INFANTILE HEMANGIOMA OF THE SUBORBITAL AND POSTERIOR REGION. CLINICAL CASE

Makedonova Y.A., Poroyskiy S.V., Djumahanova A.J., Gamayunova A.A.

Abstract

Infantile hemangioma is one of the common benign vascular tumors found in infants. This tumor can undergo reverse development after the initial proliferation, so many doctors consider pharmacotherapy in this case impractical. It should be noted that some hemangiomas are accompanied by functional disorders, scars and painful on palpation. Therefore, it is important for the Clinician to understand the etiopathogenetic factors contributing to the development of tumors, morphological features, stages of development of this pathology when choosing methods and means of pharmacotherapy. This paper describes a clinical case of infantile hemangioma in a three-month-old child in the suborbital and posterior regions. Despite its benign nature, hemangiomas had features of a clinically malignant course. Despite the possibility of self-healing and stopping the growth of hemangioma with subsequent involution, its further course still remains unpredictable, which justifies the need for dispensary observation of the patient.

Gomon M.S., Mantulina L.A., Zatolokina M.A., Lipatov V.A.

Abstract

The research purpose was to analyze images of computed tomography of the chest to study the size of the normal thymus in children and adolescents and to identify differences taking into account age and gender characteristics. Materials and methods: CT images of chest organs of 100 patients aged 3 to 18 years, performed on the basis of the Regional Clinical Tuberculosis Dispensary Public Health Institution of the Kursk Region Health Committee using the RadiAnt DICOM Viewer program, were retrospectively studied. For morphometry, we proposed a thymic-thoracic index. Results and discussion: the obtained dimensional characteristics of the thymus have differences in comparison with published data. In particular, at the age of 11-18 years, in women, the size of the gland is smaller than in the younger age group (3-10 years), while, according to the literature, the maximum size of the thymus is in the puberty period. Conclusions: the morphometric differences in the dimensional characteristics of the thymus revealed in this study indicate the need to study this organ in sexual and age aspects, with a view to their further use in the clinic.

A MODIFIED METHOD FOR STUDYING THE GLYCOXIDATION REACTION

Litvinov R.A., Kosolapov V.A., Muravyova E.A., Skachko I.V., Shamshina D.D.

Abstract

The article considers a particular model of the glycation reaction, called glycoxidation and characterized by a significant involvement of oxidative reactions and the participation of transition metals. The enhancement of oxidative processes, which makes it possible to identify the reaction as glyoxidation, was realized by introducing copper (II) cations (CuSO4 * 5H2O) into the reaction medium in a form accessible for interaction with the reaction participants. The main reagents in the glycoxidation reaction, in addition to CuSO4*5H2O are bovine serum albumin (1 g/l), glucose (0,5 M), HEPES buffer solution (24 g/l) dissolved in deionized water. The conditions have been selected under which the glycoxidation reaction linearly depends on the concentration of copper (II), which makes the cation activity a sensitive target when monitoring the reaction. The dependence of the reaction on the concentration of CuSO4 * 5H2O under the presented experimental conditions has been confirmed experimentally and mathematically. An analytical range of concentrations of CuSO4*5H2O has been established, in which a linear dependence of AGE formation on concentration is observed. For the concentration of CuSO4*5H2O, which produces the greatest potentiating effect on the AGE formation (10 mg/l), the procedure was validated by the indices of internal laboratory reproducibility and convergence. The technique is of interest for investigation of the antiglycoxidative activity of novel compounds

CLINICAL AND LABORATORY ANALYSIS OF THE EFFECTIVENESS OF TREATMENT OF DEEP CARIES WITH CALCIUM-CONTAINING PADS

Makedonova Y.A., Poroisky S.V., Gavrikova L.M., Venskel E.V.

Abstract

The task of a dentist in the treatment of deep caries (pulp hyperemia) is not only to restore the integrity of the tooth, but also to stimulate the production of replacement dentin with the help of a medical pad, to ensure reliable isolation of the pulp from the toxic effects of materials. Having a wide range of different therapeutic pads, but not having methods to objectively assess the condition of the pulp, doctors have to «blindly» give preference to a particular manufacturer. In this work, based on a clinical and laboratory study, an analysis of the effectiveness of calcium-containing pads «Life» and «Dycal» - was performed. The clinical stage was performed on patients in the treatment of deep caries. Changes in the pulp microcirculation were evaluated using laser Doppler flowmetry. During histological examination, samples of tooth cuts were studied for analysis of micro-flows of the dye at the «seal-tooth» border and the depth of penetration of the dye. The clinical results are consistent with laboratory results, which can be used to draw conclusions about the benefits of the «Life» treatment pad in the treatment of deep caries.

MODERN METHODS OF DIAGNOSIS OF POST PROSTHETIC COMPLICATIONS DURING DENTAL IMPLANTATION

Mikhalchenko D.V., Makedonova Y.A., Salyamov H.Y.

Abstract

The number of dental implants performed at the dental reception is steadily increasing. Along with this increased and the frequency of occurrence postprostatectomy complications in dental implantation. One of the main tasks of a dentist is early detection of the initial signs of inflammation of the parotid tissue, leading to mucositis. If you do not act on the site of inflammation in a timely manner, the destruction of bone tissue around the implant will begin with the development of peri-implantitis. In this work, we conducted a retrospective analysis of the frequency of occurrence of mucositis and periimplantitis. An algorithm for diagnostic measures has been developed. The patients were determined by the hygienic index, the iodine number of Svrakov, the degree of bleeding, and the depth of probing of the dental sulcus. The implant stabilization was determined using amplitude-frequency analysis. A detailed periodontal screening was performed to assess the condition of the gums and identify pathological processes occurring in the tissue surrounding the implants. The condition of the cortical plate and parotid bone tissue was evaluated radiologically. The developed algorithm for diagnostic examination of patients with postprotetic complications during dental implantation helps to detect inflammation in an early period, which in turn allows to preserve the integrity of the tooth.

APPLICATION OF OZONE THERAPY IN THE TREATMENT OF DENTAL IMPLANTATION COMPLICATIONS

Mikhalchenko D.V., Salyamov H.Y., Makedonova Y.A., Mikhalchenko A.V.

Abstract

The percentage of implant treatment failures is steadily increasing. This may be due to both General and local factors that lead to the development of parotid tissue inflammation. In this case, one of the signs of development postprostatectomy complications following dental implantation is bleeding, redness. Earlier detection of inflammatory phenomena of peri-g raft tissue may contribute to the preservation of the tooth and the chewing function of the dentition. Currently, there is no single approach to the treatment of perimplantitis and mucositis. Traditional treatment is based on the use of anti-inflammatory gels and ointments. In this paper, a comparative analysis of the effectiveness of ozone treatment relative to the standard treatment regimen is performed. During 14 days, an examination and instrumental examination were performed to determine the hygienic indices, the bleeding index, the Svrakov iodine number, and periodontal screening. The implant stability coefficient was determined using the Osstel Mentor device. In all groups, there was a positive tendency to subside inflammatory phenomena, and in some cases, there was a complete cessation of the process, but the nature and timing were different depending on the methods of exposure to the pathological focus.

An early and late results of endovascular prosthetic repair of the aorta were described. For 9 years, 71 patients with infra-renal aortic aneurysms underwent endovascular aortic prosthetics. In the early postoperative period, type II endoleaks were registered in 4,2 % of cases, and endoprosthesis branch thrombosis in 1,4 %. Systemic complications accounted for 2,8 %, with no hospital fatality. In the long-term period, the primary patency of shunts was 91,18 %. Endovascular aortic prosthetics is a safe and effective method of treating infra-renal aortic aneurysms, which demonstrates a low level of complications and mortality in the early postoperative period.

INFLUENCE OF SPELEOCLIMATOTHERAPY ON THE PSYCHO-EMOTIONAL STATE, GENETIC STABILITY AND ACTIVITY OF а-AMYLASE IN THE ORAL CAVITY OF THE STUDENTS

Nechaeva M.S., Tyunina O.I., Dorokhov E.V., Aralova A.Y., Potapova Y.O.

Abstract

The goal of investigation was to evaluate the effect of speleoclimate on the psycho-emotional state, the frequency of occurrence of buccal epithelial cells with core aberrations and the level of а-amylase activity in the oral cavity of students. Second-year students of VSMU named N. N. Burdenko were selected as subjects. The collection of psychological parameters, buccal epithelium, to assess genetic stability, and oral fluid, for evaluation of the activity of а-amylase was carried out in the first, fourth, eighth, eleventh day visit speleochamber and four days after a course of speleoclimate. The psycho-emotional state of the subjects was assessed using the SAN questionnaire and the scale of C.D. Spielberger and Y.L. Khanin. The state of genetic homeostasis of students was evaluated using a micronucleus test in the buccal epithelium. The level of activity of а-amylase was carried out by the method of solid-phase enzyme immunoassay. Comparison of the studied indicators was carried out using the nonparametric Van-der-Warden criterion. The relationship between the dynamics of а-amylase activity and cytological parameters was revealed using the Spearman correlation coefficient (rs). The results obtained indicate a positive effect of speleoclimate on the psychological and genetic stability of a person. Subjects with low а-amylase activity showed a higher level of cells with abnormal nuclei. A negative correlation was found between the dynamics of the frequency of abnormal nuclei in buccal epithelial cells and the activity of а-amylase in the group with initially low activity (rs= - 0,8; P < 0,05). Speleoclimatotherapy can be recommended as a method of non-medicamental correction of the effects of stress influence on health of students.

Polyantsev A.A., Kuznetsov A.A., Goncharov N.A., Kiseleva A.A., Morozov E.A.

Abstract

The retrospective analysis of results of urethral strictural disease surgical treatment cases (according to the materials of the urological department of the «Volgograd Regional Clinical Hospital № 1» for 2015-2020) was provided. The research included 168 surgical treatment cases of urethral strictures of different localization, longitude and etiology. The patients were operated using different surgical technics. Comparative analysis of results of surgical treatment in patients with urethral strictures was provided. The research evaluates the efficacy of used methods: internal optical urethrotomy, anastomotic plastics «edge to edge», buccal urethroplastics, urethral plastics without spongious body incisoin. It was found out that now-a-days the most prognostically favourable methods in urethral stictural disease surgical treatment are the buccal plastics and «edge to edge». Otherwise, due to few observed cases, the plastics without spongious body incision demands more detailed and thorough analysis for verifying it's comparative efficacy.

Sirak A.G., Piskareva E.I., Dolgashova M.A., Magomedova O.G., Arutyunova A.P., Lyubanskaya O.V., Nemenuschaya E.G., Didenko M.O., Kochkarova Z.M.

Abstract

The article is devoted to the functional state of blood leukocytes and peculiarities of hematopoiesis during inflammation under experimental conditions against the background of local removal of non-phagocytic granulocytes. The functional status of blood leukocytes obtained from 24 Wistar male rats weighing 180-220 g was judged by the activity of their marker enzymes. Myeloperoxidase (MPO) and acid phosphatase (CF), macrophage monocytes - а-naphthylacetate esterase (а-NAE), lymphocytes - CF and а-NAE served as markers of the functional activity of neutrophils. Found that during inflammation on the background of prior removal of NG from the focus, there is less neutrophil and monocyte entry from the bone marrow into the blood and further into the inflammation focus, changes in the intensity and dynamics of hemopoiesis, and as a result, increased lymphocyte degranulation.

EFFECT OF THERAPY WITH A FIXED COMBINATION OF PERINDOPRIL AND AMLODIPINE ON VASCULAR AGE AND 5-YEAR RISK OF CARDIOVASCULAR COMPLICATIONS IN PATIENTS WITH ARTERIAL HYPERTENSION AND MILLITUS DIABETES TYPE 2

Statsenko M.E., Turkina S.V., Skibitsky V.V., Derevyanchenko M.V., Titarenko M.N.

Abstract

In recent years, the concept of cardiovascular risk factors, one of which is the process of vascular aging, has been of great interest. 30 patients with stage 2-3 of arterial hypertension and type 2 diabetes mellitus aged 45 to 65 years were included, including 23 women and 7 men. Patients after the washout period a fixed combination of perindopril with amlodipine (the original drug Prestans, AO Servier) in the form of tablets (perindopril arginine / amlodipine bezilat) were prescribed for 12 weeks, 1 time per day. Vascular age and 5-year risk of developing cardiovascular complications were calculated on the calculator app for smartphones ADVANTAGE AGE (Les laboratories Servier version 2, 2015). After 12-week therapy with a fixed combination of perindopril and amlodipine, the achievement of target blood pressure values was noted in 100 % of patients. The average passport age of the patients included in the study was (61,9 ± 4,4) [45; 65] years. In assessment vascular age, there was a statistically significant initial increase in vascular age above the passport age - (69,3 ± 8,5) [55; 89] years. 46,7 % of patients have an average risk of developing cardiovascular complications and 53,3 % of patients have a high risk of cardiovascular accidents. After 12 weeks of therapy with arginine perindopril and amlodipine bezilate, there was a significant decrease of vascular age to 63,1 ± 5,6 [51; 73] years, a decrease in the percentage of patients with high risk of cardiovascular complications to 23,4 % (p < 0,05), due to patients with average risk (53,3 %, at p < 0,05), and the appearance of patients (3,3 %) with a low risk of cardiovascular accidents within 5 years. In patients with arterial hypertension and diabetes millitus type 2, vascular age significantly increases in comparison with the passport age. The use of a fixed combination of perindopril arginine / amlodipine bezilate for 12 weeks in this category of patients significantly reduces the vascular age of patients and reduces the frequency of detection of patients with high and medium risk of cardiovascular accidents.

Suchilin I.A., Malanin D.A., Volodin I.V., Julikov A.L.

Abstract

Complete traumatic injuries of the anterior cruciate ligament (ACL) are the most common knee injury among physically active males and females who are fond of both traditional and extreme sports. Surgical restoration of the anatomical structures of the injured joint, even with isolated ACL damage, remains an urgent problem of modern traumatology and orthopedics. The technology of arthroscopic reconstruction of the ACL is developing along the path of improving the technique of surgery, fixators, plastic materials. The article is devoted to the study of the results of arthroscopic anatomical plastic surgery of the anterior cruciate ligament with autografts from the patellar ligament with two bone blocks and the hamstring flexor tendons. The study included 100 patients of the main group and the comparison group with chronic damage to the anterior cruciate ligament and chronic anterior instability of the knee joint, which were divided into two groups depending on the type of transplant used. Long-standing injuries of the anterior cruciate ligament were diagnosed based on the results of a clinical examination, radiography, magnetic resonance imaging, and hardware arthrometry. Excellent and good results of surgical treatment were obtained in most patients of both groups, however, the highest stabilization indicators of the knee joint were noted in the comparison group.

MODERN TERMINOLOGY, PATHOGENESIS, DIAGNOSTICS AND TREATMENT OF CHRONIC ABDOMINAL SYNDROME IN CHILDREN

Khvorostov I.N., Sinizyn A.G., Kopan G.A., Damirov O.N., Andreev D.A., Verbin O.I., Shramko V.N.

Abstract

The terminology of chronic abdominal pain, as well as further treatment tactics, are still the subject of debate. Conservative treatment in some patients with chronic abdominal pain is ineffective. Laparoscopic appendectomy is a scientifically based surgical intervention for patients with chronic abdominal syndrome with localization in the right iliac region in children who do not show peritoneal signs and changes in laboratory parameters, with the exception of intercurrent pathology by non-invasive methods.
