Abstract

The article is devoted to an actual problem-combined blunt trauma of the abdomen and retroperitoneal organs. A treatment and diagnostic algorithm is proposed to standardize the approach to patients with this pathology depending on the hemodynamic status, and the indications for various diagnostic methods and the order of their use are specified. The author analyzes the literature sources of Russian and foreign authors and justifies the expediency of using different research methods to make a decision about surgical treatment, dynamic observation or conservative therapy.

SYMPHYSIOPATHIA AS AN INTERDISCIPLINARY PROBLEM

Abstract

Symphysiopathia is one of the complications of the musculoskeletal system that occurs in the second half of pregnancy and childbirth, which can limit the functionality of patients and worsen the quality of life. Symphysiopathia often becomes an interdisciplinary problem that requires participation in the diagnosis and treatment of not only obstetricians and gynecologists, but also traumatologists, urologists and other specialists. Information about the frequency of occurrences of symphysiopathia is contradictory and registered in the range from 0,03 to 2,8 %. These contradictions are caused by the lack of clear criteria for diagnosis, overall terminology and low clinical alertness of treating doctors. The terminology «symphysitis» is not relevant, because it indicates the inflammatory genesis of the disease. Currently, the most appropriate term is «symphysiopathia», which reflects the leading (but not the only) symptom of the disease-pain. Modern research shows that the development of symphysiopathia in pregnant women is based on a combination of several fundamental factors - biomechanical causes, undifferentiated connective tissue dysplasia and magnesium and calcium deficiency. There is no consensus about cause-and-effect interrelation, and this problem needs to be studied.

The peculiarity of this category of patients is high comorbidity, so the volume of surgery should be determined not only by the type and severity of the course of thyroid pathology, but also by the clinical characteristics of concomitant diseases. To date, the issue of choosing the volume of surgery in elderly and senile patients remains debatable.

Neurogenic bladder is one of the serious pathologies that strongly affect the quality of life and, sometimes, posing a threat to life. The clinical picture of neurogenic bladder presents various symptoms and severity. Depending on the etiology and pathogenesis of the disease, there are various approaches to the treatment of neurogenic bladder. Idiopathic overactive bladder has no pathogenetic treatment, because the pathogenesis of this disease is not studied. The methods of treatment are divided into drug and non-drug. Until the end, the effect on the detrusor state of drug instillations and physical methods of exposure remains unexplored.

PREVENTION OF SHORT-TERM COMPLICATIONS FOR OFF-PUMP CORONARY ARTERY BYPASS GRAFTING IN PATIENTS WITH DIABETES MELLITUS

Abstract

Diabetes mellitus is an independent risk factor for the development of early postoperative complications in coronary artery bypass grafting without cardiopulmonary bypass (off-pump CABG). A comparative analysis of early postoperative complications for off-pump CABG in patients with diabetes mellitus was made. A randomized study of the results of surgical treatment of patients with coronary heart disease who underwent coronary artery bypass grafting of 2 or more branches of the coronary arteries without the use of cardiopulmonary bypass included 191 patients. In patients of the main group (n = 32), the comorbid background was complicated by diabetes; in patients in the control group (n = 159), this disease was not detected. For statistical processing of the obtained data, nonparametric criteria were used (Fisher test and relative risk (RR) with a 95% confidence interval (CI)). It is proved that in patients of the main group (with diabetes) the relative risk of developing postoperative complications (acute myocardial infarction, pneumonia, acute renal failure, multiple organ failure syndrome, etc.) is significantly higher than in patients in the control group (RR = 1.36-4 .97). The use of combined anesthesia (with prolonged thoracic epidural analgesia) allowed patients of both groups to significantly reduce the risk of developing clinically significant complications (RR = 0.26-0.78). Performing off-pump CABG in patients with diabetes is associated with a significant increase in the risk of postoperative complications, and the use of combined anesthesia effectively reduces the risk of their development in patients of this category.

Abstract

Laparoscopic adrenalectomy is the «gold standard» in adrenal neoplasm surgery. However, in the case of bilateral lesions or lesions of the only adrenal gland, it leads to the need for lifelong hormone replacement therapy. Thus, organ-preserving operations are becoming more and more in demand today. The article presents the results of laparoscopic and laparotomic resection of the adrenal gland in 32 patients with various benign adrenal neoplasms. The possibilities of organ-preserving operations on the adrenal gland were determined. Reflected approaches in the choice of partial resection of the adrenal gland.

ASSESSMENT OF THE QUALITY OF LIFE OF PATIENTS AFTER CHOLECYSTECTOMY, DEPENDING ON THE TYPE OF SURGICAL ACCESS AND THE NATURE OF THE DISEASE

Abstract

The prevalence of cholelithiasis among the working population ranges from 10 to 20 %. Cholecystectomy ranks first in frequency among surgical interventions. The purpose of this work is to assess the quality of life in patients with cholelithiasis 6 month after cholecystectomy. The study was conducted on the basis of three medical institutions in the Russian Federation. The patient groups were homogeneous in their composition, and the quality of life was assessed using the Gallstone Impact Checklist. The groups were compared depending on the type of operative access (laparoscopic, mini-access), and depending on the nature of the disease (symptomatic and asymptomatic variants). The results were calculated as the average calculated value ± standard deviation. The following results were obtained, which show that cholecystectomy significantly reduces the quality of life of patients. Laparoscopic access is more preferable in the presence of pain syndrome, as the main factor in reducing the quality of life. Mini-access surgery is not accompanied by a deterioration in the quality of life in patients with pain and dyspeptic syndromes, but it improves the emotional and volitional sphere of human activity.

Abstract

An algorithm for diagnosing congenital kidney disease in the antenatal and early postnatal period was developed, and the management of these patients depending on the nature of the detected pathology was determined. The article presents the results of diagnostics of congenital kidney disease in 126 children. Of these, 70 children showed signs of kidney pathology in the antenatal period during ULTRASOUND examination of the kidneys in the fetus. The criteria for detected congenital pathology in the antenatal and early postnatal period are determined.

OPTIMIZATION OF PROFESSIONAL ACTIVITY IN OPERATORS OF MORNING CHRONOTYPE BASED ON BIOELECTRICAL FEATURES OF BRAIN

Abstract

Biorhythmological features and electroencephalography parameters in operators of morning chronotype demonstrate a significant mutual influence. In particular, frequency of the low-frequency beta-rhythm in left central leads and the total result of Ostberg-Stepanova test have an inverse correlation of average strength (rs = -0,697), which reflects the expressed ability of these operators to perform stereotypical, routine mental operations. In addition, total result of Ostberg - Stepanova test has a strong inverse correlation with frequency of low-frequency beta-rhythm in right central (rs = -0,704) and right temporal leads (rs = -0,738). This indicates that «pronounced larks» have the ability to quickly and efficiently perform repetitive mental operations, especially those associated with the impact of non-verbal auditory stimuli.

Abstract

With the development in dentistry of dental implants is growing steadily and the number postprostatectomy complications, manifested periimplantation mucositis and dental periimplantitis. If the phenomena of mucositis are reversible, then the destruction of bone tissue around the dental implant is impossible to restore the integrity of the bone. An important diagnostic criterion is early diagnosis by a dentist of initial changes in periimplant tissue of inflammatory origin. Stress is a factor in the development of postprotetic complications. In this paper, based on the immunological parameters of blood, the factors responsible for the development of a stress-triggering reaction are determined. Based on the General and biochemical blood analysis, indicators were determined that allow assessing the stress situation in the study groups (cortisol concentration, alkaline phosphatase, Garkavi index, epinephrine, noradrenaline) in patients with postprotetic complications in the near and long-term follow-up and treatment. Proven change of immunological indices on the background of psychoemotional stress in patients with postprostatectomy complications.

Abstract

Diabetic nephropathy takes a leading place among diseases requiring replacement therapy for renal failure, and is also a leading cause of disability and mortality among patients with type 1 diabetes. The development of arterial hypertension is one of the markers of the formation of diabetic nephropathy. We performed a prospective cohort study. A comprehensive clinical and instrumental examination of 78 children with type 1 diabetes was carried out. The features of changes in the daily profile of blood pressure and microcirculatory disorders in children before the formation of diabetic nephropathy were revealed. A mathematical model is developed for predicting the development of arterial hypertension by determining microcirculation parameters using a laser Doppler flowmeter. Early diagnosis will allow timely prescribing of pathogenetic treatment, improving the prognosis of this category of patients.

In the clinical practice of a dentist-orthopedist, patients with various diseases, abnormalities and deformities are often found. In this case, there may often be a relationship between General somatic pathology and manifestations in the oral cavity. Parkinson's disease is no exception. This paper discusses the main features of the course of the disease in the oral cavity and the principles of a differentiated approach to orthopedic treatment. So in patients with Parkinson's disease, sialorrhea (salivation, hypersalivation), dryness of the red border of the lips and oral mucosa, speech changes, «prosthetic» stomatitis are observed. At the same time, these patients have sensory and vegetative disorders, violation of muscle tone, which aggravates the process of prosthetics and creates certain difficulties for a dentist-orthopedist. FEATURES OF MICROBIOLOGICAL AND IMMUNE STATE IN PREGNANT WOMEN WITH INFLAMMATORY PERIODONTAL DISEASES AGAINST THE BACKGROUND OF GESTATIONAL DIABETES MELLITUS

Abstract

90 pregnant women with inflammatory periodontal diseases, aged 27 to 35 years were examined. The main group consisted of 60 pregnant women with gestational diabetes mellitus and the control one consisted of 30 patients with physiological pregnancy. All patients were examined before providing treatment and preventive measures at 14-15 weeks of pregnancy and after them at 37-38 weeks. Studies have shown deterioration in periodontal health, oral hygiene and increase in contamination of the oral cavity in patients of the main group as compared with patients of the control group. The use of the anti-inflammatory succinic acid based preparation is effective for inflammatory periodontal diseases in pregnant women with gestational diabetes. The improvement of periodontal tissues health was confirmed by the posttreatment indices of hygiene, immunoglobulins, microbiocenosis and periodontal indices that promote normalization of the oral cavity.

Abstract

A total of 381 girls aged 17-20 years of different health groups were evaluated. Dimensional and proportional variations of the physical parameters, body component composition were analyzed by R.N. Dorokhov and V.G. Petrukhin method. Was found that the overwhelming number of women belong to the microsomal (dimensional variation), to the microcorpulent (fat component composition), to the micro-bone (bone component composition), to the micro-muscle (muscle component composition) somatic types in all three groups. According to the proportional variation of the physical parameters, a heterogeneous distribution pattern was observed.

Abstract

A statistically significant effectiveness (18,42 vs. 2.63 %; p < 0,05) of combined treatment was established by the criterion of the frequency of detection of VAT below 5 points. Non-effective treatment, according to the VAT criterion > 5 points, was found in 30,0 % of combined and 45,0 % of patients with alpha-lipoic acid monotherapy (р < 0,05). The results obtained allow for changes in the generally accepted terms of treatment of diabetic peripheral neuropathy with alpha-lipoic acid.

CRYSTALLOGRAPHIC ANALYSIS OF ORAL FLUID IN PATIENTS WITH POST-PROSTHETIC COMPLICATIONS DURING DENTAL IMPLANTATION IN THE COURSE OF TREATMENT

Abstract

The wedge-shaped dehydration method was used to assess the state of the oral fluid of patients with post-prosthetic complications during dental implantation against the background of various treatment methods. Oral fluid (saliva) was used as a material for the study. All patients were randomized into 4 equal groups in a 1: 1: 1: 1 ratio according to the methods of pharmacotherapy. Group I - 30 patients who underwent traditional treatment of post-prosthetic complications; Group II -30 patients, ozone therapy was performed in addition to traditional therapy; Group III - 30 people who underwent transcranial electrical stimulation against the background of traditional therapy; Group IV - treatment of 30 people with the inclusion of a combination of ozone therapy and transcranial electrical stimulation in the treatment regimen. The control group consisted of healthy individuals of the same age group. Determined the nature of the organization of the structure of the salt and the distribution of the protein substance, depending on the concentration of protein in the biological fluid (saliva). Qualitative analysis of images of saliva crystallograms was carried out in the next (before treatment, 3 days, 7 days, and 14 days) observation and therapy. As the duration of treatment increased, the structuring of the crystal lattice took place, increasing in the series: patients with traditional therapy - patients with ozone therapy - patients with transcranial electrical stimulation - patients with combined use of ozone and transcranial electrical stimulation (group IV). Special attention should be paid to the fact that the complete normalization of the structure of the oral fluid facies occurred only during the complex therapy with ozone and TPP throughout the study.

THE INFLUENCE OF THE AGONIST OF RECEPTORS GPR-119 (COMPOUNDS ZB-16) ON SPERMATOGENESIS AND FERTILIZATION FUNCTION OF MALE RATS

Abstract

In experiments on outbred male beauty 4 months of age, the effect of the new compound ZB-16 (GPR 119 receptor agonist), with hypoglycemic activity [6], on spermatogenesis and fertilizing function was studied. Compound ZB-16 was administered intragastrically to animals of the experimental groups for 2 months (equivalent to one spermatogenesis cycle) in doses: 0,5 mg/kg (experimental therapeutic dose) and 100 mg/kg (higher dose than 200 times the therapeutic dose), the control group of males was intragastrically injected with a solvent of the compound - distilled water. It was found that under the influence of ZB-16, in males in both experimental groups, the spermatogenesis index is moderately increased (by 3,0 %, p > 0,05), the total number of spermatozoa (by 44,4 %, p > 0,05 and 19,2 %, p > 0,05, respectively, the 1st and 2nd experimental groups), the number of tubules with desquamated epithelium decreases, and the number of pathological forms of spermatozoa and the number of tubules with 12th stage of meiosis are not changed. According to the results of mating of the studied groups of males with intact females, a clear tendency towards an increase in the pregnancy index and a decrease in the post-implantation death of the embryos is revealed in the females relative to the control.

STUDY OF HESPERIDINE AND CURKUMIN NOOTROPIC ACTIVITY UNDER THE CONDITIONS OF FOCAL CEREBRAL ISCHEMIA

Abstract

The article presents the results of the study of the nootropic activity of hesperidin and curcumin in conditions of focal cerebral ischemia. Studies were conducted on Wistar male rats, which reproduced the focal ischemia of the right middle cerebral artery. Experimental compounds and the reference drug (Mexidol) are administered intragastrally at the next day after surgery at a dosage of 100 mg/kg for three days. Researched using memory and cognitive assessment models -the method of avoiding passive (passive avoidance reaction) and active (TEA) environments. The effect of these compounds on the level of formation of lactate, pyruvate, and the degree of necrosis was studied. Against the background of focal cerebral ischemia, hesperidin significantly improved the studied parameters, curcumin did not show a positive effect on all the studied parameters, and therefore inferior to the heotropic activity on nootropic activity. The results of experimental data suggest the possibility of further in-depth study of hesperidin for cerebroprotective and nootropic activity.

ALGORITHM FOR THE EMERGENCY BEAM DIAGNOSTICS OF A STROKE UNDER CONDITIONS OF A MODERN HOSPITAL

Abstract

The relevance of the problem of recent diagnosis of stroke has not been reduced last years. The algorithm of using radiological methods for diagnosis acute stroke is developed, but it varies depending on the technical capacities of the specialized agencies. When a patient enters a specialized hospital, a native CT scan of the brain is performed, according of its results is made a decision to perform CT angiography of the arteries of the neck and head, MR studies of the brain, CT or MR perfusion. A team of specialists works in the emergency module, which includes a neurologist in the emergency room, a radiologist in the CT room, a radiolosergion who performs a joint analysis of the results of radiation methods and decide on further treatment tactics, which leads to a significant reduction in time from the moment the patient enters the emergency room to moment of obtaining the necessary medical care. Optimization of the interaction of specialists is carried out by creating a joint chat in the WhatsApp messenger.

Abstract

Currently, miniscrews installed parallel to the roots are widely used in the practice of orthodontic treatment. When planning the placement of miniimplants, it is necessary to evaluate the anatomical factors that affect the stability of the miniscrew in the bone. For the analysis, we selected the zone of the external oblique line, where the thickness of bone tissue in the area of the first and second molars was estimated in the vertical and horizontal planes. As a result of the study, it was found that the area of the distal surface of the distal root of the second molar had the greatest thickness of bone tissue. An increase in the vestibular-oral thickness was also observed in the distal direction.

A METHOD FOR RESTORING VOICE FUNCTION OF THE LARYNX AFTER OPERATIONS FOR THYROID CANCER

Abstract

The incidence of thyroid cancer in the period from 2008 to 2018 in the Russian Federation increased from 74,8 to 114,1 cases per 100,000 population. The risk of damage to the recurrent laryngeal nerve during primary operations ranges from 0,5 to 23 %, with repeated operations increases to 62 %. The Volgograd regional clinical oncological dispensary uses the technique of intraoperative restoration of the function of the recurrent laryngeal nerve by means of a micro-neural anastomosis with the main trunk of the vagus nerve. We have experience in performing 6 similar operations. The description of this technique is given on the example of a clinical case. The described technique allows you to completely restore the voice and mobility of the vocal folds of the larynx. Respiratory function is fully restored, which makes it possible to avoid the formation of a tracheostomy in case of bilateral nerve damage, and if even unilateral paresis led to decompensated respiratory failure.

FORENSIC ASPECTS OF NON-MEDICAL CONSUMPTION OF PHENOBARBITAL IN THE TERRITORY OF VOLGOGRAD

Abstract

The analysis of the structure of mortality from the toxic effect of phenobarbital in the city of Volgograd for the period 2014-2018 was carried out. It was revealed that the mortality rate as a result of taking phenobarbital for the indicated period more than doubled, especially among the working age population (35-50 years). When comparing this indicator with a similar one for the period 1994-1998 an increase in the number of lethal poisoning with phenobarbital by 26 % was noted. It was found that taking phenobarbital is often combined with other toxic substances (ethyl alcohol - in 36 % of cases, opiates and opioids - in 11 %, benzodiazepines - in 7 %, antidepressants and neuroleptics - in 6 %, etc.) In most cases, with phenobarbital poisoning, the presence of pathological signs of secondary cardiomyopathy, chronic ischemic heart disease, ventricular fibrillation, acute alveolar emphysema, edema of the substance and membranes of the brain, toxic encephalopathy, necronephrosis was revealed. The results obtained can be considered as additional data when conducting forensic medical diagnostics of acute phenobarbital poisoning and will be useful already at the stage of studying the primary documentation, as well as during the forensic medical examination of the corpse.

ASSESSMENT OF THE PHYSICAL DEVELOPMENT OF SCHOOLCHILDREN AGED 7-8 LIVING IN ALTAI REPUBLIC

Abstract

The article presents the results of a survey of schoolchildren aged 7-8 living in the Altai Republic. Anthropometric studies of pupils in general secondary schools have been conducted and their level of physical development has been determined using the index method. The study found that the children of this age group have a low level of physical development and their type of build is defined as asthenic, more pronounced among girls. There's also a body mass deficit. Lack of physical development can be related to climatic characteristics, nutritional patterns and lifestyles.

CASE OF NEUROFIBROMATOSIS TYPE I WITH ASTROCYTOMA OF THE BRAIN IN THE PRACTICE OF PEDIATRICIAN

Abstract

The relevance of the topic is due to insufficient awareness of doctors about neurofibromatosis, which leads to a delay in diagnosis and the development of complications. The diagnostic criteria characteristic of this disease, features of the clinical course, possible complications, methods of treatment, features of dispensary observation are described. A clinical case of neurofibromatosis type I with astrocytoma of the brain is described. The diagnosis of neurofibromatosis type I (NFI) was established in a patient aged 4 years 3 months. The peculiarity of the manifestations of this clinical case is the absence of family history; the combination of cutaneous manifestations, multiple lesions of the nervous system, complicated by chronic subdural hematoma, endocrine disorders and extra-cutaneous manifestations (eye symptoms, hearing and speech disorders); the absence of significant positive dynamics despite repeated surgical treatment and long-term polyochemo-therapy. Despite the stabilization of clinical manifestations of the disease, this patient has an unfavorable prognosis with possible progression of the pathological process. For early clinical detection of patients with NFI, using for this purpose diagnostic criteria recommended by the International Committee of experts on neurofibromatosis, continuity of management and adequate measures of primary and secondary prevention of complications of NFI is important knowledge of this pathology by primary health care physicians, including pediatricians, district and family doctors, neurologists, dermatologists, ophthalmologists and surgeons.

SANITARY AND PARASITOLOGICAL CONDITION OF THE SOIL OF ASTRAKHAN REGION FOR PERIOD OF 2014-2020 YEARS

Abstract

The authors analyzed the sanitary and parasitological state of the soil of the Astrakhan region for 2014 - 6 months of 2020, using the materials of their own laboratory research and research of the center for hygiene and epidemiology in the Astrakhan region. In total, during the analyzed period, 8 144 soil samples were studied, and 16 288 studies were performed. The total number of samples that did not meet the sanitary and parasitological indicators was 6,7 %. Thus, in 2014, 16,7 % of soil samples (n = 1368) were examined for sanitar-parasitological indicators, of which 5,0 % of samples did not meet the standards. In 2015, soil studies were also conducted - 12,2 %, of which 6,4 % were unsatisfactory. Thus, in 2016, 15,1 % of soil samples were examined, of which 9,3 % were unsatisfactory. In 2017, 13,4 % of soil samples were examined, of which 7,4 % were unsatisfactory. In 2018, studies of soil samples for parasitic indicators amounted to 16,8 %, of which 7,4 % were unsatisfactory. In 2019 17,5 % of soil samples were examined - the largest number of samples studied compared to previous years. 6,5 % of the samples did not meet the standard indicators. For 6 months of 2020, 8,3 % of soil samples were examined, of which 4,2 % did not meet the sanitary and parasitological indicators. Thus, the sanitary-parasitological state of the soil in recent years continues to be relatively unfavorable, as evidenced by positive findings in the form of larvae and eggs of helminths, as well as in the form of cysts of pathogenic intestinal protozoa. The presence of Strongylid larvae and Toxocar eggs in the soil indicates contamination of this object with the feces of invasive animals (cats and/or dogs). And the presence of unfertilized Ascaris and blastocysts eggs indicates contamination of this object with the feces of infected people, or there was an accident and a break in the sewer network at this object.

ANALYSIS OF FOUR-YEAR MANAGEMENT OF PATIENTS WITH VARICOSE BLEEDING IN THE WAITING LIST FOR LIVER TRANSPLANTATION IN ROSTOV REGION

Abstract

Varicose bleeding is one of the most frequent and severe clinical manifestations of liver cirrhosis. The aim of this study was analyze of the clinical outcome of patients with varicose bleeding in the waiting list for liver transplantation (WL LT). Clinical, laboratory and instrumental data of 134 patients from WL LT were analyzed. Patients were distributed to 4 groups according to clinical outcome in WL LT: delisting, follow-up, death, orthotopic liver transplantation (OLP). Endoscopic ligation was performed in 21 patients (follow-up period from 0 to 33 months in WL LT), TlPs - 7 patients (follow-up period from 2 to 36 months.), azygoportal separation - 18 patients (follow-up period from 4 to 48 months in WL LT). The development of criteria for the priority and timeliness of measures to prevent varicose bleeding is a prerequisite for reducing mortality in the WL LT.

NFLUENCE OF THE PRODUCT AFOBAZOLE ON SEXUAL BEHAVIOR AND FERTILITY OF RATS-FEMALES

Abstract

In experiments on female rats, the effect of afobazole at doses of 5 mg/kg (group 1) and 100 mg/kg (group 2) on sexual behavior and fertility was studied. It was found that in the structure of sexual behavior in rats of females treated with afobazole with a 2-week course, the duration of sexual activity and the number of lordoses decreased statistically insignificantly and dose-dependently (by 12,5 and 37,5 %, respectively), with a simultaneous tendency to increase the number emotional approaches. In addition, according to the results of fertility studies in female rats, an increase in the pregnancy index by 14,6 % (p > 0,05) was noted in both study groups, and in pregnant women, an increase in fertility, respectively, by 33,7 % (p < 0,05) and 20,4 % (p < 0,05) and a dose-dependent decrease in the total (pre- and post-implantation) fetal death by 26,4 % (p > 0,05) and 54,5 % (p < 0,05). Based on the studies, it was concluded that afobazole had a positive effect on 2-weekly administration to rats of females at doses of 5 mg/kg and 100 mg/kg on fertility processes, with a slight tendency of its deprimative effect on sexual activity.

PARADOXAL ACTIVITY OF AMINOGUANIDINE IN THE MODEL OF GLYCOXIDATION WITH COPPER CATIONS

Abstract

The well-known inhibitor of advanced glycation-end products (AGEs) formation aminoguanidine, was studied using a previously tuned model of glycoxidation in copper-contained media (glucose 0,5 M, HEPES 0,1 M, CuSO4x5H2O 40 μM). As a result of the research, it was confirmed that aminoguanidine (1, 3, 10 mM) under conditions of glyoxidation with copper does not suppress the formation of argpyrimidine by detection of argpirimidine's self-fluorescence (Aex 330 nm, Aem 360-450 nm). Possible reasons of the enormous reaction are discussed.

ASSESSMENT OF THE DENTAL STATUS OF PREGNANT WOMEN

Abstract

A survey of 86 pregnant women was conducted in order to study the hygienic state of the oral cavity at different stages of pregnancy. It was found that 62 (72,09 %) of pregnant women were motivated to visit the dental office to maintain their own health and the health of the unborn baby, 20 women (23,25 %) were concerned about the safety of their own dental system, 4 women (4,65 %) did not understand the need to visit a dentist. The dental status of pregnant women was assessed based on dental indices: OHI-S, CPITN, PMA, and the H. R. Muhleman gum bleeding index. The dependence of the level of oral hygiene on the duration of pregnancy was revealed, at the same time, the severity of the inflammatory process did not depend on the trimester.

ANALYSIS OF CLINICAL CASES OF VENOUS THROMBOEMBOLIC COMPLICATIONS DURING PREGNANCY, CHILDBIRTH AND EARLY POSTPARTUM PERIOD

Abstract

Aim: based on clinical cases, to show an insufficient amount of measures for the prevention of venous thromboembolic complications during pregnancy, childbirth and the early postpartum period when women are managed in accordance with national guidelines. Materials and methods: the retrospective study included 25 women aged (28,1 ± 6,3) years, hospitalized during pregnancy and early postpartum period in the department of vascular surgery due to venous thrombosis of the inferior vena cava system. All patients underwent ultrasound duplex scanning of blood vessels to verify the diagnosis. The therapeutic approach to patients is considered from the standpoint of domestic and international recommendations. Results: thrombosis was most often complicated by the third trimester of pregnancy and the early postpartum period - 56 % of all cases. In the majority of patients, thrombosis was widespread and proximal: in 72 % of women, the upper border of the thrombus was localized at the level of the iliac veins. When scoring the risk of developing venous thromboembolic complications according to the 2015 Royal College of Obstetricians and Gynecologists scale in the patients participating in the study, it turned out that most of them were shown drug prevention. Conclusions: it is necessary to use national and international recommendations to prevent thrombotic complications in pregnant women and postpartum women.

LAPAROSCOPIC CORRECTION OF TYPE II ENDOLEAK AFTER AORTIC ANEURYSM RESECTION

Abstract

The article presents a clinical case of laparoscopic surgical treatment of type II endoleaks after aortic aneurysm resection on shutdown and aorto-iliac bifurcation prosthetics. This complication developed a year after the operation and was verified by CT-aortography. The growth of the aneurysmal sac up to 5,7 cm in diameter and signs of endoleak due to the right lumbar artery at the L 4 level were detected. After appropriate preparation, laparoscopic resection of the aneurysmal sac and stitching of the lumbar artery was performed. The postoperative period was uneventful. The patient was rehabilitated as soon as possible. Observation for 36 months, blood flow in the aneurysmal sac is not determined. Thus, laparoscopic techniques can be successfully used to correct complications after operations for abdominal aortic aneurysm.
