinpocetine, Cinnarizine and experimental compound PIR-4 allowed to correct the violations. So, against uses of the substance PIR-4 lactic acidosis significantly decreased to (3,64 ± 0,19) mmol/l, which is comparable to the drugs of comparison. The level of calcium in the group of animals treated with PIR-4 did not significantly increase by 9,1 % relative to the NK group of rats, glucose consumption statistically significantly increased to 15,33 %, the degree of hydration of brain tissues in the postischemic period significantly decreased by 1,97 %. The obtained results indicate the decongestant activity of the new pyrimidine-4-(1H)-a-PIR-4 derivative.

At the same time, at the site of implantation, a prosthetic-tissue complex is formed, the physicomechanical properties of which determine the number of complications and relapses after surgery. The purpose of this work was to study the physicomechanical properties of the formed prosthetic-tissue complex after implantation of the endoprosthesis. Material and methods. The study of the physicomechanical properties of the prosthesis-fabric complex was carried out on an REM-0.2-1 tensile testing machine on the basis of the research laboratory of the FSBEI HE KSMU of the Ministry of Health of the Russian Federation. The following indicators were calculated: breaking load, relative breaking elongation and tensile strength. Measurements were performed along a looped row and along a looped column. It was taken into account that the indicators of elasticity and relative rupture elongation under load characterized the elastic properties of the endoprosthesis, and the indicator of the ultimate strength - the strength properties of the endoprosthesis. The obtained digital data was processed using the software package Statistica v. 8.0 (StatSoft Inc., USA). Differences were identified at the 0,05 significance level. Results. During the implantation of the endoprosthesis «Gineflex», by the 21st day of the experiment, the prosthesis-tissue complex that was being formed was more elastic under conditions using the PRP technology. The elasticity of the prosthesis «Gineflex» ultralight after implantation was greater, since relative to the initial values, the decrease in elongation of the complex «prosthetic-tissue» with the introduction of autoplasma into the wound, enriched with platelets, ultralight was 7 %. Without the use of PRP technology, the reduction was 10 % along the hinge bar. The study of the strength properties of the emerging prosthetic-tissue complex during the implantation of the Gineflex ultralight revealed large values of the breaking load and, as a result, greater strength along the stitch row and hinge column under the conditions of PRP technology. «Gineflex» ultralight had the greatest strength, the maximum values of the breaking load of which were observed along the stitch column. By the end of the experiment, the breaking load, in comparison with the initial data, increased by 2 times along the hinge bar and 1,6 times along the hinge row. Conclusion. The use of PRP technology in endoprosthesis replacement with the «Gineflex» endoprosthesis ultralight results in a more durable and at the same time elastic prosthetic-tissue complex. Also, when implanted under conditions with the introduction of autoplasma enriched with platelets into the wound under the endoprosthesis, the inflammatory stages change faster and the active regeneration period begins, the morphological substrate of which is the appearance of mature, brightly oxyphilic collagen fibers on the 10th day of the experiment.

EXPERIENCE OF THERAPY OF CHRONIC MYELOID LEUKEMIA IN THE NOVOSIBIRSK REGION, FOURTEEN YEARS OF OBSERVATIONS: RESULTS AND DIFFICULTIES

Abstract

We have analyzed the therapy outcomes of chronic myeloid leukemia of patients, received imatinib treatment for more than 12 month (136 patients). Complete hematologic response (CHR) is attained in 88,2 % cases (120 patients), complete cytogenetic response (CCR) (ph+ <0%) - in 72,1 % cases (98 patients), major molecular response (MMR) - in 47,1 % cases (64 patients). The primary resistance to imatinib is observed in 20,6 % cases (28 patients), secondary - in 7,4 % cases (10 patients - 7 patients have lost the CHR and CCR and 3 patients have lost the CCR and MMR). The thirteen patients of those, who have primary or secondary resistance to imatinib have been treated with second-generation tyrosine kinase inhibitors (nilotinib, dasatinib) and in 84,6 % cases (11 patients) the CCR have been obtained, 5 patients (38,5 %) have attained the CCR and 4 patients (30,8 %) have achieved the MMR. Among all those patients, who have been treated with I and II generations tyrosine kinase inhibitors, 75,7 % patients have attained CCR, 50 % - MMR. We have estimated overall survival (OS) analysis and obtained following results: the median of OS have not been achieved, 5-years OS rate is estimated as 90 %, 10-years OS rate - more than 77 %, calculated 15-years OS - more than 60 %.

DIFFERENTIAL DIAGNOSTIC OF SCLEROCOMPRESSION MACULOPATHY AND CENTRAL LOCALIZED CHOROIDAL HEMANGIOMA

Abstract

The paper presents a comparative analysis and algorithm of clinical and instrumental diagnostics in complex differential cases with pathological changes in the macular area, such as sclerocompression maculopathy (SM) and choroidal hemangioma (CH). It was revealed that the most accessible, non-invasive and highly informative method for the differential diagnosis of SM with GC is high-resolution optical coherence tomography. In difficult diagnostic cases, magnetic resonance imaging of the orbits is recommended. As an additional method for identifying the cause of the development of complicated forms of SM and CH activity, invasive methods such as fluorescent angiography and angiography with indocyanine-green can be used. Differential diagnosis of SM and CH is extremely important for the correct choice of treatment tactics.
