INNOVATIONS IN PHARMACOTHERAPY OF RHEUMATOID ARTHRITIS

Abstract

Up-to-date achievements in rheumatoid arthritis (RA) treatment are based on development and clinical implementation of a novel class of disease-modifying drugs which are known as biological agents. Multi-component preparations from this group contain specific monoclonal antibodies against different key molecules such as pro-inflammatory cytokines, cytokine receptors or immune cells receptors. Biological agents offer a good chance of increasing the effectiveness of RA therapy, preventing disease progression and even of controlling the process in early RA. Since biological agents can produce severe side-effects, specialists should use them with caution according to recommendations. A recent advance in the management of RA is described as well as promising investigations in this direction.

Abstract

The level of Subst P in the main group on the first postoperative day did not exceed the corresponding figures in the comparison group (Me = 0,2 ng/ml for p = 0,1). These values subsequently rose on the third day after surgery among patients in the control group (Me = 3,72 ng/ml) was significantly different from the data in the comparison group (p = 0,04).

ROLE OF INBORN IMMUNITY MECHANISM IN NON-DEVELOPING PREGNANCY PATHOGENESIS

Abstract

A retrospective analysis of regional cases of non-developing pregnancy showed a 2,3 times increase of this condition within the period from 2003 to 2010; there was also an increase in non-developing pregnancy cases among primiparas. Parameters of inborns' immunity, of cytokines and endotoxin level in blood serum of 38 female patients with non-developing pregnancy (main group) and 32 female patients with threatening miscarriage in early period before 12 weeks (control group) were studied. To evaluate these findings in physiological pregnancy, 30 women who presented for an abortion of unintended pregnancy were tested. The test results showed a stable increase in endotoxin level in blood, proinflammatory cytokines (TNF, IL-1, IL-6) and an inborn immunity mechanism activation (expression Toll 2, Toll 4) among patients with non-developing pregnancy. These data emphasize the importance of chronic infection in the pathogenesis of pregnancy complication.

An analysis of the clinical efficacy of immunotropic therapy with ultra-low doses of antibodies to human interferon γ (anaferon) is presented. A positive effect on the major clinical symptoms, severity of exacerbations, frequency of acute respiratory infections, courses of antibiotic therapy and systemic corticosteroids was revealed.

Abstract

The research included 162 patients with acute pancreatitis of various aetiology treated in the clinic of hospital surgery from 2002 to 2010. To correct the syndrome of intestinal insufficiency 94 (58 %) patients receiving standard therapy were administered presacral multicomponent novocain block. This reduced the risk of development of multiple organ dysfunction from 26 to 20,2 %, infected pancreatitis from 15,4 to 10,8 % and mortality rate from 8,8 to 5,3 %.

We studied the interrelation of manifestations of obstructive apnea syndrome in sleep with changes of the daily profile of arterial pressure and heart remodeling. We revealed a dependence between the frequency of apnea episodes in the sleep and morphofunctional parameters of the heart, types of geometry and diastolic function of the left ventricle, indicators of daily monitoring of arterial pressure.

Abstract

The result of our study was that among men of advanced age cerebral atherosclerosis was seen most often in men with picnic constitution type. A comparative analysis of morphometric parameters of hippocampus revealed the greatest variability in the length and width of the fingers and feet. Significant morphological changes in the molecular and pyramidal layers of CA1 and CA3 areas of hippocampus were detected.

LASER AND ENZYMOTHERAPY COMBINATION TREATMENT IN POSTOPERATIVE HEMOPHTHALMOS

Abstract

The study was devoted to laser and enzymotherapy combination treatment results in postoperative hemophthalmos as a complication after surgery for cataract and glaucoma. The essence of the proposed treatment consists in oral administration of trombovazim fibrinolytic followed by laser coagulation of the hemophthalmos: argon laser with wave length of 488-514 nm or Neodim: YAG laser with double frequency and wave length of 532 nm. Three-mirror Goldmann lens was effective in providing laser influence onto all departments of the vitreous body.

To predict the development of acute esophageal, gastric, duodenal erosions and ulcers in patients with acute pancreatitis we undertook a retrospective analysis of the disease in 702 patients. The predictors of acute erosions and ulcers we found give us an opportunity to prevent the development of this complication in patients with acute pancreatitis and to reduce the number of bleeding erosions and ulcers from 40,2 to 6,5 %.

PREDICTION OF THE COURSE AND OUTCOME OF DELIVERY IN PREMATURE RUPTURE OF MEMBRANES

Abstract

A survey of 90 pregnant women with premature rupture of amniotic fluid with mature pregnancy (main group) and 30 patients with timely rupture of amniotic fluid (control group) is discussed. Studies have revealed that premature rupture of amniotic fluid while the mother's body was not ready for delivery, increased hormone production and disturbance of vegetative regulation, increased production of IL-1, IL-6 and unstable personal emotional index make the prognosis of outcome delivery for the mother and fetus worse.

Abstract

New substituted derivatives of 1-(benzyloxymethyl)-5-(arylamino)uracil were synthesized by trimethylsilyl modification of Gilbert-Johnson method. The level of anti-HIV-1 activity of methylated 5-(benzylamino) derivatives depends on the site of methylation: 3,5-dimethylbenzylamino derivatives were found to be more active than corresponding α- or N-methylated isomers.

ENOXIFOL EFFECT IN MALE RATS BEHAVIOR

Abstract

The authors established a dose-dependent effect of enoxifol on behavioral and sex motivation of male rats. At a dose of 5 mg/kg enoxifol promotes a suppression of search activity and receptiveness in males while increasing grooming and proceptivity. At a dose of 60 mg/kg enoxifol increases search activity and receptiveness as well as grooming and proceptivity.

Nuchal translucency thickness and nasal bones length were prospectively evaluated in 148 fetuses with Down syndrome at 11-14 weeks of gestation. The effectiveness of prenatal ultrasound screening for Down syndrome by combination of fetal nuchal translucency thickness and nasal bone length at 11-14 weeks of gestation was 86,5 %.
