GENETIC ASPECTS OF CERVICAL INSUFFICIENCY PROBLEM

Selikhova M.S., Kostenko T.I., Karapetyan S.A.

Abstract

Cervical insufficiency is the main cause of miscarriage in the second trimester of pregnancy (up to 40 %), and in the third trimester occurs in a one third of women with preterm birth. Diagnosis of this pathology is based on the identification of structural changes in the cervix, which determines the impossibility of predicting this pathology before pregnancy. Current morphological studies have shown that connective tissue predominates in the cervix, represented mainly by collagen, which provides up to 85 % of the weight of the cervical tissue, elastin and proteoglycans. A key factor in the development of CI is currently considered a defect in collagen synthesis and the development of cervical incompetence is considered as a manifestation of undifferentiated connective tissue dysplasia. The results of recent studies show that cervical insufficiency is a genetically determined condition and polymorphisms in genes related to the metabolism of connective tissue can play a role in the development of CI.

Koshuba M.A., Petrov S.A.

Abstract

Ethnic groups of the small peoples of the North are a unique social phenomenon, as they preserve the traditional way of life laid down in age-old traditions, preserve the linguistic heritage, continue national crafts and way of managing. The interference of industrial civilization, environmental aspects, globalization processes, economic and cultural integration have their colossal destruction not only on the natural habitat and way of life of indigenous peoples, but also impose extremely high demands on the adaptive capabilities of the organism. Younger schoolchildren were examined who underwent visiocontrast perimetry in the range of spatial frequencies from 0,37 to 18 cycles / deg. It was found that for the diagnostic assessment of central vision in health and disease, it is necessary to take into account the age of the patients and climatogeographic living conditions.

Kriventsev A.E., Popkov V.S.

Abstract

Currently, in the clinical practice of a dentist-surgeon, patients with various diseases, anomalies and deformities in the oral cavity are often found. The red lichen planus (CPL) is not an exception. This paper discusses the main features of the course of the disease in the oral cavity and possible approaches to therapeutic tactics in surgical intervention. Thus, patients with CPL have edema, hyperemia, multiple or single nodular rashes on the oral mucosa (SOPR), enlarged and painful lymph nodes, and discoloration of the gums. Among the main complaints are pain and bleeding in the area of the changed SOPR, which aggravates the process of surgical intervention and creates certain difficulties for the dentist-surgeon. Due to the spread of this disease among people from 30 to 60 years old, who often require dental care, clinicians need and need to have knowledge about this pathology.

Kutepov I.V., Lyashev Y.D., Zatolokina M.A.

Abstract

Synthetic analogues of indolicidin are considered as potential antibiotics, since they have a wide spectrum of antimicrobial activity. We used synthetic analogues of indolicidin No. 7 and 8, which were used to correct experimental periodontitis in Wistar rats. Peptides were injected intraperitoneally at a dose of 500 pg/kg for 7 days after periodontitis modeling. The anti-inflammatory effect of indolicidin analogues was assessed by changes in integral hematological indices. A statistically significant effect of the studied indolicidin analogues on the neutrophil-to-monocyte ratio and the leukocyte index was established, which indicates to the effect of peptides on the macrophage-monocytic cell pool, as well as the affector and effector mechanisms of the humoral immune response.

RANGE OF DIRECT-ACTING ANTI-VIRAL DRUGS FOR TREATMENT OF CHRONIC HEPATITIS C LISTED BY THE WORLD HEALTH ORGANIZATION, THE RUSSIAN FEDERATION AND THE VOLGOGRAD REGION

Ganicheva L.M., Panfilova O.A., Voronova A.V.

Abstract

Viral hepatitis is an international public health threat. With the adoption of the UN Assembly Resolution in 2016, the problem of hepatitis no longer remains in the shadows. An important step towards eradicating chronic hepatitis C (CHC) is the introduction of direct-acting antiviral drugs into treatment standards. They appeared in the WHO List of Essential Medicines for the first time since 2014. On the territory of the Russian Federation, the number of registered international nonproprietary names increased in 2014-2020 by 3.5 times and at the time of 2020 it was 10. Direct-acting antiviral drugs were included in the Vital and most Essential Drugs List since 2017 (10 %), and by 2020 this indicator had increased by 7 times. In general, in the Russian Federation since 2017, there has been progress in solving a global problem through the use of more effective drugs, which cannot be said about the regions of our country. So, in the Volgograd region, not a single Direct-acting antiviral drugs of preferential leave for the treatment of CHC on an outpatient basis has been registered.

COLORIMETRIC ANALYSIS IN ESTIMATING THE EFFICIENCY OF TRABECULOCLINIG

Dzhashi B.G., Balalin S.V.

Abstract

Trabeculoclining is usually called the aspiration of debris during phacosurgery, which is of fundamental importance in cataracts associated with open-angle glaucoma and pseudoexfoliative syndrome. One of the methods for objectively assessing the results of this surgical manipulation is the colorimetric analysis of the structures of the anterior chamber angle. We examined 110 patients with the initial stage of primary open-angle glaucoma (110 eyes). The aim of this study was to assess the state of the anterior chamber angle after trabeculoclining based on the colorimetric analysis. The change in the color state of the structures of the anterior chamber angle after trabeculoclining was investigated. Photo registration of zones was carried out with subsequent colorimetric analysis. The change in the average color of the Schlemm canal is clearly shown. The method confirmed the effectiveness of trabeculoclining in terms of trabecular debris aspiration and showed a reliable clarification of the photorecorded structures.

PREVALEN CE OF CALCAN EAL SPU RS IN PATIENTS WITH FLATFOOT DEFORMITY

Zhulikov A.L., Malanin D.A., Abuzyarova Y.N., Finageev S.A., Shkoda D.A.

Abstract

The article focused on the problems of calcaneal spur development, diagnosis and its treatment options. Plantar calcaneal pain (calcaneal spur) is caused by proximal plantar fasciitis induced by degenerative and dystrophic changes of plantar fascia at the site of its attachment to the calcaneal bone, which is seen in young and middle-aged patients. The prevalence of calcaneal spurs in patients of different ages with flatfoot deformities is analyzed in the article. The study involved 411 patients aged 20-74, including 150 males (36,5 %) and 261 females (63,5 %). Flatfeet were revealed in 398 patients (96,8 %). Calcaneal spurs were detected in 6 % of patients with flatfeet. In males and females aged 20-44 this indicator was 12,5 % and 16,6 %, respectively, which is slightly lower than that shown by official statistics. The patients with calcaneal spurs participated in a survey, which employed the FHSQ scale comprising 13 questions divided into 4 groups: foot pain (4 questions), foot function (4 questions), footwear (3 questions) and general condition of feet (2 questions). 8 people (33,3 %) agreed to participate in the survey, 15 patients (67,7 %) refused to participate for no valid reasons. Out of 24 patients with calcaneal spurs, 6 (25 %) patients reported enhanced quality of life, 2 (8 %) reported no changes. Therefore, we may conclude that foot orthotics using individual corrective insoles and optimization of the functional foot load on feet and overlying portions of the locomotor apparatus play an important role in reaching positive outcomes when treating plantar fasciitis.

CEREBROPROTECTIVE ACTIVITY OF NOVEL ADAMANTANE DE RIVATIVE OF 2-PYRROLIDONE IN RATS WITH BRAIN ISCHEMIA

Kovalev N.S., Bakulin D.A., Kurkin D.V., Dubrovina M.A., Tarasov A.S., Klimochkin Y.N., Tkachenko I.M.

Abstract

The cerebroprotective effect of novel adamantane derivative of 2-pyrrolidone in rats with brain ischemia was studied. Pathology was modeled by irreversible occlusion of the common carotid arteries. In rats with experimental cerebral ischemia, there was a significant impairment of coordination of movements and sensory-motor functions: it was more difficult for animals to stay on the rotating rod, as well as in the «adhesive removal test» to notice and get rid of the sticker on the volar surface of the forepaws. In the «new object recognition» test, the animals of the control group showed impaired working memory. Course administration of the investigated derivative of 2-pyrrolidone and citicoline contributed to a significant correction of motor and cognitive impairments.

Kurkin D.V., Bakulin D.A., Verholyak D.V., Kovalev N.S., Abrosimova E.E., Dubrovina M.A., Ostroglyad E.S.

Abstract

The effect of a new hydrazide derivative of 2-pyrrolidone on neuropsychiatrie deficiency in animals with chronic cerebrovascular insufficiency after course administration was studied. The pathology was modeled by narrowing the lumen of the common carotid arteries to 50 %. The test substances began to be administered from the 40th day and continued for 14 days. The effectiveness of therapy was assessed by the severity of motor, coordination and sensory-motor disorders in animals, which were respectively determined in the tests: Open Field, Rotarod and Adhesive. Stenosis of the common carotid arteries was accompanied by impaired coordination and sensory-motor function. A new derivative of alpha-pyrrolidone improved coordination and sensory function in experimental animals. Cerebroprotective activity of the new derivative of 2-pyrrolidone was comparable to the reference drugs phenotropil and citicoline

INFLUENCE OF SPELEOCLIMATOTHERAPY ON BACKGROUND EEG ACTIVITY OF THE HUMAN BRAIN

Semiletova V.A., Dorokhov E.V., Nechaeva M.S.

Abstract

It was revealed that in a state of functional rest with open and closed eyes in patients after exposure to electronegative air ions, the voltage in the central nervous system decreased. At the same time, the amplitude of theta rhythm with the state of «open eyes» tended to decrease both on the right and on the left in the parietal, temporal (T5 and T6), frontal (F8, F7) leads, but in leads T3 and T4, F3 and F4 - to increase; and in the central and occipital areas, the dynamics of the theta rhythm amplitude was multidirectional. The amplitude of theta rhythm with the «closed eyes» state had interhemispheric differences - a tendency to increase in leads O1, P3, C3, F3, T3. The amplitude of the alpha rhythm in the «open eyes» state increased both on the right and on the left in all the studied leads. The amplitude of the alpha rhythm with the «closed eyes» state had a tendency to increase in all the studied leads. The amplitude of the beta rhythm with the state of «open eyes» tended to increase in the occipital and temporal (T3, T4) areas; in the parietal leads P3 and P4 - had a tendency to decrease; and practically did not change in the central and frontal areas. The amplitude of the beta rhythm with the «closed eyes» state had a tendency to increase in almost all leads. Such changes could reflect effective rest, the readiness of patients for vigorous activity, testify to the restoration of the efficiency and mental activity of the brain. It could also be associated with a decrease in tension in the regulatory systems under the influence of a session of speleotherapy, with simultaneous active mental activity, analysis of the situation and decision-making.

Arakelyan R.S., Bogdanyants M.V., Shendo G.L., Hasanova E.D., Krasiy K.A., Krasilova A.V., Chanpalova L.S., Tveritinov E.B., Korolkova V.S.

Abstract

The purpose of the study. To study and analyze the epidemiological situation of malaria in children in the Astrakhan region for 1996-2020. Materials and methods. Epidemiological maps of individuals malaria cases (n'164 epidcards), reporting forms of FBHI «Center for Hygiene and Epidemiology in Astrakhan Region» (form 2) for 1998-2020 (25 documents), the journal of the population survey on malaria (18 journals), as well as microreparates with malaria «fat» and «thin ointment» (82 drugs). Results of the study. The largest number of cases of malaria were reported in 1999 and 2000. In these years, cases of three-day malaria were recorded, except in 2000, when in 1 case four-day malaria was recorded. In the previous 2008 and other years, only three-day malaria cases were reported. Most often, malaria was registered in children aged 2, 13 and 15 years. Thus, in children aged 2 and 15 years, malaria was registered in 12,2 % (5 people each), and at the age of 13 years - in 17,1 % (7 people). In children of other ages, cases of malaria were recorded in isolated cases. In most cases, imported cases of malaria were recorded -90,2 % (37 people). The import of malaria to the Astrakhan region was mainly carried out from Azerbaijan - 86,5 % (32 people), also from Tajikistan - 8,1 % (3 people), Armenia and Uzbekistan - 2,7 % each (1 person). In all cases, only three-day malaria was recorded. The clinical diagnosis of «Malaria» was made on the basis of a laboratory blood test - the preparations «thick» drop and «thin» smear and confirmed by employees of the Laboratory of Bacteriological and parasitological studies of the Federal Budgetary Healthcare Institution «Center for Hygiene and Epidemiology in the Astrakhan region». Conclusions. In recent years, the Astrakhan region has seen a decrease in the number of cases of imported malaria not only among children, but also among the adult population. Malaria was imported to the Astrakhan region mainly from Azerbaijan. Cases of malaria in children were reported most often from April to July, when the optimal temperature for malaria transmission is maintained in malaria-endemic countries. Detection of a case of malaria in a child in March 2008 indicates a long incubation of P. vivax (+24-25 °C for 10-11 days) in the fall of 2007.

Maltsev D.V., Spasov A.A., Miroshnikov M.V., Skripka M.O., Dementenko Y.Y., Chernikova E.M.

Abstract

For the substance under the code DAB-21, a pronounced neuropsychotropic potential was previously shown. In this study, possible directions of the influence of the studied substance on basic mediator systems of the CNS are considered. According to the results of the study, a benzodiazepine-like effect of DAB-21 was found in the «Elevated plus maze» test with flumazenil, as well as potentiation of the effects of phenamine, and some serotoninergic effect in the test with 5-GTP, and a weak a2-adrenoblocking effect, revealed using the «Clofelinic hypothermia» test. There was also a D2-activating effect according to the haloperidol test, a weak M-anticholinergic effect in the arecoline test. Some MAO-inhibitory effect was found, suggested by antagonism with reserpine, as well as the results of the L-DOPA test. The compound did not interact with nicotine and apomorphine. The neurotransmitter profile of the compound DAB-21 is of interest for further study and clarification of the mechanism of action of the substance.

Morozov E.A., Panin S.I., Kushniruk N.E., Kuznetsov A.A., Goncharov N.A., Doronin A.B., Linchenko D.V.

Abstract

The aim of the research is to evaluate the results of trapezoidal perineostomy in the treatment of patients with extended and recurrent strictures of the anterior urethra. The study included 5 males with an average age of 62 years. Among the etiological factors: urethrectomy and penectomy (n = 3), inflammatory diseases of the urethra (n = 1), iatrogenic damage to the urethra (n = 1). The duration of the operation is 60 minutes. No complications were observed in the immediate postoperative period. The average bed-day in the hospital was 4, postoperative - 3 days. When assessing long-term results and quality of life using questionnaire scales, the following results were obtained: IPSS - 5 (3-7) points and QoL - 1 (1-2) points. Thus, perineal urethrostomy requires more extensive and detailed analysis in studies with high statistical power.

Otto N.Y., Bezrukova D.A., Dzhumagaziev A.A., Bogdanyants M.V., Petrova N.V.

Abstract

The article describes clinical examples of growth retardation in children and adolescents of various origins, taken from the case histories of patients who were examined in the endocrinology department of the Regional Children's Clinical Hospital in the city of Astrakhan from 1994 to 2017. These clinical cases are a striking example of the variety of causes of growth retardation in children. It is necessary to direct pediatricians and other specialists to an earlier start of examination of children with short stature, since not only the prognosis of the patient's final growth, but also the patient's life, may depend on timely differential diagnosis and the correctly chosen therapeutic tactics.

STUDY OF SOME ASPETS NEUROPROTECTIVE ACTIVITY OF PHENYLACETIC ACID AMIDES UNDER CONDITIONS OF FOCAL BRAIN ISCHEMIA

Chernikov M.V., Sosnovskaya A.V., Pozdnyakov D.I.

Abstract

The article presents the results of studying the neuroprotective activity of carboxylic acid diamides under conditions of focal cerebral ischemia. The study was carried out on male Wistar rats, which underwent focal ischemia by irreversible occlusion of the right middle cerebral artery. The compounds under study under the codes FacAc, FacPr, FisBut, Faib, the reference drug piracetam were injected intragastrically on the next day after the operation at a dose of 100 mg/kg and then for three days. The neuroprotective activity of the compounds was studied using models for assessing memory and cognitive functions -the technique of avoiding passive (CPAR) and active (EMT) environments, and also studied the effect of compounds on the size of the necrosis zone. In the course of the study, it was found that, against the background of focal cerebral ischemia, the use of diamides of carboxylic acids contributed to the preservation of the memory trace in rats, as well as the necrosis zone. The results of experimental data indicate the possibility of further in-depth study of the compounds under study under laboratory codes FacAc, FacPr, FisBut, Faib for neuroprotective activity.

PECULIARITIES OF CHANGE IN HEMODYNAMIC INDICATORS IN PERSONS WITH DIFFERENT TYPES OF VEGETATIVE NERVOUS SYSTEM AT DENTAL RECEPTION

Starikova I.V., Piterskaya N.V., Chaplieva E.M., Bobrov D.S.

Abstract

In the dental clinic GAUZ KSP No. 10, Volgograd examined 80 patients who applied for dental care at the age of 25-30 years. Before and after the dental appointment, blood pressure - systolic (SD), diastolic (DD), pulse rate (HR), and respiratory rate (RR) were measured. The indirect method of Star (1954) was used to calculate the minute volume of blood circulation (MVC), according to the formula of F. Poiseuille (1839), peripheral vascular resistance (PSS) was determined, according to the formula of N.N. Savitsky (1974) calculated the average hemodynamic pressure of the SRS. The vegetative status was calculated according to the values of the Kerdo vegetative index (VI). The patients were divided into 3 groups, depending on the type of ANS, sympathotonics in 36 patients (including 25 men and 11 women), normotonics 18 people (8 men and 10 women), vagotonics 26 people (5 men and 21 women). During a dental appointment, patients with a symptomatic type of autonomic regulation had high blood pressure (SD, DD), PD, and heart rate, IOC, SRS. With the vagotonic and normotonic type of autonomic regulation, DM and DD were within the normal range, but before taking the numbers were higher than after it. Analysis of hemodynamic parameters in patients during a visit to the dentist made it possible to identify the features of regulation and mechanisms of tension of adaptive capabilities depending on the type of the autonomic nervous system.
